  • Must hold active CPhT certification from PTCB
  • Complete CSPT Competency Attestation Form signed by qualified supervisor
  • One-year candidacy eligibility window to complete all requirements
  • Full-time defined as at least 30 hours/week in CSP work
  • Must recertify annually with 10 compounding CE hours and competency attestation
