Section 1.2: Microbiology & Pathogens

Microbiology is the study of microscopic organisms, commonly referred to as microbes or microorganisms. For estheticians, an advanced understanding of microbiology is the bedrock of infection control. Because estheticians work directly with the skin, lashes, and brows, they are in constant physical contact with clients. This makes the spa environment a prime location for the transmission of infectious pathogens if proper sanitization, disinfection, and sterilization procedures are not strictly maintained.

Pathogenic vs. Nonpathogenic Microorganisms

Microorganisms are divided into two primary categories:

  1. Nonpathogenic Bacteria: These are harmless, and in many cases, beneficial organisms that perform useful functions. In the human body, nonpathogenic bacteria assist in digestion, support the immune system, and keep pathogenic microbes at bay. Approximately 70% of all bacteria are nonpathogenic.
  2. Pathogenic Bacteria: These are harmful microorganisms that cause disease, infection, or toxins when they invade plant or animal tissue. Protecting clients from pathogenic bacteria is the primary reason the California Board enforces strict health regulations.

Morphological Classification of Pathogenic Bacteria

Bacteria are single-celled microorganisms that exist in three basic shapes, determined by their physical structure:

  • Staphylococci: Pus-forming bacteria that grow in clusters. They cause abscesses, pustules, boils, and staph infections (MRSA), and are spread through skin-to-skin contact or dirty tools.
  • Streptococci: Pus-forming bacteria arranged in curved lines resembling beads. They cause strep throat and blood poisoning.

Bacterial Life Cycle: Active vs. Inactive (Spore-Forming) Stages

Bacteria grow and reproduce through a two-stage life cycle:

  • Active Stage: In warm, dark, damp, and dirty environments, bacteria thrive. They grow and reproduce by dividing into two new cells via a process called binary fission. Under ideal conditions, a single bacterium can divide every 20 to 30 minutes.
  • Inactive (Spore-Forming) Stage: When environmental conditions become unfavorable (extreme heat, cold, dryness, or in the presence of disinfectants), certain bacteria (primarily bacilli) form a tough outer coating known as a spore. In this spore state, the bacteria can remain dormant for years, resisting disinfection, heat, and cold. Once conditions improve, the spores resume active growth.

Exam Trap: Standard EPA-registered chemical disinfectants used in spas do not kill bacterial spores. Spores can only be destroyed through sterilization processes, such as pressurized steam autoclave heating.

Viruses in the Spa Environment

A virus is a submicroscopic, parasitic particle that infects and resides in the cells of biological organisms. Unlike bacteria, viruses cannot survive or reproduce without a host cell.

  • Hepatitis: A bloodborne virus causing inflammation and damage to the liver. Hepatitis B is the hardest to kill on surfaces and is highly infectious. Vaccines are available for Hepatitis B.
  • HIV: The virus that causes AIDS. HIV is spread through bloodborne pathogens and bodily fluids. It impairs the immune system, making the body susceptible to opportunistic infections.
  • HPV: A common virus that causes warts and can infect skin and mucous membranes. It is easily transmitted via skin-to-skin contact.

Fungi and Tinea Infections

Fungi are single-celled or multicellular plant-like organisms, including molds, mildews, yeasts, and rusts. They obtain nutrients by absorbing organic matter from their host.

  • Tinea Infections: Fungal infections of the skin. Tinea pedis (athlete's foot) is a fungal infection of the feet. Tinea barbae (barber's itch) is a superficial fungal infection of the bearded area of the face and neck. Tinea corporis (ringworm) presents as circular, red, itchy lesions on the body.

Board Rule: Under California regulations, an esthetician must never perform services on a client displaying signs of a fungal skin infection. The client must be referred to a physician.

Parasites: Scabies and Head Lice

Parasites are organisms that grow, feed, and shelter on or in another organism (the host) while contributing nothing to the survival of that host.

  • Scabies: A highly contagious skin disease caused by the itch mite (Sarcoptes scabiei), which burrows under the skin to lay eggs. It causes intense itching and red, pimple-like rashes.
  • Head Lice (Pediculosis Capitis): Tiny insects that infest the scalp and hair, laying eggs (nits) near the hair shaft.

Transmission Warning: Parasites are transmitted via direct contact or sharing contaminated items like towels, linens, headbands, or brushes.

Vectors of Transmission in Spa Settings

Pathogens can be transmitted in a spa through several pathways:

  • Direct Contact: Touching an infected client's skin, open lesions, or bodily fluids (e.g., performing extraction without gloves).
  • Indirect Contact: Touching contaminated surfaces or tools (e.g., using a spatula that was double-dipped, or using towels that were not washed).
  • Airborne Transmission & Respiratory Droplets: Inhaling pathogens suspended in the air or expelled during coughing, sneezing, or talking.
