11.2 Paper 2: Site Visit Practice, the Site Assessment and the Online HIRARC Presentation
Key Takeaways
Paper 2B presentation candidates choose one of ten topics: traffic management, piling, concrete work, electrical wiring or electrical work, welding, work at height, excavation, demolition, crane lifting, or confined space work.
Under the LPKKKP's presentation guideline, slides must contain a safety briefing, introduction, objectives, a HIRARC based on DOSH's HIRARC Guidelines 2008, a conclusion and references, in Microsoft PowerPoint 2013 or later.
In the Paper 2B marking scheme of 100 marks, HIRARC carries 20 marks, topic knowledge 16, legislation 10, the SSS's role 8, and the question-and-answer session 20.
The Paper 2B presentation runs online by Zoom for 10 minutes; the invigilator signals once to start, twice when two minutes remain, and three times when the 10 minutes end.
The presentation guideline says assessors may fail a candidate for copying or plagiarism, and candidates found guilty of examination misconduct can be suspended from the examination for up to two years.
Paper 2 at a glance
| Part | What happens | Time | Pass mark | Fee |
|---|---|---|---|---|
| 2A Site assessment (Penilaian Tapak) | Assessment of a construction site | 30 minutes | 50% | RM100 |
| 2B Presentation (Pembentangan) | Online presentation of a HIRARC on your chosen topic, followed by questions | 10 + 10 minutes | 50% | RM150 |
You must pass both parts. If you fail one, you repeat only that part (Section 11.1). Paper 2A is held on the same day and at the same venue as Paper 1. Paper 2B runs online over the next three days.
The format document describes Part A only as a 30-minute site assessment. Follow the briefing NIOSH gives on the day. The best preparation is the site visit in Part 6 of the course, and practising the inspection method below on your own site.
Part A: inspecting a site systematically
An assessor wants to see that you can find the hazards that matter, judge how serious they are, and recommend practical controls. Walk the site in a fixed order so you miss nothing:
| Stop | What to look for | Guide sections |
|---|---|---|
| Perimeter and gate | Hoarding and signs, gantries over footpaths, gate control, separate pedestrian and vehicle entry, the CDM regulation 8 notification displayed | 3.1, 3.3 |
| Traffic routes and plant | One-way routes, banksmen, reversing, separation of people and plant, parking, plant certificates and operators | 3.1, 8.4 |
| Welfare and housekeeping | Toilets, washing, drinking water, rest areas, first-aid box and first-aiders, waste, nails, storage | 3.2 |
| Work at height | Edge protection, floor openings covered and marked, scaffold tags and ties, ladders, harness use and anchors | 4.1–5.3 |
| Excavations | Support or battering, edge barriers, spoil and plant kept back, access ladders, inspection records, services | 6.1–6.4 |
| Lifting | Crane set-up and outriggers, exclusion zones, lifting gear tags, signaller, lift plan | 8.1–8.3 |
| Electrical | Distribution boards, RCDs, cable routing, damaged leads, overhead lines | 9.1 |
| Fire and hot work | Extinguishers, hot work permits, gas cylinder storage, flammables | 9.2 |
| Confined spaces | Manholes and tanks identified and controlled, permits, gas testing | 9.3 |
| Health | Silica dust, noise, chemicals and labels, manual handling, heat | 10.1–10.3 |
| Records | HIRARC, method statements, permits, inspection registers, toolbox records, NADOPOD reports | 2.4, 2.5 |
Look at people as well as conditions. Note unsafe acts, such as a worker unclipped at an edge or someone standing under a load. Note unsafe conditions, such as a missing guardrail or a damaged plug. Note good practice too: assessors credit balanced observation.
Recording findings
Write each finding as a complete line that someone could act on:
| No. | Observation (where, what) | Hazard and possible harm | Risk (L × S) | Legal or guidance reference | Recommended action | Priority |
|---|---|---|---|---|---|---|
| 1 | Level 3, east edge: guardrail removed for a delivery and not replaced | Fall of person from about 9 m; death | 4 × 4 = 16, High | CDM regulation 19; DOSH falls guideline 2007 | Stop work at the edge; refit the guardrail before work resumes; brief the delivery crew | Immediate |
| 2 | Generator area: extension lead with taped joint lying in water | Electric shock | 3 × 4 = 12, Medium | CDM regulation 27; Electricity Regulations 1994 | Withdraw the lead; supply through an RCD-protected board; route cables off the ground | Today |
| 3 | Block store: blocks stacked 2.5 m high, leaning | Collapse onto workers | 3 × 3 = 9, Medium | CDM regulation 20 (good order) | Restack lower and stable; keep an access aisle | This week |
The ratings use the HIRARC 2008 matrix (Section 2.3): 15 to 25 is High, 5 to 12 is Medium, and 1 to 4 is Low. Life-threatening findings come first. An SSS stops work where there is imminent danger and reports to the principal contractor.
Part B: the presentation rules (guideline version 1.4)
Choose a topic
| No. | Topic (official Malay title) | Guide sections |
|---|---|---|
| 1 | Traffic management (Aktiviti Pengurusan Trafik) | 3.1, 8.4 |
| 2 | Piling work (Aktiviti Kerja Cerucuk) | 7.1 |
| 3 | Concrete work (Aktiviti Kerja Konkrit) | 7.2, 7.3 |

| 8 | Demolition work (Aktiviti Kerja Perobohan) | 7.4 |
| 9 | Lifting with cranes (Aktiviti Kerja Mengangkat Menggunakan Kren) | 8.1–8.3 |
| 10 | Work in a confined space (Aktiviti Kerja di Ruang Terkurung) | 9.3 |
Repeat candidates must update their slides on the same topic they presented before.
What the slides must contain
- a safety briefing related to the topic
- an introduction to the topic
- the objectives of the presentation
- risk management: HIRARC, presented thoroughly, covering both safety and health aspects, with reference to DOSH's HIRARC Guidelines 2008
- a conclusion
- references
The slides must be in Microsoft PowerPoint 2013 or later, neat, readable by the assessment panel, and display clearly when projected or shared. They must be your own work.
The online session
- Use a laptop or desktop computer only.
- Sit somewhere you will not be disturbed, and be alone throughout.
- The session runs on Zoom, or another application the LPKKKP examination centre specifies.
- Be ready 30 minutes before the time on your examination slip.
Presentation (10 minutes):
| Signal from the invigilator | Meaning |
|---|---|
| Instruction, or one "hand" symbol or bell | Start presenting |
| Two "hand" symbols or bells | Two minutes left |
| Three "hand" symbols or bells | The 10 minutes have ended |
Questions and answers (10 minutes): one signal starts the session and two signals end it. Write down any suggestions or corrections the panel gives.
Familiarisation: candidates are encouraged to attend the Paper 2B familiarisation session held for each examination session. Dates are sent through the SSS examination Telegram channel, whose link is sent by SMS.
Integrity: assessors may fail a candidate for copying or plagiarism. Examination misconduct is dealt with by the SSS Competency Committee and the LPKKKP, and can lead to suspension from the examination for up to two years.
The marking scheme
| Element assessed | Marks |
|---|---|
| A. Presentation skills | |
| Safety briefing | 6 |
| Introduction and objectives of the topic | 8 |
| Knowledge of the topic | 16 |
| Risk assessment (HIRARC) | 20 |
| Knowledge of legislation | 10 |
| Knowledge of the SSS's responsibilities and role | 8 |
| Delivery and confidence | 4 |
| Tone (voice and volume) | 4 |
| Conclusion, presentation materials and references | 4 |
| B. Answering questions | |
| Questions on understanding of the topic | 20 |
| Total | 100 |
The HIRARC and the Q&A together carry 40% of the marks. The legal and SSS-role elements add another 18. A polished deck with a weak HIRARC will not pass easily.
Building a 10-minute deck
| Minutes | Slides | Content |
|---|---|---|
| 0:00–1:00 | 1–2 | Title, then the safety briefing: as you would deliver it to workers before the task, covering the main hazards, PPE, emergency arrangements and stop-work authority |
| 1:00–2:00 | 3–4 | Introduction to the activity (scope, plant, crew) and objectives |
| 2:00–6:30 | 5–10 | HIRARC: break the activity into steps. For each step give hazards (safety and health), the possible harm, existing controls, L, S and risk, additional controls by the hierarchy, residual risk, and the person responsible |
| 6:30–8:00 | 11–12 | Legislation and the SSS's role for this activity |
| 8:00–9:30 | 13 | Conclusion |
| 9:30–10:00 | 14 | References |
Rehearse with a timer until you finish at about nine and a half minutes. Speak to the panel, not the slides. Keep text large and tables simple.

For every topic, also cite OSHA section 18B (risk assessment) and CDM regulation 39 (the SSS appointment). Never cite BOWEC 1986 or the Factories and Machinery Act 1967 as current law. The panel will treat it as out of date.
Handling the questions
The panel tests understanding, not memory of your slides. Typical questions:
- "Why did you rate this risk as High?"
- "What would you do first if…?"
- "Which regulation requires…?"
- "Who is responsible for…?"
- "What is the SSS's role if the contractor refuses?"
Answer briefly and directly. Link your answer to the hierarchy of control and the law. If you do not know, say how you would find out, for example from the manufacturer's manual, the temporary works designer or DOSH guidance. Note the panel's suggestions as the guideline requires.
Scenario: a presentation that lost marks
A candidate presenting crane lifting spends six minutes on crane types and photographs. Their HIRARC is a single slide listing "falling load, use PPE". They cite BOWEC as the governing regulations and run out of time before the conclusion.
To fix this:
- Spend most of the time on a step-by-step HIRARC: set-up, rigging, lifting, landing and de-rigging.
- Cover health hazards as well, such as heat and noise for the signaller.
- Cite the Plant Requiring Certificate of Fitness Regulations 2024 and CDM 2024.
- Show how the SSS checks the lift plan, certificates and exclusion zone.
- Finish on time.
