Recertification and a Domain-Weighted Study Plan

Passing the Certified Flight Registered Nurse (CFRN) exam starts a four-year clock, not a lifetime stamp. You recertify by continuing-education (CE) attestation or by exam. Both paths still require a current, unrestricted RN license in the United States, a U.S. Territory, Canada, or Australia. If you are leaving the aircraft or the profession and want the letters to survive in a non-practice form, apply for the Board of Certification for Emergency Nursing (BCEN) retired designation rather than hoping an expired credential stays printable on a resume.

Recertify by CE attestation

Attestation means you affirm, on the day you apply, that the hours are already done. You cannot keep collecting hours after you submit, even if you recertify early or use a late grace period. Hours earned after the credential expires do not count.

Rules that trip flight nurses:

• 100 contact hours inside the four-year window, completed before the application.
• At least 75 of those hours must be clinical specialty content—direct care knowledge you can use on a patient (flight physiology, trauma, toxicology, ventilator management). Up to 25 hours may be non-clinical (legal aspects of transport, quality improvement, leadership).
• At least 50 hours must come from an accredited source: BCEN Learn, the American Association of Critical-Care Nurses (AACN), the American Nurses Credentialing Center (ANCC), the Emergency Nurses Association (ENA), the American Burn Association (ABA), the Society of Trauma Nurses (STN), or ASTNA. For applications submitted on or after 1 January 2027, at least 75 hours must be accredited. If your cycle crosses that date, plan the higher accredited floor now.
• One contact hour equals 60 minutes of instructional content. A continuing medical education hour counts as one contact hour.

BCEN does not accept CE activities that are approved only by an emergency medical services provider such as the Commission on Accreditation of Pre-Hospital Continuing Education (CAPCE) or the National Association of Emergency Medical Technicians (NAEMT). A base-night Prehospital Trauma Life Support (PHTLS) or Advanced Medical Life Support (AMLS) class that carries only a CAPCE number will not save a recertification. If the same activity is approved by both an EMS provider and a nursing accreditor, it counts. Log the nursing approval, not the paramedic card.

BCEN randomly audits at least 10% of CE recertifiers. If selected, you have 30 days from the audit email to upload certificates through CE Tracker in your BCEN account. Each certificate needs your name, the date, the title, the provider, the contact hours, and the approver. BCEN Learn hours populate the tracker automatically; everything else you must photograph or scan. Mail, fax, and casual email submissions are not accepted. Failed audit plus an unexpired credential means you may recertify by exam with a new fee. Failed audit plus an expired credential means you start over with an initial exam application.

Recertify by exam, and what it costs

Retaking CFRN recertifies the credential if you pass inside a new 90-day PSI window. Use this path if your CE file is thin, if you want the outline refresh, or if an audit is not going to clear. Fees for recertification by exam or by CE attestation are the same and are non-refundable and non-transferable:

A domain-weighted study plan for the first attempt

This guide's recommended first-attempt plan is 130–190 hours across 12–20 weeks, scaled to how recently you have intubated, packaged, and flown. That hour range is a study design, not a BCEN published requirement. Spend time in proportion to scored items, and start with the largest domain so resuscitation is not what you "get to later."

A practical 16-week cadence for a full-time flight RN:

1. Weeks 1–4 — Resuscitation, including cabin-altitude ventilator behavior and "resuscitate before you intubate."
2. Weeks 5–8 — Medical emergencies, especially the August 2026 additions.
3. Weeks 9–11 — General principles and aviation operations.
4. Weeks 12–14 — Trauma, with packaging and in-flight hemorrhage control.
5. Weeks 15–16 — Special populations, mixed timed sets, and weak-item repair.
