1. Communication and Hazard Reporting (AM.I.L.K6)

Lack of communication is the first of the Dirty Dozen for a reason: most maintenance work crosses at least one boundary — between technicians, between shifts, between maintenance and flight crew, between the hangar and the stores room.

Rules that hold up:

  • Confirm receipt, not just transmission. A message is not communicated until the receiver plays it back in their own words.
  • Write it down. Verbal information degrades within minutes and disappears at shift change.
  • Be specific about state, not just action. "I removed the actuator" is incomplete; "actuator removed, hydraulic lines capped, electrical connector capped and stowed, three attach bolts in a bag taped to the bracket, torque not yet applied to anything" is a state.
  • Use standard phraseology on the radio and standard hand signals on the ramp. Improvisation kills.
  • Close the loop on discrepancies. A verbal "I'll take care of it" is not a deferral, a placard, or a record entry.

Reporting hazards and the risk of selective reporting (AM.I.L.R1)

Selective reporting — reporting the hazards that are convenient and omitting the ones that are embarrassing, expensive, or self-implicating — is the specific risk the ACS names. It is corrosive because it corrupts the data the safety system runs on: the organization sees a filtered picture and allocates its defences against the wrong hazards.

Filing a Malfunction or Defect Report (AM.I.L.S1)

The skill element asks you to actually file one. FAA Form 8010-4 captures: aircraft registration and make/model/serial; the component or part make, model, part number and serial number; time since new, since overhaul, and since inspection; a specific description of the difficulty including how it was discovered; the stage of operation or inspection during which it was found; and the submitter's identity. The value of the report lies in the specificity: "cracked" is useless; "3/8-inch crack in the aft flange of the left main gear trunnion, found during eddy current inspection at 4,120 hours since new" is what generates an Airworthiness Directive.


2. Shift and Task Turnover (AM.I.L.K9, S2)

Turnover is where the most maintenance errors are seeded, because it combines a memory boundary, a communication boundary, and usually the circadian low.

A conforming turnover is written, verbal, and walked.

  1. Written. A turnover log or task card annotation stating exactly what was done, what was not done, and what state the aircraft is in.
  2. Verbal. A face-to-face briefing where the incoming technician asks questions and plays back their understanding.
  3. Walked. Go to the aircraft together. Physically show the open panels, the disconnected lines, the installed but untorqued fasteners, and the tools in place.

Every turnover must positively answer these:

  • What work is complete, and is it signed for?
  • What work is in progress, and at exactly which step?
  • What is disconnected, removed, deactivated, or capped?
  • What fasteners are installed but not torqued, and what is not safetied?
  • What panels and access covers are open?
  • What is installed but not tested?
  • What tools, stands, jacks, or equipment are on or in the aircraft?
  • What parts are on order and what is the aircraft waiting on?
  • What hazards exist — energized systems, pressurized accumulators, jacked aircraft, fuel in an open tank?

The single most dangerous turnover item is the untorqued fastener, because a fastener that is installed looks complete. If a task cannot reach a natural stopping point, the standard defence is to back the fastener out and leave the joint visibly incomplete, or to place a highly visible warning tag on the control or the panel.


3. Teamwork and Leadership (AM.I.L.K7)

Maintenance Resource Management (MRM) is the maintenance analogue of flight-crew resource management. Its core content:

  • Brief the job before it starts. Who does what, who inspects what, what data is being used, what the finish criteria are.
  • Assertiveness is a technical skill. State the observation, state the concern, state the proposed action, and get an explicit decision. If the answer is unsatisfactory, escalate — the ACS lists lack of assertiveness as a Dirty Dozen precursor precisely because deference has killed people.
  • Leadership is situational. The person with the most relevant knowledge should be leading that portion of the task, regardless of rank.
  • Independent inspection means independent. The second signature exists to break the first person's expectation bias. Signing an item you did not personally verify is not a paperwork shortcut; it removes an entire layer of defence and, if the entry is knowingly false, it is a violation of 14 CFR § 43.12, which prohibits fraudulent or intentionally false entries in maintenance records and provides for suspension or revocation of the certificate.

4. Professionalism and Integrity (AM.I.L.K8)

The mechanic's signature is the entire regulatory basis for an aircraft's return to service. There is no downstream inspector for most general-aviation work. Professionalism in this subject means specific, testable behaviours:

  • Sign only for what you actually did and actually verified.
  • Use current data. Check the revision level before you start, not after you finish.
  • Do not accept or apply pressure to compress an inspection or defer a step.
  • Report your own errors. A self-reported error is a safety input; a concealed error is a latent condition waiting for a flight.
  • Know the boundary of your certificate. Refuse work outside your rating, your recency under § 65.83, or your demonstrated ability under § 65.81.
  • Records integrity is regulatory. Beyond § 43.12, falsification is grounds for certificate action and, in serious cases, criminal exposure.

5. Event Investigation (AM.I.L.K3)

When a maintenance error does occur, the organization's job is to find the contributing factors, not the culprit.

The industry standard tool is the Maintenance Error Decision Aid (MEDA), developed by Boeing and widely adopted. Its premises are explicit and worth memorizing because they are the philosophy the ACS is testing:

  1. Maintenance technicians do not cause errors on purpose.
  2. Errors result from a series of contributing factors, most of which are under management control.
  3. Most contributing factors are identifiable and manageable.

A MEDA-style investigation walks through: the event (what happened to the aircraft), the maintenance error (what specific action or omission led to it), the contributing factors (information, equipment and tools, aircraft design and configuration, the job or task itself, technical knowledge and skills, individual factors including fatigue, environment and facilities, organizational factors, leadership and supervision, communication), and finally corrective actions aimed at those factors.
