Causes of entanglement hazards

Domain 1F asks you to name why a child strangles on playground equipment, not merely to wave a gauge. CPSC 2025 §1.8 defines entanglement as a condition in which the user’s clothes or something around the user’s neck becomes caught or entwined on a component of playground equipment. That is a catch-and-cinch mechanism. It is not a bruise from running into a bolt, and it is not a head stuck in a ladder opening.

Entanglement is not protrusion and is not entrapment

CPSC §1.8 also defines a projection as anything that extends outward from a surface and must be tested to determine whether it is a protrusion, an entanglement hazard, or both. Why slides dominate entanglement deaths

CPSC 2025 §5.3.6.7 states that children have suffered serious injuries and died by getting parts of their clothing tangled on protrusions or gaps on slides. Two physical facts make the slide the worst foreseeable path:

  • Motion plus incline. Once a hood loop, drawstring toggle, or helmet strap catches, the child’s mass continues down the chute. The fabric cinches at the neck while the inspector’s “small” projection stays fixed. The child cannot stand up to unload the cord the way a standing climber sometimes can.
  • Predictable catch points. CPSC §5.3.6.2 requires the platform-to-chute joint to have no spaces or gaps that could trap strings, clothing, or body parts. CPSC §5.3.6.7 adds that projections up to 3 in in diameter should not stick up more than 1/8 in (0.12 in) from the slide, and that there should be no gaps at the top of the chute where it meets the platform. Appendix B.2.3 tells you to walk the shaded slide path — bedway, sidewalls, and the entrance — with the projection gauges and the 1/8 in slide projection gauge. CPSC §5.3.6.8 extends the same entanglement rules to any equipment whose primary foreseeable use is sliding.

Classic equipment causes you must be able to name

Memorize this list. Each item is a cause, a citation, and a dimension from the NRPA Table of Dimensions (Rev 1/2024) or CPSC 2025.

  • Open S-hooks and C-hooks. CPSC §3.2 and §5.3.8.1: a hook is closed only if there is no gap or space greater than 0.04 in, about the thickness of a dime (NRPA Table: 0.04 in / 1 mm). Open hooks on swing seats and overhead hangers catch hoods, drawstrings, and straps as the occupant moves. A connecting device whose in-fill completely fills the interior space so clothing cannot enter is exempt from that gap rule — the exemption is about a filled interior, not about “stainless looks closed.”
  • Bolts and fasteners that can catch a drawstring. CPSC §3.2: bolts should not expose more than two threads beyond the nut. A long bolt end on a slide sidewall, swing seat, or moving member is an entanglement cause even if it also fails as a protrusion.
  • Increasing-diameter projections. CPSC §3.2 and Figure 2: the diameter of a projection should not increase in the direction away from the surrounding surface toward the exposed end. A mushroomed cap, a stacked washer that widens outboard, or a decorative knob that gets bigger as it leaves the panel can hold a loop that a straight-sided stud might release.
  • Projections in slide bedways and along slide sidewalls. CPSC §5.3.6.7 and Appendix B.2.3. Hardware, cracked plastic ridges, and joint offsets in the travel path are high-likelihood catch points.

  • Strings, straps, jump ropes, dog leashes, and other objects tied onto equipment by users. CPSC §3.2.1 and Table 3 (routine inspection): remove them. Children have died in ropes and leashes that were never part of the manufacturer’s design. CPSC §4.1 lists “user modifications (such as ropes tied to parts)” as a standing inspection item.

Non-rigid components that run between two play units are a related suspended-hazard family: NRPA Table requires a minimum height of 84 in above the surfacing and a minimum diameter of 1 in. CPSC §3.5 uses the same 7 ft / 1 in relief so a taut, thick, high cable is treated as a visibility/impact problem rather than a neck-wrap. Do not confuse that relief with permission to leave a skinny, low slack rope in place.

Clothing, helmets, and 16 C.F.R. §1120.3

CPSC 2025 §3.2.1 retitled the subsection Strings, straps, and ropes and states that cords or straps on clothing or helmets can become entangled and cause death by strangulation. The handbook’s own avoidance list is operational, not optional coloring:

  • Children should never wear jewelry, neck scarves, jackets or sweatshirts with drawstring hoods, mittens connected by strings through the arms, or other upper-body clothing with drawstrings.
  • Helmets with chin straps should never be worn on playground equipment; the strap is a ready-made loop and has also created head-entrapment incidents in openings that would otherwise pass (CPSC §3.3.1).
  • Remove ropes, dog leashes, or similar objects attached to equipment.
  • Avoid equipment with ropes that are not secured at both ends.
  • One of the handbook’s warning labels (or a similar sign) may be placed on or near slides: children have strangled when items worn around the neck caught on equipment; keep clothing drawstrings, jump ropes, and helmets away; always watch the child.

CPSC 2025 §1.7 footnote 6 states that drawstrings on children’s clothing are prohibited per 16 C.F.R. §1120.3. That rule is a clothing substantial-product-hazard listing, not a playground-equipment design statute.
