9.4 Postpartum Cardiopulmonary & Neurologic Emergencies
Core Focus: The postpartum period combines hypercoagulability, rapid autotransfusion from the contracting uterus, extracellular-fluid mobilization, blood-pressure risk, infection risk, and large shifts in cardiac preload. Symptoms that are often minimized—breathlessness, headache, swelling, fatigue, or chest discomfort—may be the first sign of life-threatening disease.
First-look assessment
Assess airway, work of breathing, respiratory rate, oxygen saturation, heart rate and rhythm, blood pressure in both trend and context, mental status, urine output, pain characteristics, bleeding, temperature, and lung sounds. Ask about orthopnea, paroxysmal nocturnal dyspnea, palpitations, syncope, hemoptysis, unilateral leg pain/swelling, visual change, weakness, speech change, and “worst headache.” Place an unstable patient on continuous monitoring, support oxygenation and circulation, and activate the appropriate rapid-response pathway.
Normal postpartum diuresis can cause urinary frequency and gradual edema improvement. It does not explain resting dyspnea, persistent hypoxemia, pink frothy sputum, new crackles, syncope, or focal neurologic findings.
Peripartum cardiomyopathy
Peripartum cardiomyopathy is systolic heart failure developing near the end of pregnancy or in the months after birth when no other cause explains the dysfunction; left-ventricular ejection fraction is typically below 45%. Risks include hypertensive disorders, multifetal pregnancy, advanced maternal age, and some tocolytic exposures, but it can occur without a recognized risk factor.
Red flags are progressive exertional dyspnea, orthopnea, cough, chest pressure, palpitations, tachycardia out of proportion to pain or fever, jugular venous distention, crackles, an S3, and edema accompanied by respiratory symptoms. Escalate early. Evaluation commonly includes ECG, chest imaging, troponin when ischemia is possible, BNP or NT-proBNP, metabolic and hematologic studies, and echocardiography, which assesses ejection fraction and structural disease.
Nursing priorities are upright positioning if tolerated, oxygen for hypoxemia, continuous monitoring, strict intake/output and daily weight, IV access, medication administration as ordered, and rapid cardiology/critical-care involvement. Avoid routine rapid fluid loading. Heart-failure treatment is individualized for blood pressure, renal function, lactation, and thromboembolic risk.
Pulmonary edema and fluid overload
Pulmonary edema may accompany severe preeclampsia, cardiac dysfunction, renal impairment, sepsis, excessive fluid administration, or medication effects. Expect acute dyspnea, tachypnea, falling oxygen saturation, crackles, anxiety, cough, and sometimes pink frothy sputum. Sit the patient upright, apply oxygen, stop nonessential fluids, notify the emergency team, and prepare for chest imaging, ECG, laboratory studies, diuresis or afterload reduction as ordered. Do not assume that every tachycardic postpartum patient needs a large crystalloid bolus; determine whether the circulation is empty, overloaded, obstructed, or failing.
Pulmonary embolism and thromboembolism
Postpartum hypercoagulability and venous stasis increase venous thromboembolism risk. Pulmonary embolism may produce sudden unexplained dyspnea, pleuritic chest pain, tachycardia, hypoxemia, syncope, apprehension, or hemoptysis. A normal leg examination does not exclude it. Stabilize, notify the team immediately, and prepare for definitive imaging selected for the clinical situation. Anticoagulation is the usual treatment when PE is confirmed or strongly suspected and bleeding risk permits; massive PE with shock may require critical-care reperfusion therapy. Coordinate decisions when active hemorrhage, recent surgery, or neuraxial procedures alter anticoagulation safety.
Pneumonia or aspiration can also cause fever, cough, focal crackles, hypoxemia, and respiratory distress. Obtain cultures when appropriate without delaying antibiotics in a severely ill patient and use aspiration precautions when consciousness or airway protection is impaired.
Stroke, CVST, and PRES
Activate a stroke or neurologic emergency response for facial droop, unilateral weakness or numbness, aphasia, visual-field loss, ataxia, new seizure, severe altered mental status, or sudden maximal-at-onset headache. Record last-known-well time and obtain rapid glucose while protecting airway and preventing injury. Do not delay neuroimaging because the patient is postpartum or lactating.
The differential includes arterial ischemic stroke, intracranial or subarachnoid hemorrhage, cerebral venous sinus thrombosis (CVST), eclampsia, and posterior reversible encephalopathy syndrome (PRES). CVST may present with progressively severe headache, seizure, focal deficit, or signs of increased intracranial pressure. PRES often occurs with severe hypertension and causes headache, visual symptoms, seizure, and altered mental status. Treatment depends on the cause: blood-pressure control and magnesium for eclampsia, anticoagulation for CVST unless contraindicated, and stroke-team decisions for reperfusion or hemorrhage management.
Discharge surveillance
Teach patients and support people to seek emergency care for chest pain, trouble breathing, fainting, seizure, new confusion, severe or persistent headache, vision change, or one-sided weakness. A hypertensive-disorder follow-up plan should include blood-pressure evaluation within the interval directed by current guidance—within 72 hours for severe hypertension and no later than 7 to 10 days for other hypertensive disorders—rather than waiting for a routine six-week visit.
