AMT RPT blueprint: Obtaining Blood Samples

Two-identifier check before a blood draw

Compare two approved patient identifiers with the requisition and the patient according to facility policy before collection. A room number is not a patient identifier; stop and resolve any mismatch before drawing.

Preferred routine venipuncture vein

The median cubital vein is commonly preferred because it is usually well anchored and away from major nerves and arteries. Site choice remains patient-specific and must follow facility policy.

Why avoid an arm with an AV fistula or graft?

It is a surgically created vascular access site. Routine venipuncture can damage the access and compromise treatment; use the other arm or obtain authorized instructions.

Tourniquet timing

Apply a tourniquet long enough to locate a vein, but release it promptly once collection begins. Prolonged application can concentrate blood components and affect results.

Order of draw: blood cultures

When blood cultures are ordered, collect them first to reduce additive carryover and to protect specimen sterility. Follow the facility's current order-of-draw procedure.

Order of draw: citrate tube

After cultures, the light-blue sodium-citrate tube is generally collected before serum and other additive tubes. It must be filled to its stated mark so the blood-to-additive ratio remains valid.

Why does additive carryover matter?

A small amount of additive transferred from one tube to the next can alter a result. Order of draw limits this preanalytical error; do not select an order solely by tube-cap color memory.

When a patient says they feel faint during venipuncture

Stop the draw safely, remove the needle, activate safety equipment, and protect the patient from falling. Follow facility policy for positioning, observation, and notifying appropriate staff.

Hematoma during a draw

A hematoma is blood collecting in tissue. Stop collection, remove the needle, apply pressure, and document or report as required. Do not continue in the same site.

If sharp, electric, or radiating pain occurs during a draw

Stop the procedure and remove the needle. Unexpected sharp or radiating pain can indicate nerve contact; do not probe or redirect blindly, and follow the facility incident process.

Needle redirection

Do not make extensive blind needle adjustments. If blood flow is not established, follow policy: release the tourniquet, remove the needle safely, and use a new device at a new authorized site if another attempt is appropriate.

Why mix additive tubes gently?

Gentle inversions distribute the additive through the specimen. Vigorous shaking can damage cells and contribute to hemolysis; use the tube manufacturer's and facility's required inversion count.

Capillary puncture: wipe away the first drop

The first drop may contain tissue fluid or residual alcohol. Wipe it away with sterile gauze, then collect the specimen according to the required capillary order of draw.

Capillary collection: excessive squeezing

Excessive pressure can dilute the specimen with tissue fluid and damage cells. Warm the approved site when appropriate, allow free flow, and recollect or escalate if a quality specimen cannot be obtained.

Why is a newborn heel puncture limited to the approved area?

Using the correct plantar surface and controlled depth reduces risk of injury to bone or nerves. Follow the device instructions and facility policy for age, weight, and puncture-site selection.

Failed venipuncture: when to ask for help

Follow the facility's limit on collection attempts. After an unsuccessful attempt, protect the patient, document as required, and ask a qualified colleague or supervisor rather than repeatedly attempting a difficult draw.

Specimen integrity

Specimen integrity means the sample still accurately represents the patient for its intended test. Correct tube, fill, mixing, labeling, storage, transport, and timing all protect it.

Hemolysis

Hemolysis is the rupture of red blood cells, releasing intracellular contents into the specimen. It can interfere with testing and may result from poor collection, rough handling, or improper transport.

Serum vs. plasma

Serum is the liquid remaining after blood clots; plasma is the liquid portion of anticoagulated blood and retains clotting factors such as fibrinogen. The ordered test determines the needed specimen type.

Why label specimens in the patient's presence?

Immediate bedside or chairside labeling connects the collected tube to the verified patient and reduces wrong-patient errors. Never prelabel empty tubes unless a controlled process explicitly permits it.

Specimen rejection

A specimen may be rejected when it cannot safely support a valid result, such as when it is unlabeled, mislabeled, clotted when anticoagulated blood is required, insufficient, or improperly transported. Follow laboratory policy rather than trying to correct the label yourself.

Timed collection

A timed collection must be drawn at the ordered interval because the clinical interpretation depends on timing. Verify the order, record the actual collection time, and promptly communicate a timing deviation.

Light-sensitive specimen

Some analytes degrade with light exposure. Protect the specimen as the laboratory or facility procedure directs and transport it promptly; do not assume every tube needs the same handling.

Chain of custody

Chain of custody documents each transfer and protects the identity and integrity of a forensic, legal, or workplace-testing specimen. Use the authorized form, seals, dates, times, and signatures exactly as required.

Prioritize a phlebotomy assignment

Use the collection list, ordered timing, specimen stability, patient condition, and facility priorities. Confirm urgent or timed draws rather than assuming arrival order is always the correct sequence.

Preanalytical phase

The preanalytical phase includes actions before laboratory analysis: patient preparation, identification, collection, labeling, handling, and transport. Many preventable laboratory errors occur here.

When an order is unclear

Do not guess. Pause the collection, verify the order through the approved clinical or laboratory channel, and document or escalate according to policy.

Before entering an isolation room

Review the isolation signage and gather supplies first, then use the required personal protective equipment. Planning reduces repeated room entries and the chance of contamination or missed collection steps.

Why document collection time accurately?

The collection time can affect interpretation, especially for timed, therapeutic-drug, or stat testing. Record the actual time, not a convenient estimate, and report deviations that matter to the order.

Professional communication with an anxious patient

Use calm, plain language to explain the collection steps, check understanding, invite questions, and avoid promises such as “this will not hurt.” Escalate clinical questions outside the phlebotomist's role.

Communicating a collection problem

Report the relevant facts promptly: patient, ordered test, what occurred, actions taken, specimen status, and who was notified. Avoid blame, assumptions, and undocumented workarounds.

Using an interpreter

Use the facility's qualified interpreter process when communication is limited. Do not rely on a child or an untrained bystander to explain a procedure, consent issue, or important instruction.

Requisition matching

Match the requisition or electronic order to the verified patient and the required collection details before drawing. Resolve discrepancies before a specimen is collected or sent.

Correcting a labeling error

Do not relabel a tube from memory or alter identifiers after the patient has left. Follow the laboratory's error and recollection policy, which may require specimen rejection and a new verified collection.

Confidentiality at a collection station

Discuss only the information needed for the task with authorized people and avoid leaving requisitions, labels, or results exposed. Privacy protection applies to spoken, printed, and electronic information.

Standard Precautions

Use infection-control practices for every patient because blood and certain body fluids may transmit infection. Core actions include hand hygiene, appropriate PPE, safe injection practices, and safe sharps handling.

Sharps safety: never recap routinely

Activate the safety device and place the used needle directly into an approved sharps container. Routine two-handed recapping creates a needlestick risk and is not a safe disposal method.

What to do after a needlestick exposure

Wash or flush the affected area promptly, report the exposure immediately, and follow the employer's exposure-control process for confidential evaluation and follow-up. Do not delay reporting while trying to identify the source yourself.

Hand hygiene moment for phlebotomy

Perform hand hygiene before patient contact and after removing gloves or contacting blood or contaminated surfaces. Gloves supplement hand hygiene; they do not replace it.

Disinfecting a venipuncture site

Clean the selected site with the required antiseptic and let it air-dry. Touching the prepared site again can contaminate it and may require repeat skin preparation.

Why not overfill a sharps container?

Overfilled containers increase the risk of sharps injury. Close and replace the container at its marked fill line or according to facility policy; never push contents down with a hand.

Blood spill response

Secure the area, use the required PPE, and clean and disinfect with the facility-approved process. Treat spills as a potential exposure hazard rather than wiping them with bare or lightly gloved hands.

Implied consent for a routine blood draw

A patient who understands the basic explanation and voluntarily presents for a routine draw may provide implied consent under applicable policy. Always confirm identity, explain the procedure, and honor a refusal or withdrawal of consent.

A competent patient refuses collection

Do not force a routine draw. Clarify the refusal if appropriate, notify the authorized clinician or nurse, document the facts, and follow policy. A patient's refusal is not permission to improvise consent.

Scope of practice

A phlebotomist collects specimens and provides procedure-level information within training and policy. Do not diagnose, interpret results, prescribe, or answer questions that require a licensed clinician; escalate them.

Minor or surrogate consent uncertainty

Verify who is authorized under law and facility policy before collecting. Rules can differ by state and circumstance, so do not rely on a family relationship alone when authority is unclear.

Why is falsifying a collection record serious?

A collection record supports patient care, specimen identity, and legal accountability. Never document a draw, consent, time, or label action that did not occur; report an error through the approved process.

Phlebotomy

Phlebotomy is the collection of blood, commonly by venipuncture. The word root phleb- refers to a vein, helping distinguish venipuncture from a capillary skin puncture.

Hemostasis

Hemostasis is the body's process of stopping bleeding through vessel response, platelet activity, and clot formation. It explains why firm post-draw pressure matters and why anticoagulant tubes are handled differently from serum tubes.

AMT's current RPT practice-exam information describes the certification examination as 200 questions with a two-and-a-half-hour time limit. The 50 cards here are a study inventory, not the exam's item count.

What score is needed to pass the AMT RPT exam?

AMT uses criterion-referenced examinations and reports the passing standard as a scaled score of 70 or greater on a 0-100 scale. It is not a promise that answering 70% of raw items correctly will always produce a passing score.

What happens if I fail the AMT RPT examination?

AMT permits a retake no sooner than 45 days after a failed attempt. You must retake the entire examination, pay the required retesting fee, and are limited to four examination attempts for one AMT certification. The same 45-day interval applies after each failed attempt; there is no separate longer wait after three failures.

Is AMT RPT the same as the HOSA Phlebotomy competition?

No. AMT states that its Registered Phlebotomy Technician certification is separate from the HOSA Phlebotomy Competition. The certification requires meeting AMT eligibility requirements and passing AMT's national RPT examination.

Do I need an employer to sponsor my AMT RPT application?

No. AMT does not require employer sponsorship. Applicants must independently establish an accepted eligibility route and submit its documentation; an employer may be involved only when verifying a work-experience route.
