OX marks an oxidizer, W (shown with a slash through it) marks a water reactive, and SA marks a simple asphyxiant. NFPA 704 rates acute hazards only; chronic health effects are not covered.
What do DOT hazard classes 4, 5 and 8 cover?
Class 4 is flammable solids (4.1), spontaneously combustible material (4.2) and dangerous when wet material (4.3). Class 5 is oxidizers (5.1) and organic peroxides (5.2). Class 8 is corrosive material (49 CFR 173.2).
Where does 49 CFR draw the line between a flammable liquid and a combustible liquid?
At the flash point. A Class 3 flammable liquid flashes at not more than 60 C (140 F); a combustible liquid flashes above 60 C (140 F) and below 93 C (200 F) when no other hazard class applies (49 CFR 173.120).
Where must hazmat shipping papers be kept when the driver is away from the vehicle's controls?
In a holder mounted inside the driver's-side door, or on the driver's seat (49 CFR 177.817(e)). With the driver belted in at the controls, the papers must be within immediate reach and either readily visible to a person entering the cab or in that door holder.
What does each colored section of the DOT Emergency Response Guidebook contain?

When does a DANGEROUS placard stop being an acceptable substitute?

What is a material's flammable (explosive) range?
The span of vapor-to-air concentrations between its lower and upper explosive limits that will burn if ignited. Gasoline's range is about 1.4-7.6 percent (NIOSH Pocket Guide): below the LEL the mix is too lean, above the UEL too rich, and a too-rich cloud becomes flammable as it dilutes at its edges.
What pressure is an MC-331 cargo tank built for, and what does it haul?
Design pressure of not less than 100 psig and not more than 500 psig (49 CFR 178.337-1). These single-shell, rounded-end tanks carry liquefied compressed gases such as propane and anhydrous ammonia.
How do the working pressures of DOT 406, DOT 407 and DOT 412 cargo tanks compare?
DOT 406 runs 2.65-4 psig and hauls fuels; DOT 407 is at least 25 psig with a circular cross-section and hauls chemicals; DOT 412 is at least 5 psig and hauls corrosives (49 CFR 178.346, 178.347 and 178.348).
Why is flame impingement on the vapor space of a pressure tank more dangerous than on the liquid space?
Liquid inside carries heat away and keeps the wetted steel below failure temperature; the unwetted shell above the liquid line overheats, loses strength and can tear, letting the superheated liquid flash to vapor at once as a BLEVE. Rising internal pressure and a relief valve that grows louder or higher-pitched are signals to withdraw.
How do you tell a cryogenic MC-338 tank from a compressed-gas tube trailer?
An MC-338 is a large, heavily insulated tank with a rear cabinet of valves and piping that may show frost or vent vapor; it carries refrigerated liquids that boil below -90 C (-130 F), such as liquid oxygen, nitrogen and argon (49 CFR 173.115). A tube trailer is a bundle of long horizontal cylinders with the valves grouped at one end, carrying non-liquefied gases such as hydrogen or helium at thousands of psi.
What volume separates a small spill from a large spill in the ERG green pages?
208 liters (55 US gallons). At or below that is a small spill, typically one drum, a small cylinder or a leak from a large package; more than that is a large spill and carries longer distances.
What is the difference between the ERG's initial isolation distance and its protective action distance?
The initial isolation distance is the radius of a zone surrounding the spill in all directions. The protective action distance is the downwind distance within which people should be evacuated or sheltered in place.
When does the ERG tell you to double the green-page distances?
For an instantaneous release of a package's entire contents, such as catastrophic failure, sabotage or terrorism. The tabulated distances assume a progressive release rather than a one-shot loss of containment.
What are the three hazmat control zones and what happens in each?
The hot zone is the contaminated area, with entry limited to responders in proper PPE. The warm zone holds the contamination reduction corridor. The cold zone holds command, staging and support and must stay clean.
From what position should Operations responders approach and stage at a release?
Upwind, uphill and upstream, so vapor, gravity and liquid runoff all carry the product away from crews. Reassess the position and move whenever the wind shifts.
Which direction should the public inside the initial isolation zone be moved?
Crosswind, perpendicular to the wind direction, and away from the spill, exactly as the ERG's Table 1 instructions direct. Moving people downwind walks them into the plume.
When is sheltering in place the better protective action?
When the vapor cloud will pass quickly, when evacuating would route people through the plume, or when the population cannot be moved fast enough, as in hospitals and detention facilities.
Who becomes the individual in charge of the incident command system at a hazmat release?
The senior emergency response official on scene. Initially that is the senior officer on the first-due apparatus, and the role passes up the previously established line of authority as more senior officers arrive (29 CFR 1910.120(q)(3)(i)).
What respiratory protection does OSHA require while an inhalation hazard is unresolved?
Positive-pressure self-contained breathing apparatus. Responders keep it until the individual in charge of the ICS determines through air monitoring that a lower level will not cause hazardous exposure (29 CFR 1910.120(q)(3)(iv)).
How does OSHA require work to be staffed in hazardous areas of an emergency scene?
Using the buddy system, in groups of two or more, with the number of responders in areas of potential or actual exposure limited to those actively performing emergency operations (29 CFR 1910.120(q)(3)(v)).
What must be standing by while crews operate in a hazardous area?
Back-up personnel with equipment ready to provide assistance or rescue, plus advance first aid support personnel with medical equipment and transportation capability (29 CFR 1910.120(q)(3)(vi)).
What authority does the designated safety official have at a hazmat incident?
The IC must designate one, and when activities are judged to be an IDLH or imminent danger condition, the safety official has authority to alter, suspend or terminate those activities (29 CFR 1910.120(q)(3)(vii) and (viii)).
What does NFPA 470 job performance requirement 7.6.1 require of an Operations responder?
Evaluate and report the progress of the assigned defensive actions: compare what is actually happening against the action plan and report the result to command so tactics can be continued or changed.
Which HAZWOPER emergency-response-plan element covers what happens after a hazmat incident ends?
'Critique of response and follow-up' (29 CFR 1910.120(q)(2)(x)). In practice that means a debriefing, a post-incident analysis, and documented exposure records for every responder, because health effects can surface long after the scene is closed.
What is emergency decontamination?
Rapid removal of contaminant when a life is at risk and there is no time for a formal corridor, usually a high-volume, low-pressure water flush. NFPA 470 makes it a core Operations job performance requirement (7.5.1), not a mission-specific add-on.
How does technical decontamination differ from emergency decontamination?
Technical decon is a planned, multi-station process for responders and their equipment with controlled entry and exit points. Emergency decon gives up that thoroughness for speed in order to stop an exposure immediately.
What is mass decontamination for?
Reducing contamination on large numbers of exposed civilians quickly, usually through apparatus-based water corridors. Throughput matters more than thoroughness because the objective is to cut dose fast.
Which single step removes the most contaminant from a victim?
Removing clothing. Disrobing first and then flushing with water strips away the bulk of the contaminant, which is why clothing removal leads nearly every gross decontamination procedure.
What defines EPA/OSHA Level A protection?
A totally-encapsulating chemical-protective suit worn with positive-pressure full-facepiece SCBA, or a positive-pressure supplied-air respirator with escape SCBA. It gives the greatest skin, eye and respiratory protection (29 CFR 1910.120 Appendix B).
What is the difference between Level B and Level A?
Level B keeps the same positive-pressure SCBA but substitutes hooded, non-encapsulating chemical-resistant clothing. It is chosen when the highest respiratory protection is needed but a lesser level of skin protection is acceptable.
What must be true before Level C protection can be used?
The air contaminants must be identified and their concentrations measured, an air-purifying respirator must exist that removes them, exposed skin must not be affected, and all criteria for APR use must be met (OSHA Appendix B).
Why is Level C never acceptable in an oxygen-deficient atmosphere?
Air-purifying respirators filter the air but supply no oxygen. OSHA Appendix B calls for Level B or better whenever the atmosphere contains less than 19.5 percent oxygen.
Which NFPA standard now covers vapor-protective and liquid splash-protective ensembles?
NFPA 1990 (2022), which consolidated NFPA 1991 (vapor-protective, EPA Level A), NFPA 1992 (liquid splash-protective, Level B) and NFPA 1994. OSHA's Appendix B still cites the older standard numbers.
How do damming, diking, diverting and retention differ?
A dam blocks a waterway to hold or slow the product; a dike is a barrier built to keep product from spreading; diverting steers product away from an exposure; retention holds it in a pit or basin for later recovery.
What is the difference between vapor suppression and vapor dispersion?
Vapor suppression blankets the liquid, usually with foam, so it gives off less vapor. Vapor dispersion uses water fog or ventilation to move and dilute vapor that has already escaped.
Why does closing a remote valve shut-off count as a defensive Operations task?
Because the responder operates a control built to be reached without contacting the leak — such as the remote emergency shutoff on an MC-331 cargo tank or a facility emergency stop — rather than working at the point of release. NFPA 470 section 9.6 lists it with damming, diking, diversion, retention, absorption/adsorption, dilution, vapor suppression and vapor dispersion.
What oxygen readings make an atmosphere hazardous?
Below 19.5 percent or above 23.5 percent by volume (29 CFR 1910.146); normal air is 20.9 percent. Monitor in the order radiation, oxygen, flammability, then toxicity — oxygen before the LEL channel, because catalytic-bead sensors under-read without enough oxygen.
At what flammable-gas reading does OSHA call an atmosphere hazardous?
More than 10 percent of the material's lower flammable limit (29 CFR 1910.146). Treat that as an early warning threshold for withdrawal, not as a safe working concentration.
What does a photoionization detector's ppm reading actually represent?
The total of all vapors the lamp can ionize, reported as isobutylene-equivalent ppm — not the identity or true concentration of any single chemical. Apply the correction factor for a known compound, and remember that gases with an ionization potential above the lamp's energy, such as methane with a 10.6 eV lamp, never register.
How does an IDLH value differ from a PEL?
IDLH is a concentration that poses an immediate threat to life, irreversible or delayed harm, or an inability to escape (29 CFR 1910.120); a PEL is OSHA's legal limit for routine exposure, usually an 8-hour time-weighted average (29 CFR 1910.1000). The NIOSH Pocket Guide lists both, plus NIOSH RELs, for each chemical.
Frequently Asked Questions
How many questions are on the NFPA 470 Hazmat Operations written exam?
It depends on who certifies you. NFPA writes the standard but does not administer exams, so each Pro Board or IFSAC accredited entity builds its own test. The Kansas Fire & Rescue Training Institute, the Missouri Division of Fire Safety and New Brunswick's FCPNB each use 100 multiple-choice questions with a 70% minimum score, and Kansas and Missouri allow two hours. Other entities publish their own counts, so check your certifying entity's candidate handbook.
Who certifies Hazmat Operations, and what do Pro Board and IFSAC actually do?
Certification is issued by state fire academies and other public-sector certifying entities. The Pro Board (National Board on Fire Service Professional Qualifications) accredits those entities rather than training programs, stating that it accredits public sector organizations that certify emergency services responders against National Professional Qualifications System standards. IFSAC's Certificate Assembly performs a parallel accreditation role. Accreditation means the entity's testing process was audited, not that the exam itself is standardized nationwide.
Which NFPA 470 chapters are tested at the Operations level?
The Pro Board's testable-sections document lists Chapter 7 as the section required to be tested for Operations Level Responders, with Chapter 5 (Awareness) as the prerequisite. Mission-specific competencies come from Chapter 9: 9.2 PPE, 9.3 mass decontamination, 9.4 technical decontamination, 9.5 evidence preservation and public safety sampling, 9.6 product control, 9.7 detection/monitoring/sampling, 9.8 victim rescue and recovery, 9.9 illicit drug labs and 9.10 radiological. Section 9.2 is the prerequisite for the rest.
What happens if I fail the Hazmat Operations exam?
There is no national retake rule. Kansas allows two attempts per application, requires all testing within one year of the first attempt, requires pre-registration for every retest, and requires a new application and fee for a third attempt, with a strong recommendation to study or train more first. New Brunswick's FCPNB publishes three attempts spaced 15 business days apart. Check your own certifying entity's policy before you register.
What is the difference between Awareness, Operations and Technician?
Awareness personnel recognize the incident, protect themselves, secure the area and call for help; they take no further action. Operations responders act defensively - OSHA 29 CFR 1910.120(q)(6)(ii) requires at least 8 hours of training beyond Awareness and describes them as responding in a defensive fashion without trying to stop the release. Technicians act offensively, approaching the point of release to plug or patch it, and OSHA requires at least 24 hours of training equal to the Operations level plus additional competencies.
Is there a published pass rate for Hazmat Operations?
No. NFPA publishes the standard, and the Pro Board and IFSAC publish accreditation records, but none of them publishes candidate pass rates for the Operations level. Individual state fire academies score their own exams and generally do not release aggregate results, so any pass-rate figure you see online is an estimate rather than an official statistic.
