What the RPFT Credential Is

The Registered Pulmonary Function Technologist (RPFT) is the advanced credential issued by the National Board for Respiratory Care (NBRC) for pulmonary function laboratory professionals. Critically for exam planning: there is not a separate “RPFT-only” test. You sit the PFT Examination. Performance relative to two cut scores determines which credential(s) you receive.

That dual-cut design means RPFT candidates and CPFT candidates study the same content outline, face the same item pool rules, and share the same time limit. Aiming for RPFT simply means you must clear the higher performance standard on the same form structure.

Exam Format at a Glance

Scored vs pretest items

You will not know which questions are pretest. Treat every item as scored. The 15 pretest items are used by NBRC for future-form development; they do not contribute to your reported score. Your credential decision is based on performance on the 100 scored items relative to the published cut scores for that form.

Testing logistics (PSI)

The PFT Examination is administered at PSI sites. Plan for:

• In-person attendance only — remote testing is not offered for this exam.
• Standard secure-testing rules (ID, no unauthorized materials, center check-in windows).
• Scheduling after NBRC application approval; confirm current scheduling instructions on the NBRC examination page and PSI communications you receive after authorization.

Because the exam is only 2 hours for 115 items, pacing is a skill: roughly one minute per item with a small buffer for review. Build timed practice habits early so format familiarity does not consume cognitive bandwidth on test day.

Fees and AARC Discount

AARC member benefit: Members of the American Association for Respiratory Care (AARC) may receive a one-time $40 discount on a first-time PFT Examination (and certain other listed NBRC exams) when registration is completed through the designated AARC registration link. The discount is not an automatic price at every checkout path — use the AARC-linked application route if you qualify. Confirm current eligibility language on AARC and NBRC registration materials before paying.

Budget for possible repeat fees if you need a second attempt. Retake timing rules (below) affect how soon you can re-register, not only how much you pay.

Eligibility Pathways

You must be at least 18 years of age and meet one of the following pathways (summarized from NBRC PFT/RPFT program requirements; always verify the live handbook before applying):

1. CoARC-accredited respiratory therapy program graduate — minimum associate degree pathway as recognized for RT program completion.
2. Hold CRT or RRT — current NBRC respiratory therapy credential pathways accepted for PFT exam eligibility.
3. College credit pathway — complete 62 semester hours of college credit including biology and mathematics (and any additional course details NBRC lists on the current application).
4. CPFT holder seeking RPFT — candidates who already hold CPFT and are pursuing the high cut / RPFT outcome on the same PFT Examination framework.

Practical note: Eligibility gets you a seat; it does not change which form content you face. A CRT/RRT-trained candidate and a college-credit pathway candidate both sit the same PFT Examination with the same dual cuts. Your study plan should be driven by the DCO domains, not by how you qualified.

Dual-Cut Scoring — Why It Matters for Study Goals

NBRC uses scaled cut scores (exact scaled numbers can change by form and are published in candidate materials). Conceptually:

• Low cut → CPFT
• High cut → CPFT + RPFT

If your career target is RPFT, plan study intensity for the high bar, not “just enough for CPFT.” Domain II (Procedures) alone is 44% of scored items; weak procedure validity skills can keep a strong instrumentation student below the high cut even if overall comfort feels “okay.”

Do not invent or memorize unofficial pass-rate folklore. NBRC does not publish a simple public “RPFT pass rate” that you should treat as a planning constant. Measure readiness with content mastery against the DCO and with timed practice (including free practice at /practice/rpft), not with third-party percentage rumors.

Retake Policy

Strategy implications:

• Use early attempts (if needed) with a full post-test gap analysis — do not burn attempts without reviewing weak DCO subtasks.
• After two unsuccessful attempts, the 180-day clock is real calendar time; build a structured remediation plan (especially Procedures + analysis-level items) before rebooking.

Credential Maintenance (CMP)

RPFT (and CPFT) credentials are maintained through the NBRC Credential Maintenance Program (CMP) on a 5-year cycle. Passing the exam is the start of professional credentialing, not the end. After you earn RPFT, track CMP requirements and deadlines on your NBRC account so the credential does not lapse while you are working in the PFT lab.

• Pediatric items are limited but guaranteed in the form mix — do not skip pediatric coaching, growth/reference issues, or age-appropriate technique simply because adult lab volume dominates your job.
• Blood gas and CO-oximetry are capped — still high-yield because each item can be analysis-heavy (validity, pre-analytical error, clinical implication). You will not face a large ABG/CO-ox “mini-exam,” but mistakes on those few items are expensive relative to total score.
• Caps apply to analysis counts as specified in the outline context; broader equipment/procedure knowledge about analyzers may still appear under Instrumentation or Procedures domains. Study systems holistically, then remember the form will not overload blood-gas item count.
