1A registered nurse in a Swedish medical ward is preparing to perform morning rounds. In accordance with the National Board of Health and Welfare regulations on basic hygiene (SOSFS 2015:10 / Vårdhandboken), which action is mandatory regarding work attire and hand hygiene?
A.Wearing a long-sleeved clinical jacket when interacting with patients who have no signs of active infection
B.Performing hand disinfection with alcohol-based hand rub only when hands are visibly soiled with organic material
C.Short sleeves, no jewellery or watches, and hand disinfection before and after contact
D.Wearing examination gloves throughout all morning patient encounters to replace the need for hand disinfection
Explanation: Swedish regulations (SOSFS 2015:10) and Vårdhandboken mandate basic hygiene routines (basala hygienrutiner) for all direct care: short-sleeved work uniforms, hair tied back if long, bare forearms with no wristwatches, rings, or false nails, and hand disinfection using alcohol-based rub immediately before and after every patient encounter. Hand rub is applied to dry hands, whereas soap and water are used only when hands are visibly soiled or contaminated with spore-forming bacteria (such as Clostridioides difficile).
2In Swedish healthcare, person-centred care (personcentrerad vård) according to the Gothenburg University Centre for Person-centred Care (GPCC) model is built upon three core routines. Which of the following best exemplifies the initiation of this model during an admission interview?
A.Eliciting the patient's narrative about daily life, resources, goals and illness experience
B.A standard checklist covering only past surgery and physical complaints
C.Delegating the care planning entirely to the patient's family members to minimize patient fatigue
D.Assigning pre-formulated standard nursing interventions based solely on the primary medical diagnosis
Explanation: The GPCC framework for person-centred care rests on three pillars: capturing the patient narrative (patientberättelsen), establishing an equal partnership, and documenting a co-created care plan (hälsoplan/vårdplan). The patient narrative explores the person's unique subjective experience of illness, individual resources, values, and life context rather than viewing the patient merely as a medical diagnosis.
3An 82-year-old bedridden patient is admitted to a geriatric ward following a femoral neck fracture. When conducting a pressure ulcer risk assessment in accordance with Swedish clinical practice (Vårdhandboken), which instrument and initial timeframe should the nurse use?
A.Modified Norton Scale or Braden Scale conducted within 24 hours of admission
B.Glasgow Coma Scale conducted once weekly
C.Mini Mental State Examination (MMSE) conducted only if redness is observed
D.Abbey Pain Scale conducted on the day of planned discharge
Explanation: Vårdhandboken and Swedish national patient safety guidelines mandate that structured risk assessments for pressure ulcers (trycksår) be performed upon admission, typically within 24 hours, using validated assessment scales such as the Modified Norton Scale (Modifierad Norton-skala) or Braden Scale, coupled with immediate skin inspection.
4During morning skin assessment, the nurse discovers a localized shallow open skin defect over the sacrum with a red-pink wound bed, without slough or bruising. An intact serum-filled blister is also noted adjacent to the lesion. According to the international EPUAP classification used in Vårdhandboken, how should the nurse categorize this pressure ulcer?
A.Category I (Icke avblekbar rodnad)
B.Category II (Delhudsdefekt)
C.Category III (Fullhudsdefekt)
D.Category IV (Djup fullhudsdefekt med blottat ben eller sena)
Explanation: According to EPUAP/Vårdhandboken, Category II (Kategori II: Delhudsdefekt) presents as partial thickness loss of dermis presenting as a shallow open ulcer with a red-pink wound bed without slough, or as an intact or open/ruptured serum-filled blister. Category I is non-blanchable erythema of intact skin, Category III involves full-thickness tissue loss with visible subcutaneous fat, and Category IV exposes bone, tendon, or muscle.
5A 74-year-old patient with chronic venous insufficiency presents at a primary care clinic (vårdcentral) with a superficial venous leg ulcer on the medial gaiter area. Before applying graduated therapeutic compression bandages, which clinical assessment is essential to confirm safety?
A.Serum albumin and prealbumin to judge healing capacity
B.Administering a prophylactic dose of low-molecular-weight heparin
C.Applying topical antibiotic ointment across the entire peri-ulcer skin
D.Ankle-brachial pressure index (ankeltrycksindex) to exclude arterial disease
Explanation: Compression is the cornerstone of venous leg ulcer treatment, but strong compression on a leg with arterial insufficiency can cause ischaemic damage. Arterial circulation is therefore checked first, usually by measuring the ankle-brachial pressure index (ankeltrycksindex) with a handheld Doppler. Standard compression is generally used when the index is 0.8 or higher; lower values call for medical and vascular assessment before any compression.
6A patient with an indwelling urinary catheter (KAD) is resting in bed after abdominal surgery. Which nursing measure is most critical to prevent catheter-associated urinary tract infection (CAUTI / vårdrelaterad urinvägsinfektion) according to Vårdhandboken?
A.Irrigating the bladder daily with 100 mL of 0.9% sterile saline
B.Disconnecting the catheter tubing every 8 hours to measure urine output accurately
C.A closed drainage system with the bag kept below bladder level
D.Applying antiseptic chlorhexidine cream directly into the urethral meatus twice daily
Explanation: To prevent catheter-associated urinary tract infections, Vårdhandboken specifies that the urinary drainage system must remain strictly closed (slutet system) without unnecessary disconnections, and the drainage bag must always be positioned below the level of the bladder to prevent retrograde backflow of contaminated urine while ensuring the bag does not touch the floor.
7The nurse uses the Downton Fall Risk Index (DFRI) to assess an 84-year-old patient admitted to an orthopaedic ward. Which set of factors is scored in this instrument?
A.Previous falls, medications, sensory deficits, mental state and gait
B.Body mass index < 18.5, serum sodium < 135 mmol/L, and elevated body temperature
C.Smoking history, alcohol intake over 14 units/week, and occupational hazards
D.History of deep vein thrombosis, elevated D-dimer, and hyperlipidemia
Explanation: The Downton Fall Risk Index (DFRI), widely used in Swedish health care and Senior Alert, scores previous falls (tidigare fall), medication (for example sedatives, diuretics, antihypertensives), sensory deficits (vision, hearing, limbs), mental state (confusion) and gait. A score of 3 or more indicates a high risk of falling.
8During nutritional screening in a municipal care home (särskilt boende), the nurse notes that a resident has lost 7% of their body weight in 3 months and has a BMI of 19.5 kg/m². What is the primary initial nursing action?
A.Immediate placement of a total parenteral nutrition central line
B.Offering low-calorie liquid diet drinks to avoid digestive strain
C.Recommend a 14-hour overnight fast to stimulate daytime appetite
D.A nutrition assessment, food diary and energy-dense meals
Explanation: Unintentional weight loss, eating difficulties and low BMI (below 22 in people over 70) are recognized risk indicators for malnutrition in Swedish guidance. The nurse should start a nutrition assessment (näringsutredning), look for causes, keep a food and fluid record (kost- och vätskeregistrering) and adapt the diet with energy- and protein-dense meals and snacks (mellanmål), while keeping the overnight fast (nattfasta) to no more than 11 hours.
9An 86-year-old female living at home is visited by the home healthcare nurse (hemsjukvården). The patient appears lethargic, has dry oral mucosa, poor skin turgor on the sternum, dark concentrated urine, and an orthostatic systolic blood pressure drop of 25 mmHg. Which nursing diagnosis and intervention take immediate priority?
A.Fluid deficit; support drinking, chart fluids, contact the doctor
B.Impaired skin integrity; apply corticosteroid ointment to the chest skin
C.Excess fluid volume; restrict oral fluid intake to 500 mL per 24 hours
D.Hypothermia; apply an electric warming blanket set to maximum heat
Explanation: The clinical presentation (dry mucous membranes, tenting skin turgor, concentrated urine, lethargy, and significant orthostatic drop) is classic for dehydration (dehydrering / vätskebrist), common in frail older adults whose thirst mechanism is blunted. The immediate nursing priority is structured oral rehydration, fluid chart monitoring (vätskelista), and consulting the physician to evaluate need for parenteral rehydration.
10In the Swedish nursing documentation model (VIPS: Välbefinnande, Integritet, Prevention, Säkerhet), how should a nurse properly document a patient's nursing problem or diagnosis?
A.By stating only the physician's ICD-10 medical diagnostic code
B.By writing a narrative chronological diary without standardized keywords
C.A statement of the problem, its cause and its signs or symptoms (PES)
D.By listing solely the medical prescriptions ordered by the responsible physician
Explanation: Under the Swedish VIPS framework and professional nursing standards, an omvårdnadsdiagnos (nursing diagnosis/problem) is structured using the PES format: Problem (omvårdnadsproblem), Etiologi (orsak), and Symtom/Tecken (kännetecken). This distinguishes nursing science from medical diagnostic codes and ensures targeted, measurable nursing interventions (omvårdnadsåtgärder).

Frequently Asked Questions

How is the written test structured?

It is a Modified Essay Questions (MEQ) examination testing clinical reasoning, the same as the national clinical final examination (NKSE) for Swedish nursing students. Part I has two realistic patient cases worth up to 50 points (pass at 33). Part II is drug calculation, where every answer must be correct; a digital calculator is provided, rounding is done last and units must be correct. You can only move forward through the pages.

What is the practical test?

After passing both written parts you register for a half-day OSCE in a simulated setting at a clinical training centre, where your skills are assessed at six stations, for example venous blood sampling, urinary catheterization and peripheral venous cannulation. It is held in Göteborg, Umeå and Kalmar.

When are the tests held?

Written tests are scheduled several times a year in Göteborg or Kalmar; remaining 2026 dates are 11 November and 11 December, and 2027 dates include 5 February, 7 April, 14 May, 27 August, 10 November and 10 December. Practical tests are held on a rolling basis, for example 17 November 2026 in Kalmar and 30 November 2026 in Göteborg.

How many attempts do I get, and does it cost anything?

The test is free, though you pay your own travel and accommodation. You have up to five attempts at the written test and three at the practical test within five years of the first written sitting. If you fail one written part, you only retake that part.

Is the test available in English?

No. The test is in Swedish. This bank is an English-language multiple-choice study adaptation: it helps you review the knowledge areas but is not an official translation, and it cannot simulate the MEQ answers or the OSCE.
