1A 36-year-old primigravida presents to the antenatal care clinic (mödrahälsovården, MHV) requesting combined ultrasound and biochemical screening (KUB-test) for fetal aneuploidy. According to Swedish national guidelines, within which gestational window must the ultrasound component for nuchal translucency (nackuppklarning, NUPP) measurement be performed?
A.Gestational week 11+0 to 13+6
B.Gestational week 8+0 to 10+6
C.Gestational week 14+0 to 16+6
D.Gestational week 17+0 to 19+6
Explanation: In Swedish antenatal care, the KUB examination combines maternal biochemical markers (free beta-hCG and PAPP-A) with an ultrasound examination measuring fetal crown-rump length (CRL) and nuchal translucency. This ultrasound scan must be performed between gestational week 11+0 and 13+6 (corresponding to a fetal CRL of 45 to 84 mm). Outside of this precise developmental window, nuchal translucency measurements are no longer valid for statistical risk calculation.
2A 29-year-old multigravida undergoes a 75 g oral glucose tolerance test (OGTT / glukosbelastning) at 28 weeks of gestation due to elevated risk factors. According to Swedish national criteria (Socialstyrelsen and SFOG), which venous plasma glucose result confirms the diagnosis of gestational diabetes mellitus (graviditetsdiabetes)?
A.Fasting venous plasma glucose of 4.8 mmol/L
B.1-hour post-load venous plasma glucose of 9.2 mmol/L
C.2-hour post-load venous plasma glucose of 8.6 mmol/L
D.Random capillary glucose of 7.2 mmol/L
Explanation: Under the Swedish National Board of Health and Welfare (Socialstyrelsen) and SFOG diagnostic thresholds for a 75 g oral glucose tolerance test using venous plasma, gestational diabetes is diagnosed if fasting glucose is >= 5.1 mmol/L, 1-hour glucose is >= 10.0 mmol/L, or 2-hour glucose is >= 8.5 mmol/L. A 2-hour value of 8.6 mmol/L exceeds the 8.5 mmol/L cutoff and definitively establishes gestational diabetes.
3A 32-year-old nulliparous woman at 34 weeks of gestation visits the mödrahälsovård for a routine check-up. Her blood pressure is 152/96 mmHg (confirmed on repeat measurement after 20 minutes of rest), and a urine dipstick shows 2+ proteinuria. She is asymptomatic without headache, visual changes, or epigastric pain. What is the most appropriate immediate action by the midwife?
A.Reassure the patient and schedule a routine follow-up appointment in one week at the health center
B.Prescribe oral labetalol 100 mg twice daily and advise bed rest at home
C.Order a 24-hour urine collection for total protein and ask the patient to return when results arrive
D.Refer for same-day evaluation at the obstetric unit
Explanation: New-onset hypertension (≥140/90 mmHg) after 20 weeks with proteinuria indicates preeclampsia. When preeclampsia is suspected at the antenatal clinic, the midwife arranges same-day assessment at the obstetric unit (förlossning or specialistmödravård) for maternal blood tests, CTG and ultrasound, because the condition can worsen quickly.
4A pregnant woman at 35 weeks of gestation is admitted to the labor ward with severe preeclampsia (blodtryck 175/115 mmHg, hyperreflexia with clonus, and persistent frontal headache). The obstetrician orders intravenous magnesium sulfate (magnesiumsulfat) for eclampsia prophylaxis. Which clinical parameter must the midwife monitor closely as the primary indicator of magnesium toxicity?
A.Pupillary light reflex and intraocular pressure
B.Patellar reflexes, respiratory rate, and hourly urine output
C.Serum sodium and potassium levels every 30 minutes
D.Bowel sounds and abdominal circumference
Explanation: Magnesium sulfate prevents and treats eclamptic convulsions through central neuroprotection and anticonvulsant activity. Toxicity occurs as serum levels rise: loss of deep tendon reflexes (patellar reflex) is the earliest clinical sign of hypermagnesemia (typically at 3.5–5.0 mmol/L), followed by respiratory depression (< 12 breaths/min) and cardiac arrest. Because magnesium is eliminated exclusively by the kidneys, monitoring hourly urine output (> 25–30 mL/h) is vital to detect accumulation.
5According to Swedish obstetric guidance (SFOG), which pregnant woman has an indication for prophylactic low-dose acetylsalicylic acid (for example Trombyl) to prevent preeclampsia?
A.A parous woman who had early-onset preeclampsia delivered at 31 weeks
B.A 24-year-old primigravida with a BMI of 22 and normal family history
C.A 28-year-old nulliparous woman whose sister had gestational hypertension at term
D.A 35-year-old parous woman whose first pregnancy resulted in an uncomplicated vaginal birth at 40 weeks
Explanation: A previous pregnancy with preeclampsia, especially early-onset disease requiring preterm delivery, is a major risk factor. Low-dose acetylsalicylic acid started before 16 weeks (ideally from about week 12) and continued until about week 36 reduces the risk of recurrent preeclampsia and associated fetal growth restriction.
6During a routine antenatal visit at gestational week 32, the midwife plots the symphysis-fundus (SF) height. The measurement is 27 cm, demonstrating a flat growth trajectory (avplanande SF-kurva) that has crossed two percentile lines downwards since week 28. What is the standard management in Swedish primary antenatal care?
A.Advise the mother to increase caloric intake and re-measure SF-height in 3 weeks
B.Perform an immediate digital vaginal examination to assess fetal station
C.Order maternal serum alpha-fetoprotein and human placental lactogen levels
D.Refer for ultrasound with fetal biometry, amniotic fluid and umbilical Doppler
Explanation: A symphysis-fundus curve that flattens or crosses percentile lines downwards raises suspicion of fetal growth restriction or oligohydramnios. The midwife refers for an obstetric ultrasound within a few days to assess fetal biometry (weight estimate from head, abdomen and femur measurements), amniotic fluid volume and umbilical artery Doppler blood flow.
7A 34-week fetus with suspected growth restriction has umbilical artery Doppler showing absent end-diastolic flow. Which class in the Swedish blood flow classification (blodflödesklass) does this represent?
A.Blodflödesklass 0
B.Blodflödesklass I
C.Blodflödesklass II
D.Blodflödesklass IIIA
Explanation: In the Swedish umbilical artery blood flow classification, class 0 is a normal pulsatility index (within ±2 SD), class I is a PI of +2 to +3 SD, class II is a PI above +3 SD with positive diastolic flow, class IIIA is absent (zero) end-diastolic flow and class IIIB is reversed diastolic flow. Absent end-diastolic flow is therefore blodflödesklass IIIA and indicates markedly increased placental resistance.
8A RhD-negative pregnant woman attends her routine 28-week visit at the mödrahälsovård. Screening for fetal RhD genotype via cell-free fetal DNA in maternal blood at 10 weeks confirmed an RhD-positive fetus. Her antibody screen (immuniseringskontroll) is negative. What is the standard Swedish management?
A.Withhold anti-D immunoglobulin until delivery and administer it only if the baby develops hyperbilirubinemia
B.Administer oral prednisone to suppress maternal antibody synthesis until labor begins
C.Give routine antenatal anti-D immunoglobulin (1500 IU) at weeks 28–30
D.Perform weekly amniocentesis from week 28 onwards to measure bilirubin optical density
Explanation: Sweden offers routine antenatal anti-D prophylaxis to non-immunized RhD-negative women whose fetus is RhD-positive on cell-free fetal DNA testing. Anti-D immunoglobulin is given at gestational weeks 28–30 to protect against alloimmunization from silent fetomaternal haemorrhage in the third trimester, and a further dose is given after birth when the baby is RhD-positive.
9A 33-week pregnant woman contacts the maternity triage unit reporting sudden, bright red, painless vaginal bleeding that saturated a sanitary pad over the past hour. Her uterus is soft and non-tender on abdominal palpation, and fetal movements are felt. Which clinical intervention is strictly contraindicated prior to ultrasound evaluation?
A.Digital vaginal examination (inre palpation / vaginalundersökning)
B.External cardiotocography (CTG) monitoring
C.Intravenous cannulation with a wide-bore cannula
D.Maternal vital signs assessment including blood pressure and pulse
Explanation: Painless bright red vaginal bleeding in the third trimester is the classic presentation of placenta previa (föreliggande moderkaka). Performing a digital vaginal examination can inadvertently puncture the placenta or detach the placental edge, precipitating catastrophic maternal hemorrhage and fetal exsanguination. Digital examination is strictly contraindicated until placenta previa is ruled out by transabdominal or transvaginal ultrasound.
10A 36-week pregnant woman presents with acute continuous severe abdominal pain, dark vaginal bleeding, and a tense, woody ('brädhård') uterus that does not relax. The fetal heart rate tracing demonstrates persistent fetal bradycardia at 85 bpm. What is the most probable diagnosis and immediate required management?
A.Placenta previa; administer tocolysis with terbutaline and observe overnight
B.Vasa previa; perform artificial rupture of membranes to assess amniotic fluid color
C.Uterine rupture following spontaneous contractions; insert a Foley bulb for mechanical cervical ripening
D.Placental abruption; immediate emergency caesarean section (urakutsnitt)
Explanation: Sudden severe abdominal pain, dark vaginal bleeding, a hard uterus that does not relax ('brädhård' uterus) and fetal bradycardia are the hallmark signs of a major placental abruption. This is a life-threatening emergency for mother and fetus that calls for immediate delivery by emergency caesarean section together with fluid resuscitation and monitoring for coagulopathy.

Frequently Asked Questions

How is the Kunskapsprov för barnmorskor structured?

It has a theoretical and a practical part, both held in Lund. The theoretical test consists of four parts taken on the same day: reproductive, perinatal and sexual health and the profession; pregnancy and its complications; labour and birth; and postnatal care. After passing it, you take a one-day practical test with six midwifery tasks at Practicum Clinical Skills Centre.

Do I also need to pass the nurses' knowledge test?

Yes. You can register for the midwives' theoretical test once you have passed the theoretical knowledge test for nurses, and you need to have passed both the theoretical and the practical test for nurses before taking the midwives' practical test.

When and how do I register?

You register with Lunds universitet after Socialstyrelsen has decided that you may take the test. Registration closes four weeks before the test date. For 2027, Lunds universitet plans theoretical tests in January, May and September; practical tests are usually held in April, August and December.

How many attempts do I get?

Five attempts at the theoretical test and three at the practical test, and the whole test must be passed within five years of your first theoretical sitting. Not attending without cancelling counts as a used attempt.

Is the test available in English?

No. The test is in Swedish, and answers must be written in Swedish unless stated otherwise. This bank is an English-language multiple-choice study adaptation: it helps you review the knowledge areas but is not an official translation, and it cannot simulate the written answers or the practical test.
