1A 58-year-old man who smokes reports retrosternal pressure that appears when he climbs stairs, lasts about 3 minutes and goes away with rest. His resting ECG is normal. Which health problem best explains his symptoms?
A.Unstable angina
B.Acute pericarditis
C.Stable angina pectoris
D.Gastroesophageal reflux disease
Explanation: Chest discomfort that is predictably brought on by exertion and relieved by a few minutes of rest is chronic stable angina. A normal resting ECG is common between episodes, so the history is what identifies the problem.
2A 47-year-old asymptomatic woman has a fasting plasma glucose of 131 mg/dL, and a repeat test on another day is 128 mg/dL. Under NOM-015-SSA2-2010, how should she be classified?
A.Diabetes mellitus, because fasting plasma glucose is 126 mg/dL or more on two occasions
B.Prediabetes (glucosa anormal de ayuno), because both values are below 140 mg/dL
C.Normal glucose metabolism until an HbA1c of 7.0% or more is documented
D.Undetermined until a 75 g glucose load gives a 2-hour value of 200 mg/dL or more
Explanation: NOM-015-SSA2-2010 diagnoses diabetes with a fasting plasma glucose of 126 mg/dL or more, a value of 200 mg/dL or more 2 hours after a 75 g glucose load, or a casual glucose of 200 mg/dL or more with classic symptoms. Without unequivocal symptoms, the result is confirmed by repeating the test on a different day, as happened here.
3A 52-year-old man has newly diagnosed type 2 diabetes with an HbA1c of 7.8%, an eGFR of 78 mL/min/1.73 m² and no cardiovascular or kidney disease. Along with diet and exercise, which drug is the usual first choice?
A.Glibenclamide
B.NPH insulin at bedtime
C.Pioglitazone
D.Metformin
Explanation: Metformin is the usual first-line drug for type 2 diabetes: it lowers HbA1c effectively, does not cause hypoglycemia on its own, is weight-neutral and is inexpensive. It is avoided when the eGFR is below 30 mL/min/1.73 m².
4A 63-year-old man with type 2 diabetes on metformin had a myocardial infarction two years ago. His HbA1c is 8.2%, eGFR 55 mL/min/1.73 m² and urine albumin-to-creatinine ratio 210 mg/g. Which added drug class has shown reductions in cardiovascular events and in the progression of kidney disease?
A.A sulfonylurea such as glimepiride
B.An SGLT2 inhibitor such as dapagliflozin
C.A DPP-4 inhibitor such as sitagliptin
D.An alpha-glucosidase inhibitor such as acarbose
Explanation: In outcome trials, SGLT2 inhibitors reduced heart-failure hospitalization and cardiovascular events and slowed the loss of kidney function in people with albuminuric chronic kidney disease. Current guidelines recommend them in type 2 diabetes with atherosclerotic disease or CKD, independently of the HbA1c.
5A 2-year-old has had a runny nose for two days and tonight developed a barking cough, a hoarse voice and inspiratory stridor that worsens with crying. His temperature is 37.9 °C. What is the most likely diagnosis?
A.Acute epiglottitis
B.Viral laryngotracheitis (croup)
C.Acute bronchiolitis
D.Foreign-body aspiration
Explanation: Croup, usually caused by parainfluenza virus, follows a coryzal prodrome and produces subglottic swelling with a barking cough, hoarseness and inspiratory stridor that worsens with agitation.
6A 2-year-old with croup has stridor only when crying, no chest-wall retractions at rest and normal oxygen saturation. What is the recommended treatment?
A.Oral amoxicillin for 7 days
B.Nebulized salbutamol every 4 hours
C.Hospital admission for hourly nebulized epinephrine
D.A single oral dose of dexamethasone
Explanation: Mild croup is treated with a single oral dose of dexamethasone, which reduces airway swelling, shortens symptoms and lowers return visits. The child can then be managed at home with clear warning signs for the family.
7A 45-year-old woman has community-acquired pneumonia. She is oriented, her urea is normal, her respiratory rate is 22/min and her blood pressure is 124/78 mmHg. What is her CURB-65 score and the usual care setting?
A.0 points; she can usually be treated as an outpatient
B.1 point; she needs a short hospital admission
C.2 points; she needs admission to a hospital ward
D.3 points; she should be assessed for intensive care
Explanation: CURB-65 gives one point each for confusion, urea above 7 mmol/L, a respiratory rate of 30/min or more, a systolic pressure below 90 or diastolic of 60 mmHg or less, and age 65 or older. She meets none, so the score is 0 and outpatient oral treatment is usually appropriate.
8A 27-year-old non-pregnant woman has had dysuria, frequency and suprapubic pain for one day, with no fever or flank pain. Which is an appropriate first-line oral treatment for this uncomplicated acute cystitis?
A.Ciprofloxacin for 14 days
B.Amoxicillin for 10 days
C.Nitrofurantoin for 5 days
D.Cefixime for 14 days
Explanation: Nitrofurantoin for 5 days, or a single dose of fosfomycin, is first-line for uncomplicated cystitis in non-pregnant women. These drugs keep good activity against Escherichia coli and spare antibiotics needed for more serious infections.
9A 30-year-old woman has a fever of 38.9 °C, right flank pain, costovertebral angle tenderness and dysuria. Before starting antibiotics, which test should be obtained?
A.Contrast-enhanced abdominal CT
B.Cystoscopy
C.Serum procalcitonin alone
D.Urine culture with susceptibility testing
Explanation: Acute pyelonephritis is diagnosed clinically, but a urine culture taken before antibiotics identifies the organism and its susceptibility so that empirical therapy can be adjusted. Resistance to common oral agents is frequent in community Escherichia coli.
10A 34-year-old woman has weight loss, heat intolerance, tremor, a diffuse painless goiter with a bruit and bilateral proptosis. TSH is suppressed and free T4 is high. What is the most likely cause?
A.Toxic multinodular goiter
B.Graves disease
C.Subacute (de Quervain) thyroiditis
D.Exogenous levothyroxine intake
Explanation: Graves disease is caused by antibodies that stimulate the TSH receptor. A diffuse goiter with a bruit plus ophthalmopathy is specific for it among the causes of hyperthyroidism.

The disciplinary section has 140 items: Abordaje clínico (47), Promoción de la salud (44) and Fundamento de las decisiones médicas (49). The transversal section has 60 Spanish reading and indirect-writing items. It is given in two 4.5-hour sessions on the same day, and about 15% of the items are unscored pilot items.

What can I bring and how can I take it?

A non-programmable scientific calculator is allowed. The exam is given on paper or online at an institutional site, or remotely through Examen desde casa. In 2026 the individual Examen desde casa costs MXN 1,885, with sittings on 23-26 March, 31 August-3 September and 7-10 December 2026.

How is the EGEL Plus MEDI scored?

Each area and section is reported on the Índice Ceneval from 700 to 1300: 700-999 Aún no satisfactorio, 1000-1149 Satisfactorio and 1150-1300 Sobresaliente. Satisfactorio or better in the disciplinary section earns at least a Testimonio de Desempeño Satisfactorio, while a Testimonio de Desempeño Sobresaliente requires Sobresaliente in the disciplinary section and at least Satisfactorio in Lenguaje y Comunicación.

Why is this practice bank in English if the official exam is in Spanish?

The official Ceneval exam is in Spanish. This bank is an independent English-language study adaptation with four-option questions, not an official translation or format simulation, so you can review the medical content. Official Mexican names (NOM, expediente clínico, Vida Suero Oral, Plato del Bien Comer) are kept in Spanish so they match your references. The bank covers the disciplinary areas only.
