Acute Management Protocol for DKA

  1. Fluid Resuscitation (First Priority): 0.9% Normal Saline (1.0–1.5 L in the 1st hour). Switch to 0.45% NaCl once corrected serum sodium is normal or high. Add 5% Dextrose when blood glucose reaches 200–250 mg/dL to prevent cerebral edema and allow continued insulin for ketoacidosis clearance.
