1.1 Current CWCN Exam Facts

Quick Answer: The WOCNCB® Certified Wound Care Nurse (CWCN®) exam is a 120-question, 120-minute computerized multiple-choice test delivered by PSI at a test center or by live remote proctoring. One hundred ten items are scored; ten unidentified items are pretest. WOCNCB uses a criterion-referenced passing point that can change by form and does not publish a raw number of items needed to pass. Five domains from the 2023 job-task analysis—with new forms launched in March 2025—weight Treatment heaviest at 29.49% (32 scored items).

The Certified Wound Care Nurse (CWCN®) credential is the Wound, Ostomy and Continence Nursing Certification Board's specialty registered-nurse mark for wound practice. It is not the advanced-practice CWCN-AP® exam. CWCN-AP uses a separate outline and APRN-level eligibility; do not study the AP blueprint if you are sitting for CWCN. Passing CWCN documents specialty competence in wound assessment, intervention, treatment, care planning, education, and referral across the continuum of care.

WOCNCB and the WOCN® Society are separate organizations. The Society accredits WOC Nursing Education Programs; the Board owns the certification exams and the CWCN mark. CWCN is accredited by the Accreditation Board for Specialty Nursing Certification (ABSNC) and the National Commission for Certifying Agencies (NCCA). You may hold CWCN alone, or you may add ostomy and/or continence exams (section 1.2 covers combined CWOCN® / CWON® timing).

What the exam looks like on test day

PSI delivers a computerized, one-best-answer multiple-choice exam. There are no multi-select items and no true/false items. You may skip a question, mark it, and return later to change an answer. When you finish, you receive pass/fail results before you leave a test center; remote sessions also provide immediate scoring. You have 120 minutes for 120 questions—about one minute per item on average.

The 10 pretest items are not labeled. Treat every screen as if it counts. Pretesting is a standard psychometric practice so WOCNCB can evaluate new questions before they become scored. Your reported result is based on the 110 scored items, which are previously used questions whose statistics are known.

Passing standard — published vs unpublished

WOCNCB sets a criterion-referenced passing point: the standard is competence on the content, not a curve that fails a fixed share of candidates. Because forms differ slightly in difficulty, the passing point can change from one form to the next. WOCNCB does not publish how many items you must answer correctly. Do not convert the 2025 pass rate of 70.52% into a personal target such as "get 70% of 110." That 70.52% is the share of 2025 administrations that passed, not a cut score.

Cognitive levels: recall, application, analysis

The 2024 test specifications assign complexity as recall, application, and analysis. Recall asks you to recognize a fact (a classification category, a contraindication, a lab that belongs in a nutrition screen). Application asks you to use a rule in a patient situation (choose an offloading method for a plantar neuropathic ulcer; pick a dressing property for heavy exudate). Analysis asks you to prioritize, distinguish etiologies, or select the next best action when more than one option looks plausible. Build study time around case stems, not isolated term lists.

Blueprint: 2023 job-task analysis and March 2025 forms

Every five years WOCNCB conducts a job-task analysis (role delineation). In 2023 a panel of wound, ostomy, and continence subject-matter experts listed tasks; a validation survey rated criticality, frequency, and importance. The Board approved five CWCN performance domains covering 14 tasks. New CWCN exam forms launched in March 2025 using items written to that 2023 outline. The official CWCN 2024 Test Specifications allocate 110 scored items. Use these official weights when you plan hours.

Assessment (26 items) covers factors that affect wounds and healing, health history, diagnostic clues, focused skin and wound exams, etiology (pressure, venous, arterial, neuropathic, surgical, atypical, and others), classification systems, periwound and wound characteristics, healing phases, and risk, perfusion, and sensation skills such as Braden-type tools, pulses, and ABI/TBI when indicated.

Intervention (18 items) covers recommending and providing interventions: wound-bed-preparation principles, pain, cultures, studies, and prevention—moisture management, pressure redistribution, offloading, friction and shear, nutrition, and support-surface selection.

Treatment (32 items) is the largest domain. It covers cleansing, debridement types with indications and contraindications, topical therapies for moisture balance and bioburden, and advanced or adjunctive options (compression, negative-pressure wound therapy, hyperbaric oxygen, bioengineered tissue, total contact casting, and related modalities).

Care Planning (13 items) is the smallest domain but still more than 10% of scored items. It covers setting patient-centered goals (preventive, palliative, maintenance, or curative) and then evaluating, interpreting response, and modifying the plan.

Education and Referral (21 items) covers teaching patients and caregivers, teaching other clinicians, and multidisciplinary referral (nutrition, rehab, surgery, infectious disease, dermatology, podiatry, social work, and indicated studies).

Assessment plus Treatment together are 58 of 110 scored items. If your calendar is short, those two domains still cannot be your entire plan: Education and Referral (21) is larger than Intervention (18).

2025 volume and pass rate

On the WOC exam statistics page, WOCNCB reports 1,399 CWCN test administrations in 2025, a 70.52% pass rate, 986 newly certified, 1,078 recertified, and 8,523 active CWCN credentials as of 12/31/2025. The pass rate counts all administrations (first-time and retake). Combined CWOCN and CWON counts are reported separately and are not a substitute for the CWCN row. A 70.52% pass rate means a substantial minority did not pass on that attempt—plan as if you need competence across all five domains, not a casual review of dressing names.

Scheduling, six-month NTS, and PSI support

Apply and schedule at https://test-takers.psiexams.com/wocncb/. After approval, initial candidates have a 6-month Notice to Schedule (NTS) window to complete the exam(s) on that application. The scheduler typically shows appointments 2 to 30 days out; seats are first-come, first-served. You may sit at a PSI center or use live remote proctoring. Bring required identification; remote sessions include a room scan and system check.

Candidate scenario

Maya, BSN, RN, works as a long-term-care wound nurse. She completed a WOCN Society-accredited WOC Nursing Education Program four years ago and is applying for CWCN only. She books a PSI date 10 weeks out. She does not search for a published cut score. She maps hours to the five-domain table: about three weeks on Treatment (32 items), two weeks on Assessment (26), a week-plus on Education and Referral (21), a week on Intervention (18), and a week on Care Planning (13), with mixed cases in the final days. She practices skipping and returning so the 120-minute clock feels familiar, and she treats every practice item as if it could be an unlabeled pretest question.
