Additional Requirements
  • Must pass both the Written Examination (100 questions, 80% required) AND the Clinical Skills Evaluation (5 randomly selected skills, 100% accuracy required)
  • Indiana is unique: exam administered by Ivy Tech Community College, NOT Prometric or Headmaster
  • Must test within 2 years of completing an approved training program
  • Indiana Nurse Aide Registry (INAR) maintained by ISDH under 410 IAC 16.2
  • INAR renewal every 24 months: must have completed at least 8 paid hours of nursing aide work during that period
  • INAR records both active certifications and substantiated abuse/neglect/misappropriation findings
  • Substantiated findings of abuse, neglect, or misappropriation are permanently noted on the INAR and may bar future employment
  • Mandatory reporting: CNAs must report suspected resident abuse/neglect to supervisor and Adult Protective Services (APS) / ISDH
  • Indiana Long-Term Care Ombudsman operates under the Indiana Family and Social Services Administration (FSSA) — advocate for resident rights
  • Reciprocity available for CNAs with active unencumbered listing in another state — apply through ISDH

Licensed Practical Nurse (LPN)
