21.1 Initiation Training and Readiness Walkthroughs
Key Takeaways
Training should prepare affected people before they perform the task.
PI attendance throughout one meeting is not a universal ICH format requirement.
A practical walkthrough can expose access, handoff and supply gaps despite complete certificates.
Initiation Training and Readiness Walkthroughs
A rigorous SIV follows a structured, multi-hour instructional agenda:
-
Protocol In-Depth Review & Scientific Objectives:
- Study background, preclinical rationale, and primary/secondary endpoints.
- Detailed walkthrough of inclusion and exclusion criteria to prevent screen failures and accidental enrollment of ineligible participants.
- Visit schedule and flow: specific testing windows (e.g., Day ), protocol-mandated fasting, and order of clinical assessments (e.g., patient-reported outcomes before invasive blood draws).
-
Safety Surveillance & Serious Adverse Event (SAE) Reporting:
- Differentiating Adverse Events (AEs), Adverse Drug Reactions (ADRs), and Serious Adverse Events (SAEs).
- The non-negotiable protocol-defined immediate reporting process: the site must report any SAE to the sponsor within the trial-specific deadline of first awareness, regardless of perceived causality or relationship to the investigational product.
- Emergency unblinding procedures: locating emergency unblinding codes in the IRT system and protocol rules for code breaking.
-
Investigational Product Management & Accountability:
- Unblinded pharmacy procedures: reconstitution, dilution, and kit numbering.
- Storage requirements, continuous temperature logging, and excursion response workflows.
- Maintenance of Master Drug Accountability Logs (DAL) and Subject Dispensing Logs.
-
Biological Sample Collection, Processing & Logistics:
- Laboratory kit inventories, the actual laboratory manual and required processing instructions.
- protocol-specified specimen conditions, storage labeling, and chain-of-custody logs.
- Appropriate packaging, trained handling and shipment under applicable requirements for the actual materials.
-
Electronic Data Capture (EDC) & Data Integrity Standards:
- Training staff on eCRF completion guidelines, ALCOA+ principles, and source documentation standards.
- Sponsor expectations for data entry turnaround times (under the trial's data-entry requirements).
- Electronic query resolution processes and investigator electronic signature requirements.
-
Investigator Site File (ISF) Organization & Delegation of Authority:
- Inspecting the regulatory binder to ensure all essential documents are filed contemporaneously.
- Finalizing the Delegation of Authority (DOA) Log: ensuring that tasks delegated by the PI align with staff qualifications, licenses, and specific protocol training.
Post-SIV Activities, Action Item Tracking & The Path to First Patient Screened
Conducting the SIV meeting does not automatically authorize the site to begin patient enrollment. Several post-visit administrative gates must be cleared:
SIV Monitoring Report and Follow-Up Letter
Within the timelines in the applicable sponsor procedures:
- SIV Report: The CRA drafts a detailed, auditable monitoring report documenting the date, location, attendees, topics trained, facility re-inspection findings, and outstanding action items. The report undergoes review and sign-off by the Clinical Project Manager.
- SIV Follow-Up Letter: The CRA issues a formal follow-up letter addressed to the Principal Investigator. The letter outlines all agreed-upon action items, specifies the responsible individual for each item, and assigns strict target completion dates.
Common Post-SIV Action Items
Start-up frequently requires resolving minor operational bottlenecks post-SIV:
- Obtaining missing system credentials for EDC, IRT, or central laboratory portals.
- Completing protocol-specific training for a secondary sub-investigator or weekend coordinator who could not attend the general SIV.
- Finalizing the calibration certification for a newly installed pharmacy backup data logger.
- Obtaining signed and dated normal reference ranges for a newly selected local backup laboratory.
Activity-specific authorisation and first screening
Once the required readiness conditions are verified, the sponsor issues the formal written Regulatory Green Light. The site is activated in the IRT system, investigational drug supplies are released for dispensing, and the clinical research team is officially authorized to begin pre-screening patient databases, consenting candidates, and screening the trial's first subject.
Verifying readiness through a practical walkthrough
Before release for the relevant activity, ask staff to describe a participant's first planned visit from contact through data entry. Identify which approved information is used, who checks eligibility, how a critical result reaches the investigator, where product is obtained and what happens if a system is unavailable. A walkthrough can expose an interface gap that a document checklist misses.
For example, staff may all have training certificates while no one has active access to the laboratory portal. If the first dose depends on a laboratory result, that access gap matters clinically. Resolve the dependent readiness issue and retain evidence. A low-impact filing action can sometimes remain open under an authorised plan, while a critical access or safety gap cannot be dismissed as paperwork.
Record what the site is authorised to do and what remains restricted. A participant-screening permission is not necessarily permission to dose, and a successful staff meeting is not authority approval. The sponsor's process should make these distinctions clear enough that the site can follow them without guessing.

The PI misses the main meeting, but a suitable separate training plan is possible. What is sound?
Follow the sponsor process, complete required investigator preparation and critical readiness checks before dependent activities.
ICH always prohibits any staff training in the PI’s absence.
Activate immediately because any coordinator can replace investigator oversight.
Backdate attendance to make the meeting complete.
