The NEBOSH HSE Introduction to Incident Investigation (Unit INV1) is a 1-day joint qualification with the GB Health and Safety Executive. The official assessment is a 1-hour practical examination analyzing a scenario evidence pack and witness interview video. The syllabus covers moral, legal, and financial reasons for investigation; incident causation models (Swiss Cheese model, active vs latent failures, human error taxonomy); the 4-step investigation process (immediate action, evidence gathering, root-cause analysis via 5 Whys and fishbone diagrams, SMART action planning); and the PEACE witness interviewing model.
