Stratum germinativum (stratum basale)
This is the deepest epidermal layer: a single row of columnar or cuboidal keratinocytes sitting on the basement membrane. Mitosis happens here. New cells push older ones toward the surface. Melanocytes live in this layer and produce melanin, then transfer pigment into nearby keratinocytes — that is the biology behind even tone, a tan, and post-inflammatory hyperpigmentation. Merkel cells (touch) also sit here.
A classic NIC-style stem says the basal layer is the germinativum. Do not pick spinosum. Do not pick corneum. Two names, one layer. This layer is living. Illinois does not license you to target it with a peel, a needle, or a remodeling device and still call the service esthetics.
Stratum spinosum
Several rows of keratinocytes, still living. Under the microscope they look spiny because desmosomes (protein bridges) hold neighboring cells together. Those bridges are why intact epidermis resists shear. Langerhans cells — immune sentinels — are concentrated here. Keratin synthesis ramps up. The spinosum is still living epidermis. A product or peel intended to wound this layer so the dermis will scar-remodel is not a facial under 3A-1.
Stratum granulosum
Cells flatten. Keratohyalin granules appear. Nuclei and organelles degrade. Lipid lamellae are extruded into the intercellular space — the waterproof mortar the corneum will inherit. This is the transition from living cell to dead corneocyte. It is not the Illinois treatment plane, but it is why over-stripping the surface later increases transepidermal water loss (TEWL): you are ransacking the lipid envelope this layer built.
Stratum lucidum
A thin, clear band of dead keratinocytes packed with eleidin (a keratin precursor). It is present in thick skin: palms and soles, and sometimes described on the fingertips. It is not a facial-cheek layer. If a stem asks where lucidum is found, the answer is palms and soles, not “every area you steam.”
Stratum corneum
The horny layer: stacked corneocytes (dead keratinocytes filled with keratin) plus intercellular lipids. Roughly 15–20 cell layers on typical facial skin, thicker on palms and soles. This is the visible surface you cleanse, exfoliate, and protect. The acid mantle sits on it — sebum, sweat, and lipids at about pH 4.5–5.5 — and helps inhibit pathogens while keeping the barrier slightly acidic. Desquamation is the shedding of these dead cells. This is the only epidermal layer Illinois 3A-1 treats as the esthetician’s work surface.
Keratinization
Keratinization (cornification) is the programmed trip from basal mitosis to a dead, keratin-filled corneocyte. Keratinocytes are born in the germinativum, fill with keratin as they rise, lose their nucleus, become the horny layer, and eventually desquamate. That process is why a controlled superficial enzyme or acid can even the look of the surface: it loosens the already-dead cells that were going to shed. It is not a license to accelerate mitosis in the basal layer or to “resurface living tissue.”
Why Illinois Scope Is the Stratum Corneum
Section 3A-1 defines esthetics as specified cosmetic acts when done for cosmetic or beautifying purposes and not for the treatment of disease or of a muscular or nervous disorder. The living-layer sentence is the exam filter: techniques, products, and devices that affect the stratum corneum may be esthetics; techniques, products, and practices intended to affect the living layers of the skin are not, unless a physician delegates the work and it is not held out as esthetics.
Public Act 104-0134 (effective August 1, 2025) confirmed microdermabrasion, hydrodermabrasion, and dermaplaning as stratum-corneum skin-care methods when used for cosmetic purposes. Map the in-scope surface services to the horny layer:

Superficial peels versus anything that targets living epidermis
Superficial chemical peels that target the stratum corneum remain within Illinois esthetics. Medium and deep peels intended to reach living epidermis or dermis are on IDFPR’s Statement on Prohibited Practices (04/06/2026) as the practice of medicine (“chemical peels except superficial”). Do not sell a “medium peel,” a living-layer acid, or a wounding protocol as a facial upgrade.
If a device, acid, or needle is intended to injure living epidermis so the dermis remodels, it is not Illinois esthetics even when a manufacturer card says “facial.” Intention plus living-layer target is the test, not the marketing name.
Desquamation Timeline
Young-adult epidermal turnover is about 28 days from basal mitosis to surface shed. The clock lengthens with age (often described in the 45–60-day range on mature skin). That is why weekly gentle exfoliation can even the look of the corneum, and why daily acid stacking is not “faster results” — it is barrier damage. You do not speed basal mitosis with an esthetics license. You remove what is already dead, on a schedule the acid mantle can tolerate.
The exam trap is consistent: naming the germinativum correctly is required. Treating the germinativum is forbidden as esthetics. When the stem asks which layer an Illinois cleanse, superficial peel, microdermabrasion, hydrodermabrasion, or dermaplaning is meant to affect, the answer is the stratum corneum.
