1.1 Current Exam Facts

The Certified Pediatric Hematology Oncology Nurse (CPHON®) credential is the specialty certification for registered nurses who care for children, adolescents, and young adults with cancer and blood disorders. It is awarded by the Oncology Nursing Certification Corporation (ONCC), the independent certification body affiliated with the Oncology Nursing Society (ONS). ONCC partners with the Association of Pediatric Hematology/Oncology Nurses (APHON) on this credential. CPHON is accredited by the National Commission for Certifying Agencies (NCCA), which means the exam rests on a documented role-delineation study, a published test content outline, and a legally defensible passing standard — not on a commercial vendor's topic list.

If you work at the bedside on a pediatric oncology unit, in a hematology clinic, in hematopoietic stem cell transplant, in radiation or infusion, or in administration, education, research, or consultation, you are in the CPHON population. CPHON is not an advanced-practice exam. It tests basic specialty registered-nurse practice: what a competent pediatric hematology/oncology nurse knows and does after meeting ONCC's experience and continuing-education gates.

A typical candidate has already started tumor-lysis precautions on a new leukemia admission, taught a family why a central line cannot go in the bathtub, and recognized that a toddler who is 'just tired' after delayed intensification is not the same assessment problem as an adolescent's fatigue after cranial radiation. The exam expects that pediatric-specific judgment, written as four-option multiple-choice items.

What the 2026 exam looks like

The CPHON examination is a computer-based test (CBT) delivered at PSI test centers. ONCC reports more than 1,500 PSI sites worldwide. Testing is year-round in the United States, its territories, and Canada. The 2026 ONCC Test Registration Manual does not describe live remote or at-home proctoring for ONCC exams. Do not plan to sit CPHON from a kitchen table. Schedule a physical PSI center and treat travel time as part of test day.

You will answer 165 multiple-choice items. Each item is a question or incomplete statement with four options and one correct answer. Of those 165 items, 125 are scored and 40 are pretest (experimental). Pretest items are mixed throughout the form so ONCC can gather statistics for future exams. You cannot tell which items count. There is no experimental flag, no different font, and no cluster of throwaway questions at the end. Treat every item as scored.

The appointment is a 3-hour session. That session includes an approximately 15-minute computer-based tutorial before the exam starts and a short post-test survey after you finish. There are no scheduled breaks. If you leave the workstation, the clock keeps running. Use the restroom before you click into the tutorial. A pediatric nurse who is used to stepping off the floor for fifteen minutes will not get that courtesy here.

After the tutorial, a practical pacing target is about 90 to 100 seconds per item if you protect a short review buffer at the end. Flag and move. You cannot save time by skipping 'experimental' questions because you cannot identify them. The 125 scored items are what ONCC equates onto the 25–75 scale.

ONCC tests use generic drug names only. If you studied 'Oncovin' in an older protocol binder, the item will say vincristine. If you chart 'Zofran,' the item will say ondansetron. Asparaginase, methotrexate, cyclophosphamide, mesna, rasburicase, tretinoin, and filgrastim appear as generics. Brand-name fluency is a trap, not a shortcut.

How ONCC scores CPHON

ONCC uses a scaled score. The reported scale runs from 25 to 75. The passing score is 55. That 55 is not a percentage of items correct, and it is not '55 out of 125.' Different forms of the exam have slightly different raw cut scores because some forms are harder than others. Equating converts your raw number-correct into a scaled score so that a 55 on a harder form means the same competence as a 55 on an easier form.

ONCC does not currently publish a public CPHON pass rate on the credential page. Ignore third-party blogs, forum screenshots, and outdated study-guide sidebars that quote 'about 75%.' The number you must remember is the scaled passing standard of 55.

Passers see PASS on the score report. They do not receive a numeric scaled score. Failers see their numeric scaled score compared with 55, plus subject-area performance feedback so they can target a retake. You receive same-day results at the PSI center. ONCC updates its certification database the next business day by 2:00 p.m. Eastern Time. The paper certificate is mailed in 4–6 weeks. ONCC also issues a Credly digital badge you can share with employers and professional networks.

What a fail report is for

A fail is not a mystery percentage. You will see a number on the 25–75 scale next to the passing standard of 55, plus subject-area bars. Use those bars. Restudying 'everything' after a 52 wastes the only diagnostic ONCC gives you. The retake rules themselves — 90-day spacing, DoubleTake's 180-day window, and the three-fails-in-three-years pause — belong in the next section; the scoring fact to lock here is that only failers see a number.

2026 fees and DoubleTake

For 2026, the CPHON examination fee is $300 if you hold current membership in ONS or APHON, and $420 if you do not. Membership must be current at the time of application if you claim the member rate. The member discount is $120 — often enough to justify a year of APHON or ONS membership before you apply, especially if you also want journal access and conference pricing.

DoubleTake is ONCC's prepaid second-attempt option. You add $100 to the initial application. If you fail the first attempt, you may sit a second time within 180 days of that first attempt without paying a full new exam fee. If you pass the first time, the $100 is not refunded. DoubleTake is not available with FreeTake, international testing, or beta exams. It is a hedge for first-time testers who want a defined retake path, not a discount on a passing attempt.

International candidates in 2026 test only June 1–30, pay an extra $75, must apply by March 1, 2026, and need a foreign license equivalency evaluation. Those rules, plus Authorization to Test, identification, and retake limits, are covered in the next section.

Why these facts change how you study

Nurses lose CPHON points for two reasons that have nothing to do with how many acute lymphoblastic leukemia phases they can recite. First, they study adult oncology outlines or outdated domain lists and miss the current pediatric six-area outline. Second, they mismanage the 3-hour, 165-item, no-break session and burn time on items they cannot identify as pretest. This section is the logistics layer: 165 items, 125 scored, scaled 55, PSI only, generic names, $300/$420, DoubleTake $100 with a 180-day second-attempt window.

Picture a concrete test-day sequence. You arrive at PSI, complete check-in, and start the tutorial inside the 3-hour appointment. You then work 165 four-option items that mix leukemia presentation, body-surface-area dosing, typhlitis assessment, and family teaching. Forty of those items will never count, and you will never know which forty. When you finish, you complete the survey and receive a printout: PASS, or a number next to 55 plus subject-area bars. Passers wait 4–6 weeks for the paper certificate and can post the Credly badge once ONCC updates the database the next day by 2:00 p.m. Eastern Time. That is the entire scoring story. There is no published percentage pass rate to chase, and there is no home-proctor backup if weather closes your center — reschedule inside your Authorization to Test window, which the next section explains.

Keep one pediatric habit attached to these facts. When a practice item says 'the nurse prepares vincristine,' you should already be thinking milligrams per square meter, vesicant precautions, and constipation teaching — not the brand name on the vial. The exam will not translate brand names for you, and the clock will not pause while you do.
