15.2 Corrective Action & Maintenance Methods

Domain 4E is the difference between a clipboard and a safer playground. The CPSC Public Playground Safety Handbook, Publication 325 (July 2025) §4.1 tells the owner to build a comprehensive maintenance program and to inspect systematically for wear, deterioration, and the hazards in Table 3. §4.2 Repairs then delivers the sentence the exam will quote: inspections alone do not constitute a comprehensive maintenance program. Any problem found during the inspection should be noted and fixed as soon as possible. A signed form with an open Priority 1 and no work is not maintenance. It is a dated admission that the owner knew.

What CPSC §4.2 actually requires

Read the bullets, not the heading.

  • All repairs and replacements of equipment parts should be completed following the manufacturer’s instructions. A hardware-store bolt that is “close enough,” a welded homemade plate that changes the geometry of a hanger, or a substitute slide bed that the manufacturer never listed is not a §4.2 repair. If there are no manufacturer instructions — common on homemade or unlabeled imports from Domain 4D — the owner still owes a repair that meets current F1487-21 general performance and CPSC hazard rules, not a creative shop project.
  • User modifications, such as loose-ended ropes tied to elevated parts, should be removed immediately. CPSC Table 3 and Appendix A put the same item on the routine list: ropes tied to parts, equipment rearranged, swings looped over top rails. A jump rope on a top rail is an entanglement and strangulation path (CPSC §3.2.1). It is not décor. It comes off before the next recess, not at the next capital meeting.
  • Thorough-inspection frequency depends on type and age of equipment, amount of use, and local climate. Consult the manufacturer; build a playground-wide schedule from those intervals. That schedule does not replace regular inspections.

CPSC §4.1 Table 3 is the punch list your work orders should empty: broken equipment (loose bolts, missing end caps, cracks), glass and trash, plastic cracks, loose anchoring, hazardous debris, insect damage, surfacing problems, displaced loose-fill, unitary holes/flakes/buckling, user modifications, vandalism, worn or missing parts, wood splitting, rusted metals, and rot. §4.3 still owns loose-fill restoration (rake, post marks, flush wear mats). §4.4 requires signed, dated records of inspections and repairs, the manufacturer’s instructions, checklists used, and any reported accident or injury.

Eliminate when you can. A clothing-catch projection is not “managed” by a sign. Engineer only when the modified geometry still passes protrusion, entrapment, crush/shear, and trip tests. Administrative controls (out of service, fencing, signage) are what you do until the real repair lands — and signage is the weakest of those, because children do not read their way out of a head entrapment. Supervision is not a retrofit. CPSC §2.2.7 already says one should not rely on supervision alone to prevent injuries. Personal protective equipment is not a playground design control, and bicycle helmets on equipment are themselves a documented entanglement mechanism.

Repairs must not create the next finding

A plate welded over an opening that leaves a 3/8-inch bolt proud of the face is a new protrusion. Infill that stops at 4 inches on a 6-foot deck is still a head entrapment. A wear mat that stands an inch above the wood fiber is a new trip (CPSC §4.3 and §3.6). A replacement S-hook that still admits the 0.04 in feeler is still an entanglement. Domain 4E is not “metal was added.” It is “the cited test now passes, and no new F1487 or CPSC test fails.” Re-gauge, re-probe, and photograph the corrected condition before anyone closes the work order.

Follow the manufacturer for torque, lock nuts, covering devices, and replacement parts. CPSC §2.5.2 requires fasteners that do not loosen or come off without tools. A finger-removable cap that was “easier for the crew” recreates the laceration or clothing-catch the audit just paid to remove.

Work orders follow the audit priority list

Chapter 3 taught the professional-practice ranking: Priority 1 (imminent / life-threatening) out of service the same day; Priority 2 on a short deadline with interim protection; Priority 3 scheduled, not ignored. Domain 4E is how those ranks become work orders. Each order should carry the audit item number, the citation (F1487 performance clause or CPSC Handbook section), the priority, the assigned crew, the manufacturer instruction used, and the re-inspection result. Do not let the capital-project list swallow a Priority 1 hanger while the shop rebuilds a bench.

Temporary fixes are not corrections. Yellow caution tape, a laminated “closed” card, a cone in the use zone, or a custodian’s verbal warning can support an out-of-service decision for a few hours. They do not infill an opening, restore critical height, or replace a cracked weld. If the tape is the only work, the finding stays open. Closing a Priority 1 because tape was hung is a Domain 4E failure and a records problem for Domain 5.

The inspector’s job at this stage is still to advise. You specify the noncompliance, the citation, the recommended control level, and whether the finished repair passed re-test. The owner/operator decides who buys the part and when the bay is funded. You do not invent a published NRPA “maximum days to repair” that the Handbook does not state. You do use CPSC’s words: as soon as possible, manufacturer instructions, user modifications immediately.
