Scope of Practice Across WCC-Eligible Professions

The National Alliance of Wound Care and Ostomy (NAWCCB) offers the Wound Care Certified (WCC) credential to a diverse group of licensed healthcare professionals, including Registered Nurses (RN), Licensed Practical/Vocational Nurses (LPN/LVN), Physical Therapists (PT), Physical Therapist Assistants (PTA), Occupational Therapists (OT), Podiatrists (DPM), Physicians (MD/DO), Physician Assistants (PA), and Nurse Practitioners (NP). While WCC certification validates advanced knowledge in wound assessment, dressing selection, and healing science, certification alone does not confer legal authority to perform clinical procedures. Legal scope of practice is dictated strictly by State Professional Practice Acts, state licensing boards, and facility-specific credentialing and privileging policies.

Scope Boundaries by Clinical Discipline

1. Registered Nurse (RN)

• Assessment & Staging: Responsible for independent baseline wound assessments, validated pressure injury staging (NPIAP), etiology identification, and care plan design.
• Debridement Scope: Authorized to perform autolytic, enzymatic, and mechanical debridement. Authorized to perform Conservative Sharp Debridement (CSD)—defined as the selective removal of devitalized, necrotic tissue (slough, eschar) above the basement membrane using sharp instruments (scalpel, scissors, curette) without cutting viable tissue or causing significant bleeding—if permitted by the State Nurse Practice Act, completed institutional competency training, and covered by written physician orders or standardized facility protocols.
• Prohibitions: Strictly prohibited from performing deep surgical excisional debridement into viable tissue, fascia, muscle, or bone, and cannot write prescription orders unless licensed as an Advanced Practice Registered Nurse (APRN).

2. Licensed Practical / Vocational Nurse (LPN / LVN)

• Data Collection & Dressings: Authorized to collect clinical data (measuring wound length/width, describing exudate), perform routine cleansing, and apply primary/secondary dressings per established physician or RN orders.
• Prohibitions: Strictly prohibited from performing independent initial wound assessments, establishing care plans, staging pressure injuries independently, or performing any form of sharp debridement (CSD or surgical). Must work under the direct supervision of an RN or physician.

3. Physical Therapist (PT) & Physical Therapist Assistant (PTA)

• PT Scope: Physical Therapists are autonomous practitioners fully licensed in all 50 states to perform comprehensive wound evaluation, Conservative Sharp Debridement (CSD), mechanical debridement (pulsed lavage with suction), biophysical agents (negative pressure wound therapy, low-frequency ultrasound, electrical stimulation, ultraviolet light), and biomechanical offloading/gait evaluation.
• PTA Scope: Physical Therapist Assistants can apply advanced dressings, perform mechanical cleansing, and operate biophysical modalities under PT direction, but are prohibited from performing initial evaluations or sharp debridement in most jurisdictions.

4. Occupational Therapist (OT)

• Upper Extremity & Function: Specializes in upper extremity wound management, post-surgical scar remodeling, custom pressure garment fabrication, splinting, positioning, and adapting Activities of Daily Living (ADLs) to eliminate pressure and shear forces.

5. Advanced Practice Providers (NP / PA) & Physicians (MD / DO / DPM)

• Surgical & Prescriptive Scope: Possess legal authority for deep surgical excisional debridement (removing viable/non-viable tissue below basement membrane, muscle, fascia, and bone), incisional drainage of abscesses, tissue biopsy, application of high-risk cellular and tissue-based products (CTPs), hyperbaric oxygen therapy (HBOT) ordering, and prescribing systemic antibiotics and controlled analgesics.
• Podiatrists (DPM): Hold full surgical, debridement, and prescriptive authority specifically restricted to the lower extremity, foot, and ankle structures.

Institutional Privileging & Collaborative Delegation

Legally, a clinician cannot perform a procedure simply because their state license allows it; the procedure must also be approved under institutional privileging policies. Healthcare facilities must maintain written policies establishing:

1. Educational Prerequisites: Completion of formal didactic and hands-on skill validation (such as WCC certification or specialized CSD workshops).
2. Proctored Competency: Documented completion of proctored clinical debridements supervised by a privileged physician or advanced provider.
3. Annual Privileging Renewal: Re-evaluation of clinical volume, outcome metrics, and technique compliance.
4. Delegation Rules: RNs cannot delegate sharp debridement or initial assessment to LPNs or unlicensed assistive personnel (UAP).
