7.1 Spoken Medical Consultations, Symptoms & Pharmacy Advice

Key Takeaways

  • In body-part pain expressions, doer agrees with the body-part subject and uses an indirect-object clitic for the person affected, requiring singular 'dói-me' for singular nouns ('dói-me o pé') and plural 'doem-me' for plural nouns ('doem-me os pés').

  • Clinical consultations in Portugal occur in health centres ('centros de saúde') and pharmacies ('farmácias'), where healthcare providers routinely address patients using the formal 3rd-person register ('o senhor / a senhora' or third-person indicative/imperative verbs).

  • Dosage frequency is idiomatically expressed in spoken Portuguese with the pattern 'de X em X horas' (e.g., 'de oito em oito horas' for every eight hours) rather than direct translations from English or Spanish.

  • Crucial false friend alert: 'estar constipado' in Portugal means suffering from a common respiratory cold or head cold, never intestinal constipation (which is 'prisão de ventre' or 'obstipação').

  • Over-the-counter medications ('medicamentos não sujeitos a receita médica') are dispensed by pharmacists ('farmacêuticos') who provide spoken instructions on duration, meal timing, and medical escalation triggers.

7.1 Spoken Medical Consultations, Symptoms & Pharmacy Advice

In Component 2 (Compreensão do Oral) of the CIPLE examination, healthcare interactions represent one of the most frequently tested communicative domains. Audio recordings often simulate real-world conversations between patients and doctors (médicos), nurses (enfermeiros), or pharmacists (farmacêuticos) at a Portuguese National Health Service facility (Centro de Saúde do SNS) or community pharmacy (Farmácia). Success requires candidates to accurately identify physical complaints, interpret spoken instructions regarding pharmaceutical dosages, and understand conditional advice regarding when to seek further medical evaluation.

Physical Symptoms & Anatomical Pain: The Mechanics of Doer

Describing physical pain in European Portuguese relies heavily on the irregular verb doer (to ache / to hurt). Unlike general action verbs, in these expressions, doer is used with a body-part subject that mirrors the syntactic behavior of verbs like gustar in Spanish or plaire in French: the body part causing discomfort acts as the grammatical subject, while the suffering person is represented by an indirect object clitic pronoun (me, lhe, nos, lhes).

Sintaxe do Verbo Doer:
[Forma Verbal: Dói / Doem]-[Pronome] + [Artigo Definido] + [Parte do Corpo]
Exemplo Singular: Dói-me a cabeça. (A cabeça dói a mim)
Exemplo Plural: Doem-me as costas. (As costas doem a mim)

Singular vs. Plural Agreement with Doer

Candidates must listen attentively to whether the speaker is referencing a singular or plural anatomical structure. For the body-part subjects practised here, use third-person singular (dói) or plural (doem):

  • Singular Structure (Dói-me):

    • Dói-me a cabeça. (My head hurts / I have a headache.)
    • Dói-me a garganta. (My throat hurts / I have a sore throat.)
    • Dói-me a barriga / o estômago. (My belly / stomach hurts.)
    • Dói-me o pé direito. (My right foot hurts.)
    • Dói-me o dente molar. (My molar tooth hurts.)
  • Plural Structure (Doem-me):

    • Doem-me as costas. (My back hurts — costas is grammatically plural in Portuguese).
    • Doem-me as pernas. (My legs hurt.)
    • Doem-me os ouvidos. (My ears hurt.)
    • Doem-me os pés. (My feet hurt.)
    • Doem-me os dentes. (My teeth hurt.)

Note

Notice the contrast with Spanish: Portuguese uses doer (present tense: dói / doem), whereas Spanish uses doler (duele / duelen). Furthermore, in standard European Portuguese affirmative main clauses, clitic pronouns follow the verb with a hyphen (ênclise), as in Dói-me or Doem-lhe. However, in negative sentences or following subordinating conjunctions, the pronoun moves before the verb (próclise): Não me dói nada (Nothing hurts me) or Ele disse que lhe doía a perna (He said his leg hurt).

Somatic Complaints, Illnesses & False Friends

In addition to doer, Portuguese uses specific auxiliary constructions with ter (to have), estar com (to be experiencing temporarily), and sentir-se (to feel) to express clinical conditions:

Expressão ClínicaSignificado em InglêsExemplo de prática
Ter febreTo have a feverO meu filho tem trinta e oito graus de febre.
Ter tosse (seca / com expetoração)To have a cough (dry / chesty)Tenho uma tosse seca e irritativa à noite.
Ter tonturasTo experience dizziness / vertigoQuando me levanto rapidamente, tenho tonturas.
Ter náuseas / vómitosTo feel nauseated / vomitingTenho náuseas e má disposição desde o almoço.
Ter dores no corpoTo have generalized body achesSinto dores no corpo todo e falta de forças.
Estar constipado(a)To have a head cold / common coldEstou constipado, com o nariz entupido e a espirrar.
Estar com gripeTo have the influenza / fluEstou de cama com gripe há três dias.
Sentir-se indisposto(a)To feel unwell / queasyDepois da refeição, comecei a sentir-me indisposto.
Ter alergia a...To be allergic to...Tenho alergia severa a penicilina e aspirina.

Important

Critical False Friend: The adjective constipado / constipada in European Portuguese refers strictly to a common respiratory cold (a constipação), characterized by sneezing (espirros), a runny or congested nose (nariz entupido), and mild fatigue. It does not mean intestinal constipation. The Portuguese medical term for bowel constipation is prisão de ventre or obstipação.

Decoding Spoken Medical Instructions & Dosage Regimens

In the second half of a listening dialogue, the healthcare practitioner (doctor or pharmacist) delivers actionable therapeutic instructions. At CIPLE A2 level, candidates are expected to identify dosage quantities, frequency intervals, timing relative to meals, and safety precautions.

Expressões de Frequência e Administração de Medicamentos:
├── Frequência: "de oito em oito horas" (3x ao dia), "de doze em doze horas" (2x ao dia)
├── Relação com Refeições: "antes das refeições" / "a seguir ao almoço" / "em jejum"
├── Modo de Aplicação: "tomar por via oral" / "aplicar a pomada na pele"
└── Precauções: "fazer repouso" / "beber muitos líquidos" / "evitar bebidas alcoólicas"

Key Frequency Formulas in Spoken Portuguese

  • "De X em X horas": Spoken European Portuguese overwhelmingly favors this formula rather than a cada X horas:
    • De oito em oito horas = Every 8 hours (3 times daily).
    • De doze em doze horas = Every 12 hours (twice daily).
    • De seis em seis horas = Every 6 hours (4 times daily).
    • Uma vez ao dia, ao deitar = Once a day, at bedtime.
    • Duas vezes ao dia, de manhã e à noite = Twice a day, morning and night.

Keep the spoken conditions separate

Antes das refeições means before meals and depois de comer means after eating. Neither phrase alone specifies an exact number of minutes or a medical reason. The language exercise should not be used to decide a treatment. For an actual medicine, use the current leaflet and the professional’s instructions.

A fictional message says: A consulta é amanhã às dez. Chegue quinze minutos antes e traga o documento de identificação. The appointment is at 10h00; requested arrival is 09h45. The fifteen minutes is an arrival offset, not the duration of the consultation. A listener who answers 10h15 has reversed antes and depois.

Annotated Spoken Dialogue: Consulta no Centro de Saúde

This original practice exchange reports symptoms without making a diagnosis:

Profissional: Bom dia. O que se passa?
Utente: Dói-me a garganta desde ontem e tenho tosse.
Profissional: Tem alergia a algum medicamento?
Utente: Sim, tenho uma alergia. Trouxe a informação do meu médico. Pode ler, por favor?
Profissional: Obrigada. Diga-me também quando começaram os sintomas.
Utente: A dor começou ontem à tarde. Pode repetir a última pergunta mais devagar?

Listen with the text hidden as a partner reads. Identify the complaint, its starting time and the question about allergy. The reply reports an allergy; it does not identify which medicine caused it. Do not invent that missing detail or infer that a particular treatment is appropriate.

Ask for emergency help

CAPLE includes asking for help in an emergency among speaking skills. Practise clear language: Preciso de ajuda. Há uma pessoa ferida. Estamos na Rua Nova, junto à biblioteca. Pode chamar uma ambulância? State the need, location and relevant facts; answer the responder’s questions. For a life-threatening emergency in Portugal, use 112. For non-emergency symptom guidance, SNS 24: 808 24 24 24 is listed by gov.pt. This language practice does not diagnose a condition or replace instructions from emergency services.

A partner can ask Onde está? and Qual é o problema? Reply using the location and observed problem, not a long account of your travel plans. If you do not understand, say Pode repetir? and repeat the location back to confirm it. Never make a real emergency call merely to rehearse.
