7.1 Structures of the Nail Unit

NIC Theory Domain 2, Anatomy and Physiology, is about 15% of the scored written exam. A large slice of that domain is the nail unit: every living and nonliving part that grows, supports, or seals a fingernail or toenail. Washington then asks you to treat those parts inside the practice of manicuring in RCW 18.16.020 and the sanitation rules in WAC 308-20-110. If you mix up dead cuticle with living eponychium, or file the nail plate as if it were the nail bed, you will miss classic NIC items and you can create a blood exposure in a Seattle booth.

The nail unit is a system, not just a plate

The nail unit (also called the natural nail unit) is every structure that produces, supports, or seals the nail. It sits over the distal phalanx, the fingertip or toe-tip bone. The hard surface you polish is only one piece. Under, around, and behind that surface are living skin, a growth factory, grooves, folds, and ligaments. Washington manicurist services — cleaning, shaping, polishing, decorating, cuticle care, and the application and removal of sculptured or otherwise artificial nails — act on these structures. They do not act on bone, and they do not include medical treatment of the matrix.

Memorize the roster by location, not by nickname. Clients will say cuticle for three different tissues. NIC will not.

Nail plate: hard keratin, no nerves, no vessels

The nail plate (nail body) is the hard, keratinized surface you file, polish, and enhance. It is built from layers of dead, compacted cells that the matrix produced. A healthy plate has no nerves and no blood vessels. That is why an intact plate can be shaped with a file and why clipping the free edge does not bleed. Pain or bleeding during ordinary filing means you have left the plate and reached living tissue, or you have generated enough heat to irritate the bed underneath.

The pink color of a healthy fingernail is not pigment in the plate. It is blood in the nail bed showing through a translucent plate. A white free edge is plate that has grown past the bed. Do not read every color change as a product problem; often you are seeing the bed, a lift, or a bruise under an otherwise nerve-free plate.

Enhancement prep belongs on this plate: light surface buffing, dehydrating, and product placement. Prep does not belong on the living eponychium, the lateral folds, or the hyponychium.

Nail bed: nerves and blood vessels (the classic NIC item)

The nail bed is the living skin beneath the plate, from the lunula area out toward the hyponychium. Unlike the plate, the bed contains nerves and blood vessels. That contrast is a classic NIC theory item. The plate can be filed. The bed cannot. If you thin an enhancement until the bit is on the natural plate and then keep going, you can heat, thin, or tear into a vascular, innervated bed.

The bed does not manufacture the plate. It supports the plate and helps it slide distally as new plate is born at the matrix. A thin layer of tissue, the bed epithelium, keeps plate and bed attached in a healthy nail. When that attachment fails, the plate can lift and the bed is exposed to water, product, and pathogens. You do not file into a lifting area to clean it up as if it were dead plate. You stop short of living tissue.

Never file into the nail bed. That is both an anatomy rule and a Washington service rule. Filing into the bed is a wound. WAC 308-20-110(1)(e) does not let you perform or continue services on visible open sores or inflamed skin. If you created the sore, you have a blood-spill situation, not a keep-blending situation.

Matrix and lunula: the factory and its window

The matrix is the actively growing part of the nail unit. Matrix cells keratinize and become plate. The matrix lies under the proximal nail fold, protected by the eponychium. It has nerves, blood vessels, and lymph. Section 7.2 is devoted to growth; for structure, know where it lives and that you cannot see most of it.

The lunula is the whitish half-moon at the base of the plate. It is the visible part of the matrix. Thumbs show it most often. Some clients have no visible lunula because the proximal fold covers it; that is a variation, not a disease you treat with a bit. Do not buff the lunula away. You would be working over matrix.

Cuticle versus eponychium: dead on the plate, living at the base

This pair fails more candidates than any other nail-anatomy pair.

The cuticle is dead, colorless tissue that adheres to the nail plate from the underside of the proximal nail fold. It is a seal between the plate and the living fold. Dead cuticle on the plate may be gently pushed and, when it is truly dry, loose, and nonliving, carefully reduced. It is not living skin.

The eponychium is the living skin at the base of the nail plate covering the matrix area. Clients call it the cuticle. NIC does not. The eponychium is vascular. Cutting it is cutting a person. Over-nipping living eponychium is a blood-exposure event and a scope-and-sanitation problem in Washington. WAC 308-20-110(1)(e) bars services on inflamed or open tissue. A Bellevue client who wants cuticles cut short is asking you to respect anatomy, not to obey a nickname.

RCW 18.16.020 includes caring for and treatment of the cuticles. That statutory word is cosmetic care of the dead tissue on the plate and careful work around the nail, not a license to resect living eponychium as if you were in a medical office. You do not diagnose, you do not inject, and you do not cut living skin.

Perionychium, hyponychium, folds, and grooves

The perionychium is the living tissue that surrounds the nail plate, especially along the sides — the tissue of the lateral folds. It is living. Hangnails tear here. Soaking and oil can make this tissue more pliable. Nippers on a hangnail that still has a living base will bleed.

The hyponychium is the slightly thickened living skin under the free edge. It seals the junction between plate and bed so pathogens do not march under the nail. Filing or picking the hyponychium to clean under the nail breaks that seal. Never file the hyponychium. Never use a bit under the free edge as if it were a plate surface.

Nail folds are the folds of normal skin around the plate. The proximal nail fold sits at the base and covers the matrix. The lateral nail folds (sidewalls) sit in the grooves. Nail grooves (nail sulci) are the tracks along the sides in which the plate grows forward. Product packed into a groove, or a bit run along a groove until it nicks the fold, injures living tissue.

The free edge is the part of the plate that extends beyond the fingertip and past the hyponychium. There is no bed under the free edge. This is the part you shape. Length lives here. Do not confuse free-edge filing with filing the plate down to the bed in the nail-bed zone.

Mantle, ligaments, and the bone underneath

The mantle is the deep fold of skin at the base of the nail where the nail root is embedded. Older textbooks use this word for the pocket that holds the root and protects the matrix. You do not clean the mantle with a sharp bit.

A ligament is a tough band of fibrous tissue that connects bone or holds structure in place. Specialized ligaments attach the nail bed and the matrix to the underlying distal phalanx. They are not cuticle. They are not something you nip. Trauma that rips plate from bed can stress these attachments; that is a medical referral, not a fill.

Salon scenario — Tacoma nipper. A Tacoma client asks the manicurist to cut the cuticles all the way back. The tissue at the base is pink, living eponychium. The tech refuses to nip living skin, soaks, pushes dead cuticle on the plate only, and oils the folds. That is in-scope cuticle care. Nipping until it bleeds would be a WAC 308-20-110 blood-exposure and open-tissue problem.

Salon scenario — Spokane e-file. A Spokane fill looks bulky near the sidewalls. The tech runs a bit down the nail groove until the lateral fold bleeds. That was perionychium, not plate. Stop, follow the blood-spill order, and do not finish the enhancement on inflamed tissue.

Salon scenario — Seattle under-nail cleaning. A Seattle pedicure guest wants the hyponychium scraped so it looks clean. The tech files only the free edge and cleans only the underside of the plate, not living hyponychium. Breaking that seal invites infection under the plate.

Salon scenario — Olympia enhancement prep. An Olympia student buffs the eponychium until it is shiny for better adhesion. Adhesion is a plate job. Shiny living skin is an injury. Primer stays on the plate.

Know the map: plate and free edge are the work surface; bed, matrix, eponychium, perionychium, and hyponychium are living and off-limits to cutting and aggressive filing. Domain 2 will name these parts. Washington sanitation will test whether you treat living tissue as living.
