World Health Organization (WHO) Classification of OPMDs

The World Health Organization defines an Oral Potentially Malignant Disorder (OPMD) as "any mucosal abnormality that carries an increased risk of developing oral squamous cell carcinoma (OSCC), either in the site of the lesion or elsewhere in the oral cavity." It replaces older terms such as "precancer" or "premalignant lesion", recognizing that generalized field cancerization may affect the entire upper aerodigestive tract.

1. Oral Leukoplakia

  • Definition (WHO): "A predominantly white plaque of questionable risk having excluded (other) known diseases or disorders that carry no increased risk for cancer." Leukoplakia is strictly a clinical diagnosis of exclusion. Before designating a white lesion as leukoplakia, the clinician must exclude identifiable conditions such as frictional keratosis, cheek-biting keratosis, candidiasis, lichen planus, chemical burns, leucoedema, and white sponge nevus.
  • Clinical Subtypes:
    1. Homogeneous Leukoplakia: Uniform, flat, thin, smooth, wrinkled, or finely cracked ("cracked mud") white patch with clear margins. Low risk of malignant transformation (1% to 5%).
    2. Non-Homogeneous Leukoplakia: Carries an substantially higher malignant transformation potential (15% to 30%+). Subdivided into:
      • Speckled Leukoplakia (Erythroleukoplakia): Mixed red and white lesion where white keratotic patches are interspersed with erythematous atrophic patches. Carries the highest risk of malignant progression among leukoplakias.
      • Nodular Leukoplakia: White surface composed of irregular rounded excrescences, nodules, or pebbles.
      • Verrucous Leukoplakia: Exophytic, heavily keratinised, warty, or corrugated surface.
  • Proliferative Verrucous Leukoplakia (PVL): A distinct, aggressive, non-smoking-related form of OPMD, predominantly affecting elderly females. It begins as flat homogeneous hyperkeratosis that insidiously spreads across multiple oral sites, transforming into diffuse verrucous plaques with an exceptionally high rate of malignant transformation (>60–70%) into verrucous carcinoma or OSCC, refractory to surgery.
  • High-Risk Anatomical Sites: In the UK, leukoplakias located in the floor of the mouth, ventrolateral tongue, and retromolar complex / soft palate exhibit the highest propensity for harboring severe dysplasia or invasive carcinoma. Sublingual keratosis (diffuse white patch on the anterior floor of the mouth resembling an "eburnated butterfly") carries an exceptionally grave prognosis.

2. Oral Erythroplakia

  • Definition (WHO): "A predominantly red patch of the oral mucosa that cannot be characterized clinically or pathologically as any other identifiable red condition." Specific conditions to exclude include erythematous candidiasis, denture stomatitis, vascular malformations, trauma, and desquamative gingivitis.
  • Clinical Appearance: A smooth, fiery red, velvety, granular, or slightly depressed plaque with distinct, sharply demarcated margins. Often asymptomatic, though patients may notice mild burning.
  • Malignant Transformation Risk: Erythroplakia is the single most dangerous OPMD. Histopathologically, >85% to 90% of all true erythroplakias demonstrate severe epithelial dysplasia, carcinoma-in-situ, or frank invasive OSCC at the time of the initial diagnostic biopsy. Any unexplained persistent solitary red lesion MUST be treated with immediate suspicion and biopsied without delay.

3. Oral Lichen Planus (OLP)

  • Etiopathogenesis: A chronic, immune-mediated mucocutaneous inflammatory disorder mediated by CD8+ cytotoxic T-lymphocytes. Antigenic alteration of basal keratinocytes induces T-cells to release perforin, granzymes, TNF-α, and interferon-gamma (IFN-γ), driving programmed cell death (apoptosis) of the basal cell layer and matrix metalloproteinase-mediated destruction of the basement membrane.

  • Histological Diagnostic Criteria of OLP:
    1. Hyperparakeratosis or hyperorthokeratosis with a characteristic "saw-tooth" pointed morphology of the epithelial rete ridges.
    2. Liquefactive (hydropic) degeneration of the basal cell layer, where basal keratinocytes become vacuolated, lose hemidesmosomal attachments, and rupture.
    3. Civatte (colloid / cytoid) bodies: Apoptotic, rounded, anucleate, eosinophilic hyaline structures found in the lower epithelial strata and papillary lamina propria, representing dead necrotic keratinocytes.
    4. A dense, well-demarcated, band-like (ribbon-like) infiltrate of mature T-lymphocytes confined strictly to the superficial subepithelial lamina propria, directly abutting the basement membrane.
    5. Absence of epithelial dysplasia (the presence of cellular atypia indicates lichenoid dysplasia or OSCC).
  • Oral Lichenoid Reactions (OLRs): Lesions that clinically and histologically mimic OLP but possess an identifiable extrinsic trigger:
    • Oral Lichenoid Drug Reactions (OLDR): Triggered by systemic drugs including ACE inhibitors (captopril, enalapril), beta-blockers, NSAIDs, sulfonylureas, antimalarials, and thiazide diuretics. Characteristically unilateral, more erosive, and resolve upon physician-guided drug substitution.
    • Contact Lichenoid Reactions: Caused by delayed Type IV contact hypersensitivity to corrosion products (mercury salts) leaking from adjacent dental amalgam restorations. Strictly confined to the mucosal surface in direct physical contact with the restoration; resolves within 3–6 months following replacement of the amalgam with composite or ceramic.

4. Oral Submucous Fibrosis (OSMF)

    • Fibrotic stage: Progressive loss of mucosal elasticity, diffuse blanching (pale, leathery, marble-like porcelain mucosa), and loss of lingual papillae. Palpation reveals dense, vertical, fibrous bands running through the buccal mucosa, soft palate, and labial mucosa.
    • Advanced stage: Severe, progressive restriction of mouth opening (trismus), loss of uvular mobility ("bud-shaped" shrunken uvula), sunken cheeks, and difficulty with speech, mastication, and swallowing.
  • Malignant Transformation Risk: OSMF carries a high transformation rate of 7% to 10%. The dense fibrosis is irreversible; cessation of betel quid halts further progression but does not restore tissue elasticity. Surgical release of bands often results in contracture and recurrence.

5. Actinic Cheilitis (Farmer's Lip / Sailor's Lip)
