NIC Domain I.B.1 is levels of infection control — sanitation, disinfection, and sterilization. Learn the national three-rung ladder first, then overlay Illinois 68 Ill. Adm. Code 1175.115. The exam will mix the two. Illinois also tests this under 1175.710 as sanitary procedures. A candidate who can recite “clean, disinfect, sterilize” but then soaks tweezers in hand antiseptic, or who wipes a facial chair only at closing, misses both the NIC science and the Illinois rule.

The Three NIC Levels

Cleaning (often still called sanitation on NIC items) is the lowest level. It is the removal of visible debris — dirt, oil, product film, dead skin, wax — usually with soap or detergent and water. Cleaning reduces the number of microbes by physically washing them away. It does not destroy remaining pathogens to a disinfection standard. Cleaning is mandatory before disinfection because organic debris inactivates many chemical disinfectants. A tweezer coated in wax that is dropped into a wet jar has not been disinfected, no matter how long it soaks.

Disinfection is the spa standard for nonliving (nonporous) surfaces and implements. A proper disinfectant destroys most bacteria, fungi, and viruses. It does not reliably destroy bacterial spores. Illinois 1175.115(a)(2) defines disinfect as “the use of a chemical agent that eliminates harmful bacteria, fungi and viruses on surfaces.” That is the same idea in Illinois words. Disinfection is what you owe every reusable metal extractor, tweezer, and stainless spatula after you have washed the debris off.

Sterilization destroys all microbial life, including spores. The usual method is a steam autoclave (moist heat under pressure). Dry-heat ovens also sterilize when used at the manufacturer’s time and temperature. Sterilization is required for instruments that enter sterile tissue in medicine. For intact-skin esthetics — tweezers, metal spatulas, stainless extractors used on unbroken skin — Illinois requires clean then disinfect (or discard). Autoclaves are excellent extra control; they are not a substitute for skipping the Illinois hospital-grade disinfectant on the facial chair, and they are not required for every intact-skin tool the way a surgical pack is required in an operating room.

The Antiseptic Trap

An antiseptic is a chemical formulated for living skin. It reduces microbes on tissue. It is not a disinfectant for tools. A classic NIC sample item asks what level of decontamination a product containing antiseptic reaches. The keyed answer is sanitation, not disinfection and not sterilization.

Why the trap works: students hear “kills germs” and jump to disinfection. Disinfection is a process for nonliving surfaces. If you soak tweezers in an antiseptic, or wipe a comedone extractor with a skin toner “because it is antibacterial,” you have not met the disinfection standard. Conversely, you never use an EPA-registered hospital disinfectant as if it were a toner. Disinfectants injure living tissue. Keep the pair straight:

  • Antiseptic / hand sanitizer / soap on skin = sanitation (or antisepsis) of living tissue.
  • EPA-registered disinfectant on implements and chairs = disinfection of nonliving surfaces.
  • Autoclave = sterilization, including spores.

Illinois language helps you keep the categories from collapsing. Clean is physical removal. Disinfect is a chemical agent on surfaces. Hospital-grade disinfectant is an EPA-registered hospital-level product that is a bactericide, a virucide, and a fungicide. Hand washing uses soap and water or an equally effective cleanser or waterless hand sanitizer — that is skin hygiene, not tool disinfection.

Illinois “Clean” Versus “Disinfect”

Do not treat the NIC ladder and 1175.115 as identical sentences. NIC still says sanitation / disinfection / sterilization. Illinois writes four definitions in 1175.115(a):

  • Clean — free of soil, dust, contaminants, or impurities; recently laundered and unused; or the removal of soil, dust, and similar matter by washing, sweeping, clearing away, or any other appropriate method.
  • Disinfect — use of a chemical agent that eliminates harmful bacteria, fungi, and viruses on surfaces.
  • Disinfectant — a chemical agent that eliminates harmful bacteria, fungi, and viruses.
  • Hospital-grade disinfectant — a disinfectant registered with the Environmental Protection Agency (EPA) as a hospital-level disinfectant that performs the functions of bactericides (kill harmful bacteria), virucides (kill pathogenic viruses), and fungicides (destroy fungus).

A product is not “hospital grade” in Illinois because it smells strong, is labeled “antibacterial,” or was purchased from a medical-supply catalog. It must be EPA-registered as hospital-level and must claim all three functions. That Illinois definition is stricter, and more specific, than a generic textbook line about “salon disinfectant.”

Manicure tables, work stations, and facial chairs shall be cleaned and disinfected with a hospital-grade disinfectant before and after serving each client (1175.115(b)(7)). The before-and-after pair is Illinois-specific. NIC science tells you disinfection is for nonliving surfaces. Illinois tells you the chair is one of those surfaces, twice per client. Wiping only after the client leaves, or only at opening, fails the rule even if the sheet was disposable.

1175.115(b)(3) is the per-client implement rule: all tools, implements, and items that come in direct contact with a client shall be cleaned and disinfected or disposed of after each client. New or cleaned-and-disinfected tools are stored separately from everything else. Storage drawers for clean tools must themselves be clean, free of hair, and used only for clean tools. A mixed drawer of “clean-ish tweezers and yesterday’s extractors” is a level-of-control failure, not a storage preference.

Single-Use and Porous Items Skip Disinfection Because They Are Discarded

Disinfection is for nonporous, multi-use items that can survive a chemical process. Porous items absorb liquid. You cannot reliably disinfect an emery board, a foam sponge, a wooden spatula, a cotton round, or a pumice stone. Soaking a porous object in disinfectant does not convert it into a multi-use tool; it creates a contaminated sponge that cannot be made safe.

Illinois 1175.115(b)(5) lists items designed for single use that shall be disposed of after each use: orangewood sticks, cotton, gauze, neck strips, nail wipes, tissues, sponges, paper towels, wooden applicators and spatulas, emery boards or porous nail files, buffer blocks, pumice stones, sanding bands or sleeves, and disposable nail bits. 1175.115(b)(11) is the catch-all: any article that contacts the client’s skin and cannot be cleaned or disinfected is disposed of immediately after use.

That is why “I’ll soak the wooden stick in alcohol” is a wrong exam answer. Alcohol on a porous stick is not Illinois disinfection, and the stick is on the discard list. Single-use items skip disinfection because the control method is discard, not a second chemical step. The same logic applies to a facial sponge used to remove cleanser: it touched the client’s skin, it is porous, and it goes in the trash — even if the facial was only a “mini.”

What Each Level Is For in an Illinois Facial Room

Walk one client through the ladder:

  • Hands: wash with soap and water or equally effective cleanser/waterless sanitizer before and after each client (1175.115(b)(14)) — sanitation/antisepsis of living skin.
  • Metal tweezers and stainless extractors: wash off debris (clean), then fully disinfect with an EPA-registered hospital-level product, then store in a clean-only drawer.
  • Facial chair and workstation: hospital-grade disinfectant before and after the client.
  • Towels, sheets, gowns, head coverings: launder (clean) and store covered; use fresh for each client. That is sanitation of textiles, not tool disinfection.
  • Cotton, gauze, wooden spatulas, sponges: discard. No disinfection step, because they cannot be disinfected.

Lancets and other items that would require true sterility sit outside ordinary esthetics practice. If an item contacts blood, you follow a blood-exposure protocol rather than a “wipe and reuse” protocol. If a question mixes “antiseptic on the extractor” with “hospital-grade on the chair,” pick the Illinois chair rule and the NIC antiseptic-is-not-disinfection rule. Those two traps carry a large share of Domain I.B.1 items, and they are the difference between a textbook ladder and a license you can keep.
