Too-extreme language. The passage makes a moderate, hedged claim ("many," "often," "can," "in some cases"), but a wrong answer choice replaces the hedge with an absolute word -- "always," "never," "all," "none," "only," "completely," "guarantees." Extreme wording is a red flag: even when an option otherwise sounds close to the passage's claim, an absolute word that the passage never actually used should make you suspicious.
Right words, wrong context. This is the subtlest trap. The wrong option is built almost entirely from vocabulary and phrases lifted straight out of the passage -- so it "feels" familiar and safe -- but it reassembles those words to support a claim the passage never actually makes. The fix is to ask not "do I recognize these words?" but "does the passage connect these specific words in exactly this way?"
Partially-true answers. The wrong option is true of one example, one paragraph, or one part of the passage, but the question asks about the passage as a whole (or vice versa). A passage that describes three different hospitals, only one of which saw an improvement, invites a wrong answer that generalizes that one hospital's result to all of them.
Opposite/reversed-logic answers. The wrong option flips a cause and its effect, or states the reverse of a relationship the passage establishes -- for example, claiming that A causes B when the passage actually says B causes A, or claiming a factor "has no effect" when the passage's whole point is that it does.
Out-of-scope answers. The wrong option might be true in general, or true of similar situations, but it is simply never addressed by this specific passage.
