1Which pharmacokinetic parameter relates the total amount of drug in the body to its plasma concentration?
A.Total body clearance
B.Elimination half-life
C.Volume of distribution
D.Area under the curve (AUC)
Explanation: Vd = amount of drug in the body ÷ plasma concentration. A large Vd (well above total body water) means the drug is extensively bound in tissues, and Vd is used to calculate loading doses.
2How do ACE inhibitors such as enalapril lower blood pressure?
A.By blocking angiotensin II at AT1 receptors
B.By blocking vascular L-type calcium channels
C.By directly inhibiting the enzyme renin
D.By reducing angiotensin II formation
Explanation: ACE inhibitors reduce formation of the vasoconstrictor angiotensin II and also slow bradykinin breakdown. Accumulated bradykinin explains the dry cough and, rarely, angioedema.
3Which antibiotic binds the D-Ala-D-Ala end of peptidoglycan precursors, blocking cell wall synthesis?
A.Penicillin G
B.Vancomycin
C.Ciprofloxacin
D.Gentamicin
Explanation: Vancomycin, a glycopeptide, caps the D-alanyl-D-alanine terminus and prevents transglycosylation and transpeptidation. Enterococci that change the terminus to D-Ala-D-Lac become vancomycin-resistant.
4Which toxicities of aminoglycosides such as gentamicin make therapeutic drug monitoring important?
A.Nephrotoxicity and ototoxicity
B.Aplastic anaemia and grey baby syndrome
C.Tendon rupture and QT prolongation
D.Pulmonary fibrosis and corneal deposits
Explanation: Aminoglycosides accumulate in proximal tubular cells and inner ear fluids, causing kidney injury and cochlear or vestibular damage. Levels, renal function and duration of therapy are monitored.
5A patient stable on warfarin starts rifampicin for tuberculosis. What is the expected effect?
A.INR rises because rifampicin inhibits CYP2C9
B.Warfarin is displaced from albumin, causing acute toxicity
C.Renal excretion of warfarin is blocked
D.INR falls because rifampicin induces warfarin metabolism
Explanation: Rifampicin is a strong inducer of CYP enzymes, including CYP2C9 and CYP3A4, so warfarin is cleared faster and the INR drops. The INR must be monitored closely and the dose adjusted, including when rifampicin is stopped.
6Which class of oral antidiabetic drugs lowers glucose by blocking sodium-glucose cotransporter 2 in the proximal tubule?
A.Gliptins, such as sitagliptin
B.Sulfonylureas, such as gliclazide
C.Gliflozins, such as empagliflozin
D.Glitazones, such as pioglitazone
Explanation: SGLT2 inhibitors reduce renal glucose reabsorption, causing glycosuria. They also have heart failure and kidney benefits, but can cause genital infections and, rarely, euglycaemic ketoacidosis.
7What is the antidote for paracetamol overdose, given to restore glutathione and detoxify NAPQI?
A.Naloxone
B.N-acetylcysteine
C.Flumazenil
D.Pralidoxime
Explanation: N-acetylcysteine replenishes hepatic glutathione, which conjugates the toxic metabolite NAPQI. It is most effective when started within about 8 hours of ingestion, guided by the plasma level where available.
8How does digoxin increase the force of contraction of the failing heart?
A.It stimulates cardiac beta-1 adrenoceptors
B.It inhibits phosphodiesterase-3 in myocytes
C.It inhibits the sodium-potassium pump
D.It opens potassium channels in the myocardium
Explanation: Inhibiting the sodium pump raises intracellular sodium, which reduces calcium removal by the Na+/Ca2+ exchanger and increases calcium available for contraction. Hypokalaemia increases digoxin binding and toxicity.
9Dolutegravir, part of the TLD first-line HIV regimen used in Nepal, belongs to which drug class?
A.Integrase strand transfer inhibitors
B.Nucleoside reverse transcriptase inhibitors
C.Non-nucleoside reverse transcriptase inhibitors
D.Protease inhibitors
Explanation: Dolutegravir blocks HIV integrase, preventing viral DNA from being inserted into the host genome. In TLD it is combined with tenofovir and lamivudine, which are NRTIs.
10Which first-line antitubercular drug causes peripheral neuropathy that is prevented by giving pyridoxine?
A.Rifampicin
B.Pyrazinamide
C.Ethambutol
D.Isoniazid
Explanation: Isoniazid interferes with pyridoxine (vitamin B6) metabolism, so pyridoxine is given especially to people at risk, such as those with malnutrition, diabetes, HIV or alcohol use, and during pregnancy.

NPC B.Pharm: Suggested Study Strategy

1Start from the NPC syllabus and model MCQs: four of the seven areas carry about 25 questions each, so pharmacology, pharmaceutical technology, pharmacognosy/medicinal chemistry and community/social pharmacy deserve the most time.
2Read the English text of the Drugs Act 2035 rather than summaries: drug categories (Ka, Kha, Ga), registration and certificate validity, inspectors and analysts, and penalties.
3Practise calculations such as loading and maintenance doses, creatinine clearance, Beer-Lambert assays and pharmacoeconomic ratios.
4Know Nepal-specific content such as major medicinal plants, the DDA and National Medicines Laboratory, the National List of Essential Medicines and the hospital pharmacy guideline.
