Why does the absence of ST elevation not rule out an acute myocardial infarction?
NSTEMI can produce ischemic ST depression, T-wave changes, or even an initially nondiagnostic ECG. Continue serial ECG and troponin assessment when symptoms suggest acute coronary syndrome; do not use one normal-looking tracing to dismiss evolving ischemia.

Frequently Asked Questions
Which PCCN pathway do these flashcards cover?
They cover the AACN PCCN Adult Direct Care pathway for RNs or APRNs who directly care for acutely ill adult patients. PCCN Knowledge Professional is a separate eligibility pathway for nurses who influence care but do not primarily or exclusively provide direct care; the site routes that qualification separately as PCCN-K.
What is the current PCCN exam format and passing score?
AACN's current handbook lists 150 multiple-choice items in 3 hours. Of those, 125 are scored and 25 are unscored items used to gather performance data. The current cut score is 82 correct answers among the 125 scored items, effective January 31, 2024. AACN emphasizes that 82 is a raw cut score, not a percentage.
What is the PCCN pass rate?
AACN reports a 70.10% first-time pass rate for PCCN in calendar year 2025, when 3,321 candidates tested. That statistic is a first-time candidate rate and is different from the raw passing cut score of 82 out of 125 scored items.
How is the current PCCN blueprint weighted?
The current test plan is 80% Clinical Judgment and 20% Professional Caring and Ethical Practice. Published clinical subdomain weights are Cardiovascular 20%, Respiratory 14%, Endocrine 6%, Hematology/Immunology/Oncology 3%, Neurology 7%, Gastrointestinal 7%, Renal 4%, Musculoskeletal 2%, Multisystem 15%, and Behavioral/Psychosocial 3%. Professional content is split into Advocacy/Caring Practices/Response to Diversity/Facilitation of Learning 11% and Collaboration/Systems Thinking/Clinical Inquiry 9%. AACN notes that the published subdomain percentages total 101% because of rounding.
What are the PCCN Adult Direct Care eligibility requirements?
You need a current, unencumbered U.S. RN or APRN license and must satisfy one direct-care option: 1,750 hours caring for acutely ill adult patients in the previous 2 years, including 875 hours in the most recent year; or 2,000 hours in the previous 5 years, including 144 hours in the most recent year. Eligible hours must meet AACN's location, direct-care, and verification rules.
What is AACN's PCCN retake policy?
AACN permits the same certification exam up to four times in a rolling 12-month period and requires a new honor statement after 12 months. AACN's current PCCN handbook and FAQ do not publish a fixed number of days between ordinary attempts. The zero values in this page's structured retake fields mean no fixed day interval is published; they do not mean unlimited attempts or that a same-day retest is guaranteed.
How is PCCN certification renewed?
PCCN certification is granted for 3 years. Direct Care renewal requires a current unencumbered license and eligible practice hours, plus either 100 Synergy CERPs in the required categories or passing the PCCN exam before expiration. AACN currently requires 432 direct-care hours during the 3-year renewal period, including 144 hours in the 12 months before the scheduled renewal date.
