Confined Space (1926.1202)
A confined space is a space that meets all three of the following criteria:
- Is large enough and so configured that an employee can bodily enter it;
- Has limited or restricted means for entry and exit; and
- Is not designed for continuous employee occupancy.
All three prongs must be satisfied. A crawl space you can enter through a small hatch and were never meant to occupy all day is a confined space. A finished mechanical room with a standard door is not, because entry and exit are not restricted.
Permit-Required Confined Space (PRCS)
A confined space becomes a permit-required confined space when it has one or more of these characteristics:
- Contains or has the potential to contain a hazardous atmosphere;
- Contains a material with the potential for engulfment of an entrant (grain, sand, sludge, loose soil, water);
- Has an internal configuration such that an entrant could be trapped or asphyxiated by inwardly converging walls, or by a floor that slopes downward and tapers to a smaller cross-section; or
- Contains any other recognized serious safety or health hazard (unguarded machinery, exposed live conductors, heat stress, falls).

[!CAUTION] The attendant's most important rule is the one most often broken: the attendant never enters the space to attempt a rescue. Doing so leaves the entrant unattended, removes the only person who can summon help, and typically converts a single-fatality incident into a double.
Coordination on Multi-Employer Sites
Subpart AA assigns distinct duties to the host employer (owns or manages the property), the controlling contractor (overall responsibility for construction at the site), and each entry employer. The controlling contractor is the central information conduit: it gathers permit-space information from the host, passes it to entry employers, coordinates entry operations when more than one entity is working in or near the same space, and debriefs the entry employers afterward.
4. Atmospheric Monitoring and Rescue
Continuous Monitoring
A defining feature of the construction rule is its monitoring expectation. Where an employer uses the alternate entry procedures of 1926.1203(e), the atmosphere must be continuously monitored unless the employer can demonstrate that continuous-monitoring equipment is not commercially available or that periodic monitoring is sufficient. - Each authorized entrant uses a chest or full body harness with a retrieval line attached at the center of the back near shoulder level, above the head, or at another point presenting a small enough profile for successful removal. Wristlets or anklets may substitute only where a harness is infeasible or creates a greater hazard.
- The other end of the retrieval line is attached to a mechanical device or fixed point outside the space so rescue can begin the moment the need is recognized.
- A mechanical device must be available to retrieve personnel from vertical-type permit spaces more than 5 feet deep — a tripod-and-winch, davit arm, or equivalent. A rope in the hands of two workers cannot lift a limp adult straight up out of a manhole.
