1A barangay health nurse reviews clinic records and finds many infants missed measles-containing vaccine appointments. Which action best reflects the nursing process in community health nursing?
A.Validate the defaulter list, identify reasons for missed visits, and plan follow-up with the family and barangay team.
B.Wait until the next national campaign before contacting families.
C.Refer every child immediately to the emergency department.
D.Record the children as noncompliant and close the immunization file.
Explanation: Community health nursing uses assessment before intervention. Validating the list and exploring barriers such as access, fear, illness, or migration allows the nurse to plan realistic follow-up and health teaching.
2Which principle is central to primary health care in a Philippine barangay setting?
A.Care should be available only in hospitals with specialist staff.
B.Community members should participate in identifying needs and carrying out solutions.
C.Health services should focus only on treatment after disease develops.
D.Programs should be designed without local data to keep them uniform nationwide.
Explanation: Primary health care emphasizes accessibility, community participation, intersectoral collaboration, and appropriate technology. Barangay-level participation helps services fit local risks and resources.
3Which numerator is used when computing the infant mortality rate for a municipality?
A.Deaths among children under 5 years old during the year
B.Deaths among all live-born infants in the neonatal period only
C.Deaths among infants younger than 1 year during the year
D.Stillbirths plus deaths among infants younger than 1 year
Explanation: The infant mortality rate uses deaths of infants under 1 year of age as the numerator and live births as the denominator. It is a key indicator of maternal-child health and health-system performance.
4A rural health unit receives reports of several learners with fever and rash. Before launching broad control measures, what should the nurse do first?
A.Close all classes for 2 weeks without further assessment.
B.Give antibiotics to all students in the school.
C.Ask parents to manage symptoms at home and stop reporting.
D.Verify the diagnosis and confirm that the number of cases is above what is expected.
Explanation: Outbreak investigation begins with verifying the diagnosis and confirming that an outbreak exists. Accurate case definition and confirmation prevent unnecessary measures and guide appropriate reporting and control.
5Under the routine infant immunization program, which vaccine is normally given as a birth dose to protect against severe childhood tuberculosis?
A.BCG vaccine
B.MMR vaccine
C.Rotavirus vaccine
D.Japanese encephalitis vaccine
Explanation: BCG is given at birth or as early as possible to protect infants from severe forms of tuberculosis such as TB meningitis and miliary TB. Birth-dose immunization is a common public-health nursing responsibility.
6During a barangay consultation, an adult reports cough for more than 2 weeks, weight loss, and night sweats. What is the best nursing action?
A.Advise the client to buy cough suppressants and return if symptoms persist for 6 months.
B.Refer the client for tuberculosis evaluation, sputum testing as indicated, and counseling on cough etiquette.
C.Give a short course of leftover antibiotics from the clinic supply.
D.Tell the client to avoid all vaccines because TB is suspected.
Explanation: A cough of 2 weeks or more with systemic symptoms is a classic trigger for TB evaluation. Nursing care includes prompt referral or specimen collection according to local protocol, infection control teaching, and adherence support if treatment begins.
7Which teaching point is most appropriate for a client newly diagnosed with hypertension at the rural health unit?
A.Stop medication once the blood pressure feels normal.
B.Use herbal products instead of prescribed medicine to avoid side effects.
C.Reduce dietary salt, take prescribed medicines consistently, and return for blood pressure monitoring.
D.Exercise only when systolic blood pressure is above 180 mm Hg.
Explanation: Hypertension control requires lifestyle measures, medication adherence when prescribed, and follow-up monitoring. The nurse should teach concrete actions and check barriers to long-term control.
8In disaster triage at an evacuation center, which client should be assessed first?
A.A client with a small abrasion who is asking for antiseptic
B.A client with mild anxiety who is pacing near the entrance
C.A client with an old ankle sprain requesting a bandage
D.A client with noisy breathing, cyanosis, and difficulty speaking
Explanation: Airway and breathing problems are immediate priorities in triage because they threaten life within minutes. Noisy breathing, cyanosis, and inability to speak suggest severe respiratory compromise.
9A 17-year-old pregnant client has had no prenatal visits and says she is afraid her parents will find out. Which nursing response is best?
A.Assess safety, explain confidential services within legal limits, and link her to prenatal care.
B.Tell her prenatal care can wait until the third trimester if she feels well.
C.Call her parents immediately before assessing her concerns.
D.Advise her to avoid the health center to prevent embarrassment.
Explanation: The nurse should first assess health and safety, then reduce barriers to prenatal care using respectful communication. Early prenatal care identifies risks, supports nutrition and immunization, and prepares for safe delivery.
10After a fiesta meal, 15 of 75 people who ate pancit developed vomiting and diarrhea within 8 hours. What is the attack rate among those exposed?
A.15%
B.20%
C.60%
D.75%
Explanation: Attack rate is the number of new cases among the exposed divided by the total exposed, multiplied by 100. Here, 15 divided by 75 equals 0.20, or 20%.

PNLE Study Tips from Top Performers

1Study by Nursing Practice subject, but drill nursing process, safety, infection control, pharmacology, documentation, and ethics across every subject because the TOS embeds outcomes across clinical situations.
2Use 100-item timed blocks to simulate one Nursing Practice test, then review missed items by topic and by nursing judgment error.
3For community health, connect facts to barangay-level decisions: surveillance, immunization coverage, TB and dengue control, disaster triage, UHC referral, and health education.
4For maternal-child nursing, prioritize danger signs, safe labor and newborn care, postpartum hemorrhage, hypertensive disorders, family planning, growth and development, and pediatric dehydration or infection scenarios.
5For medical-surgical nursing, master ABCs, fluid and electrolyte patterns, perioperative complications, oxygenation, sepsis, endocrine emergencies, stroke, renal care, and high-acuity prioritization.
6For psychiatric and professional practice, practice therapeutic communication, suicide and violence safety, psychopharmacology adverse effects, delegation, informed consent, incident reporting, RA 9173, and patient autonomy.

Does the Philippine Nurses Licensure Examination exist?

Yes. PRC publishes Board of Nursing programs, schedules, results, board law, and enhanced tables of specifications for the Nurses Licensure Examination.

The current structure is five Nursing Practice subjects. The PRC enhanced TOS Annex A specifies 100 items per subject, for 500 total items.

RA 9173 requires a general average of at least 75% with a rating of not below 60% in any subject. If the general average is 75% or higher but a subject is below 60%, the examinee retakes only the subject or subjects below 60% and must earn at least 75% in the repeated subject or subjects.

How much is the examination fee?

The PRC Board of Nursing fee source consulted lists PHP 900 for the complete examination and PHP 450 for conditioned or removal. Applicants should verify the current amount in LERIS and current PRC notices before payment.

Are these copied board questions?

No. These are original practice questions written from official PRC content outlines, nursing standards, laws, and public-health references. They are not copied from official examinations or unofficial dumps.
