1.3 Exam Process: Board Application, Prov NIC Theory & Board Practical

Two tests, two vendors, one license

Quick Answer: Candidates seeking an Oklahoma cosmetology license must pass two examinations. Prov, Inc. delivers only the National-Interstate Council of State Boards of Cosmetology (NIC) written National Cosmetology Theory Examination. The practical is administered by the Oklahoma State Board of Cosmetology and Barbering, not by Prov. The Board exam page (modified August 7, 2025) still states you must pass written and practical at a minimum of 75 percent. Prov's Oklahoma bulletin dated 2026.01.05 prices the written exam at $65 each attempt. The Board page and Title 59, Section 199.14, list Examination for license $50.00 — the Board's registration/practical figure, not a substitute for the Prov invoice.

The Board exams hub (modified September 23, 2025) states that Prov is the only provider of the NIC Theory Examination for Oklahoma. Prov administers theory for Cosmetology, Barbering, Facialist/Esthetician, Manicurist/Nail Tech, and Master Instructor. OSBCB itself still administers state-specific safety and sanitation exams for eyelash extension specialist and instructor, blow-dry/styling, makeup artist, and reciprocity. Cosmetology operator candidates are in the Prov-plus-Board-practical group, not the Board-only safety-exam group.

Do not import leftover national templates that say the Oklahoma written test is "100 questions," "2 hours," or "70–75 percent, varies by state." Those figures are not the current owner sources for this exam. Use the NIC bulletin for item count and time, the Board exam page for the 75 percent passing standard, and the two fee documents for the two dollar amounts.

The application sequence you cannot skip

Prov's 2026.01.05 bulletin is blunt about order of operations:

1. Apply to the Board first using the Board's exam-registration process (the exams hub links Exam Registration for the Disciplines, file 201.2208 Exam Registration.pdf). Licensing questions go to OSBCB at (405) 521-2441.
2. Register with Prov at provexam.com/register (or call/text (801) 733-4455). Select certifying body OK Cosmetology Related Fields, then the Cosmetologist (NIC Cosmetology exam) license. Enter your name exactly as it appears on your government ID, plus address, Social Security number, email, telephone, and birthdate.
3. Prov forwards your file. The Board reviews experience and training against the Board application and approves you for testing.
4. After approval you receive an emailed voucher. Only then do you schedule: return to provexam.com, choose Pre-Approved Registration, enter Candidate ID and last name, pick date, time, and site, and pay the $65 written fee (MasterCard, Visa, AMEX, or a school coupon code).
5. Sit the NIC theory at a Prov center. Sit the Board practical on the Board's schedule, with the Board's kit rules.

Prov is not authorized to decide which examination you need. If you are unsure whether you should schedule Cosmetologist versus another NIC form, resolve that with your school or the Board before you pay. The June 30, 2025 minutes add a second caution for 1,000-hour nonchemical graduates: as of that meeting NIC had no non-chemical cosmetology theory exam (a non-chemical barber exam did exist). Confirm with OSBCB which written form your approval letter actually covers.

What is on the NIC written theory — and what it is not

The NIC National Cosmetology Theory Examination CIB (effective July 1, 2022; revision January 1, 2026) is the content document Prov's Oklahoma bulletin points cosmetologist candidates to. Time allowed is 90 minutes. The examination contains 110 items, of which 100 items are weighted and contribute to the score; the other 10 are pretest items that do not count. There is no penalty for guessing. You may flag items and review them from the Summary screen before you end the test.

Those percentages describe the national theory form NIC sells. They are not an Oklahoma statute, and this OpenExamPrep chapter does not claim the Board has rewritten Oklahoma law to match them. They are the weights on the written test Prov will actually time at 90 minutes.

References NIC lists for the January 1, 2026 revision include Milady Standard Cosmetology and Foundations (14th edition, 2023) and Pivot Point Fundamentals: Cosmetology (2nd edition, December 2023), plus older supplemental editions. Schools still using only a 2016 textbook should check that list rather than assume an older chapter map still covers every scored item.

Test-day rules at a Prov center

Prov has 17 Oklahoma written sites in the 2026.01.05 bulletin, including Western Oklahoma State College (Altus), Northwestern Oklahoma State University (Alva), Southern Tech (Ardmore), Red River Technology Center (Duncan), Kiamichi campuses (Durant, Poteau, Talihina), Redlands (El Reno), Moore Norman (Norman and Oklahoma City), OSU-OKC, Pioneer (Ponca City), OSU Stillwater, Tulsa Community College Northeast, Mid-America (Wayne), and Southwestern Oklahoma State University (Weatherford). Border-state sites can appear in the scheduler even though they are omitted from that printed list. Always use the live calendar.

Doors open at least 15 minutes before the appointment. Arrive early. If the session has started, you are turned away, you forfeit the fee, and you must reschedule. Bring current government-issued photo ID with signature; first and last name must match the admission documents. Driver's license, passport, or military ID are listed; temporary IDs need a photo; expired IDs are refused. You will sign a log and be photographed. Cell phones, watches of any kind, notes, bags, and study materials stay out of the room. Restroom breaks do not pause the clock. Computer freezes are restarted without loss of remaining time if you notify the proctor.

Cancel or reschedule online at no charge at least three days before the theory date. Miss that deadline, or no-show, and the exam fee is forfeited. Emergencies require written documentation to Prov. ADA accommodations go through Prov's ADA forms with documentation from a licensed physician or psychologist; Prov states accommodations are not granted solely for inability to read English.

Results: unofficial now, official in three days

Prov scores the written exam at the center and gives unofficial results before you leave. Official results are emailed within 3 days, and official results are sent to the State within those same 3 days. The June 30, 2025 minutes noted that turnaround had improved from five-to-six weeks down to about one week under the current vendor arrangement — consistent with the bulletin's three-day official email, not with older "six-week wait" advice.

Board practical: mannequins, kit, and the $50 figure

The Board's cosmetology exam page is still the owner source for practical logistics, even though its hour count is stale. Current posted rules include:

• Mannequin head and mannequin hand must be used. No live models.
• The Board provides over-the-chair mannequin head stands.
• Mannequin hands must have artificial nails glued on and polished. No jointed hands.
• Students bring their own suction or clamp hand stands; hand stands are optional.

The same page groups supplies by service family. You are not memorizing a shopping receipt for its own sake; you are learning what the Board still tells candidates to put in the kit:

• Thermal / set work: dried (not pre-curled) mannequin hair 6–7 inches, 3/4-barrel spring or Marcel iron, cape, at least 12 double-prong clips, hard rubber heat-resistant non-metal comb, tissue or neck strip, towel or neck-strip drape.
• Finish: one hairbrush; one styling, tail, or pick comb.
• Haircut: spray bottle, cutting or tail comb, clips, cape, thinning shears, shears, razor with guard (no carving combs), towel or neck strip.
• Chemical wave mock: 2 white, 2 grey, 2 pink rods (no half rods), non-metal comb, spray bottle, cotton protective strip, end papers, towels, chemical cape, applicator bottle with water for mock application, disposable gloves.
• Color mock: cholesterol, spatula, disposable gloves, non-metal-tail chemical brush and bowl (no applicator bottle), clips, non-metal comb.
• Facial: cleansing cream, astringent, cape, moist cotton pledgets for eye protection, spatulas, cotton for cream removal, towel or neck strip, headband/cap/turban.
• Wax mock: wooden applicators, cloth or paper strips, cotton swabs or tissue, mock wax (thick cream), soothing lotion or gel.
• Manicure: polish remover, cotton, wet sanitizer with solution, 70% alcohol or hand sanitizer, manicure bowl and brush, liquid soap, cuticle remover, nippers, pusher or orangewood stick, file or emery board, emollient cream, red polish, clear plastic bag taped to a towel, at least three towels.

Read that list as the Board's current published kit, and re-check the page before your date. Kit details can move without a statute change.

Keep the two fees in different columns

Passing both written and practical at 75 percent is the Board exam-page standard. After both parts are official, you still complete license issuance through the Board portal. The homepage two-year $80 operator fee is the post-exam credential, not a third testing invoice.

Retakes, no-shows, and cheating

Prov's retesting policy is a Board-first loop: you must apply again to OSBCB, wait for approval, then schedule with Prov as a returning user (provexam.com or 1-866-720-7768). The same re-application is required if you fail to appear. You cannot retake an exam you have already passed unless the State of Oklahoma requires it. The $65 written fee is the same for a retake as for a first attempt.

Unethical conduct (giving or receiving assistance, unauthorized materials, leaving with questions) is reported, the session is terminated, fees are forfeited, scores can be frozen, and Prov states it will prosecute theft of copyrighted testing materials. Threatening a proctor can result in a testing ban and a report to the Board.

Criminal-history applicants should use the Board's criminal-history page linked from the exams hub before paying Prov. That review is a Board process, not a Prov scheduling question.

Once licensed, keep the credential posted in public view with a current photo, work in a licensed establishment unless an emergency house-call exception in Section 199.9 applies, and follow the two-year renewal cycle that began November 1, 2025.
