1A patient arrives at a Swedish community pharmacy with an electronic prescription for Lipitor (atorvastatin) 20 mg tablets. The prescribing physician has not ticked 'Får ej bytas ut'. According to the Swedish generic substitution scheme managed by Tandvårds- och läkemedelsförmånsverket (TLV), what is the dispensing receptarie's legal duty?
A.Dispense Lipitor directly without mentioning alternative brands, as the prescriber named the originator product
B.Offer generic substitution only if the patient has exhausted their high-cost protection threshold (högkostnadsskydd)
C.Contact the prescriber by telephone to seek written authorization before dispensing any generic alternative
D.Substitute the prescribed product with periodens vara, after informing the customer
Explanation: Under the Act on pharmaceutical benefits (lagen 2002:160 om läkemedelsförmåner m.m.), when a prescribed medicine has interchangeable alternatives and neither the prescriber nor the pharmacist has opposed substitution, the pharmacy must inform the customer and substitute the product with the lowest-priced available interchangeable product, periodens vara, designated by TLV. The customer may still decline and pay the price difference.
2A customer in a Swedish community pharmacy refuses generic substitution to 'periodens vara' and insists on receiving the prescribed branded originator product, even though the physician has not opposed substitution. How must the receptarie process the transaction regarding the pharmaceutical benefits scheme (läkemedelsförmånen)?
A.Refuse the dispensation entirely because patients have no statutory right to decline substitution
B.Dispense the prescribed branded product under full benefit coverage without any out-of-pocket surcharge
C.Dispense the brand; the patient pays the price difference, which does not count toward högkostnadsskydd
D.Charge the full retail price of the branded drug and revoke the patient's high-cost ceiling eligibility for 12 months
Explanation: When a customer opposes generic substitution ('motsätter sig utbyte') without a medical reason documented by the prescriber, they are entitled to receive the prescribed product, but they must pay the difference between the pharmacy sales price of the prescribed product and the ceiling price of 'periodens vara'. This price difference is completely excluded from the pharmaceutical benefit and does not accumulate towards the high-cost protection ceiling (högkostnadsskydd).
3Which cells release renin, and which stimulus most directly increases its release?
A.Juxtaglomerular cells of the afferent arteriole, in response to reduced renal perfusion pressure
B.Adrenal cortex cells, in response to a rise in plasma potassium
C.Hepatocytes, in response to a rise in blood pressure
D.Pulmonary endothelial cells, in response to low oxygen levels
Explanation: Renin is released by the juxtaglomerular (granular) cells in the wall of the afferent arteriole. Release increases when renal perfusion pressure falls, when the macula densa senses less sodium chloride, and with sympathetic β1 stimulation. Renin converts angiotensinogen from the liver to angiotensin I, which ACE, mainly in the lungs, converts to angiotensin II.
4Which change in arterial blood is the main stimulus for the central chemoreceptors to increase breathing in a healthy person?
A.A rise in arterial carbon dioxide (PaCO2)
B.A fall in arterial oxygen (PaO2) from 13 to 11 kPa
C.A rise in blood glucose after a meal
D.A fall in plasma potassium
Explanation: Central chemoreceptors in the medulla respond to the pH of the cerebrospinal fluid, which falls when arterial CO2 rises because CO2 crosses the blood–brain barrier and forms carbonic acid. In healthy people PaCO2 is the dominant drive to breathe; the peripheral chemoreceptors' response to hypoxaemia becomes important mainly when PaO2 falls below about 8 kPa. Opioids and benzodiazepines blunt the CO2 response, which explains their respiratory depression.
5Which vitamin is required as a cofactor for the gamma-carboxylation of clotting factors II, VII, IX and X?
A.Vitamin B12 (cobalamin)
B.Vitamin C (ascorbic acid)
C.Vitamin K
D.Vitamin D (calciferol)
Explanation: Reduced vitamin K is the cofactor for the gamma-glutamyl carboxylase that carboxylates clotting factors II, VII, IX and X (and proteins C and S), allowing them to bind calcium and phospholipid surfaces. Warfarin inhibits vitamin K epoxide reductase (VKORC1), which recycles vitamin K, so these factors become non-functional; vitamin K is the antidote.
6Under the Swedish pharmaceutical benefit rules, what is the maximum supply that a community pharmacy may dispense at one time within the benefit (högkostnadsskyddet) for a chronic condition?
A.30 days (1 month)
B.90 days (3 months)
C.60 days (2 months)
D.180 days (6 months)
Explanation: Under förordningen (2002:687) om läkemedelsförmåner m.m., a purchase within the pharmaceutical benefit may cover at most three months' consumption (often called the 90-day rule), calculated from the prescribed dosage. A new purchase within the benefit is normally possible once two-thirds of that period has passed.
7A patient taking metoprolol 50 mg once daily was dispensed a 100-tablet pack under the pharmaceutical benefit scheme 30 days ago. The patient now requests another 100-tablet pack under the benefit scheme before leaving for a two-week vacation. How should the receptarie evaluate this request?
A.Decline a benefit dispensing until two-thirds of the last supply is used, unless special reasons apply
B.Dispense the pack under the benefit without question because metoprolol is a non-narcotic chronic medication
C.Cancel the prescription immediately because early collection indicates intentional drug hoarding
D.Convert the prescription to an over-the-counter preparation and charge the standard pharmacy retail margin
Explanation: Under förordningen (2002:687) om läkemedelsförmåner m.m., a new purchase within the benefit is normally possible only when two-thirds of the period the previous purchase was meant to last has passed, calculated from the dosage (about 67 days for a 100-day supply). An earlier purchase within the benefit requires special reasons, such as a long stay abroad, and the reason is documented. After 30 days this patient does not qualify unless such reasons exist.
8HIV is a retrovirus. What does its reverse transcriptase do, and which drug classes inhibit it?
A.It copies viral RNA into DNA; inhibited by NRTIs and NNRTIs
B.It cleaves viral polyproteins into functional proteins; inhibited by protease inhibitors
C.It inserts viral DNA into the host genome; inhibited by integrase inhibitors
D.It helps the virus fuse with the CD4 cell membrane; inhibited by fusion inhibitors
Explanation: Reverse transcriptase is an RNA-dependent DNA polymerase that copies the viral RNA genome into DNA, which integrase then inserts into the host cell's DNA. Nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs, such as tenofovir and emtricitabine) act as chain terminators, while non-nucleoside inhibitors (NNRTIs, such as efavirenz) bind an allosteric site on the enzyme.
9In Sweden, certain pharmaceuticals are categorized as 'särskilda läkemedel' (special medicinal products) under Läkemedelsverket's prescription regulations. Which of the following groups of medicinal products belongs to this category?
A.All cardiovascular antihypertensives and antiarrhythmics
B.All prescription oral broad-spectrum antibiotics and antifungal agents
C.Narcotic medicines, anabolic steroids and growth hormone (somatropin)
D.Over-the-counter NSAIDs and antihistamines packaged in blister packs
Explanation: HSLF-FS 2021:75 (Bilaga 11, version from 15 September 2025) defines särskilda läkemedel as medicines containing narcotics (with listed exceptions), codeine-containing medicines, and medicines in certain ATC groups: anabolic steroids, androgens, somatropin (growth hormone) and erythropoiesis-stimulating agents. Special rules apply: the quantity must always be stated, a starter pack may not be prescribed, and the identity of the person collecting the medicine must be checked.
10A regular customer has run out of insulin glargine on a Saturday evening. The e-prescription has no dispensings left and the vårdcentral is closed. What should the receptarie do?
A.Hand out one pack as an emergency supply and ask the prescriber to renew the prescription on Monday
B.Help the patient reach a prescriber, for example via 1177 or an on-call clinic, to get a new prescription
C.Dispense a full 90-day supply and register the prescription as renewed for the doctor
D.Advise the patient to pause insulin until Monday and monitor blood glucose at home
Explanation: Swedish dispensing rules (HSLF-FS 2021:75) give pharmacies no general right to hand out a prescription-only medicine when no valid prescription remains; there is no emergency-supply rule of the kind found in some other countries. The receptarie should help the patient get a new prescription quickly, for example through 1177 Vårdguiden or an on-call or emergency service, because stopping insulin risks diabetic ketoacidosis. The rule that lets a pharmacist hand out a smaller number of doses when the prescriber cannot be reached applies to unclear or incomplete prescriptions, not to prescriptions that have run out.

Sweden Dispensing Pharmacist Knowledge Exam (Kunskapsprov för receptarier): Suggested Study Strategy

1Spend most of your time on biosciences, which make up 60% of the receptarie theory test: pharmacology, pharmacokinetics, physiology, biochemistry and infection biology.
2Do not skip chemistry: concentrations, buffers, functional groups, stereochemistry and analytical methods make up 20% of the theory test.
3Work through the previous receptarie tests that Uppsala universitet publishes, and practise reading a scientific article in English for Part C.
4Drill drug calculations step by step (mg/kg dosing, dilutions, infusion rates, renal function) and check units at every step.
5For the practical stations, practise counselling in Swedish and searching FASS and Janusmed under time pressure.
6Know the core Swedish dispensing rules in HSLF-FS 2021:75 and the benefit rules on generic substitution and three months' supply.

Frequently Asked Questions

Is the receptarie test the same as the apotekare test?

No. Uppsala universitet runs both tests and uses the same structure (Parts A–C and a seven-station practical test), but the receptarie test is separate, based on the receptarie programme, and weighted biosciences 60%, chemistry 20% and pharmacy 20% instead of 50/25/25.

How is the test structured?

The theoretical test is taken on a computer in Uppsala and has Part A (factual multiple-choice), Part B (applied multiple-choice) and Part C (questions on an English scientific article). Each moment must be passed at the same sitting. After passing it you take a one-day practical test with seven stations, such as FASS and Janusmed searches, counselling, interprofessional communication and a laboratory skill.

How many attempts do I get, and what does it cost?

You have five attempts at the theoretical test and three at the practical test, and the whole test must be passed within five years of your first sitting. The test itself is free; Socialstyrelsen charges SEK 990 when you apply to have your education reviewed, and you pay your own travel and accommodation.

Is the test available in English?

No. The test is in Swedish, although Part C uses a scientific article in English with questions in Swedish. This practice bank is an English-language multiple-choice study adaptation: it helps you review the knowledge areas but is not an official translation and does not simulate the Swedish test or its practical stations.

What happens after I pass?

Uppsala universitet reports the result to Socialstyrelsen. The next steps on Socialstyrelsen's route are a course in Swedish health and medicines legislation, a period of practical service and finally the application for a Swedish licence.
