Defibrotide's profibrinolytic and endothelial-protective activity is inseparable from its bleeding risk, and the therapy runs for weeks in a patient who is already thrombocytopenic, which makes bleeding surveillance a continuous nursing responsibility rather than an admission check.

Concurrent systemic anticoagulant or fibrinolytic therapy is contraindicated, so the medication list is reviewed at initiation and whenever a new order is written. Prophylactic anticoagulation ordered by another service is a real and recurring hazard.
