Human Anatomy is about 20% of scored NLN NEX Science (11 of 55 scored items). Many of those items assume you can locate structures relative to one another, not only name organs. Directional terms, body planes, and cavity maps are the shared language of anatomy textbooks, surgical notes, and nursing documentation. If you confuse proximal with distal, or medial with lateral, you lose easy points—even when you know the organ involved.

Nursing relevance is immediate: describing an IV site as proximal to the wrist, documenting a wound as lateral to the midline, or understanding that a transverse CT slice is a horizontal cut all depend on this vocabulary. Master it once; every later anatomy chapter (integumentary through senses) reuses it.

NEX focus: This section is pure anatomy foundation—structure and spatial language. Physiology (how systems function) comes in later chapters. No physics is required or tested on NEX Science.


Anatomical Position: The Universal Reference Frame

All directional terms assume the body is in the anatomical position:

  • Feet roughly parallel and flat on the floor

Why this matters: the palm-forward posture places the radius and ulna in a standard relationship and makes “thumb side = lateral” consistent. A patient lying in bed is still described as if they were in anatomical position. The navel remains anterior to the spine whether the person is supine, prone, or seated.

Exam trap: Describing directions from a patient’s current posture instead of anatomical position (e.g., calling the back “up” when someone is face-down). Always translate to anatomical position first.

Proximal and distal are used mainly for limbs (and sometimes vessels or tubes) relative to their attachment or origin on the trunk—not for comparing head-to-toe on the torso (use superior/inferior there).

Details That Prevent Mix-Ups

  • Midsagittal (median) plane passes through the midline and yields equal left and right halves. A parasagittal plane is parallel but off-center.
  • Frontal = coronal on exams—same idea, two names. “Coronal” is common in radiology.
  • Transverse sections are like cutting a loaf of bread into slices stacked head-to-toe; organs appear as cross-sections.
