Exam Format at a Glance

The Board of Governors Examination in Healthcare Management is a computer-based, multiple-choice exam administered for ACHE through Pearson VUE testing centers. Core logistics you should memorize early:

You cannot tell which items are pretest while testing. Treat every question as scored. Skipping “maybe experimental” items is a self-inflicted trap.

Scoring: Criterion-Referenced, Pass/Fail, No Published Percentage

The Board of Governors Exam is criterion-referenced: each candidate is measured against a fixed standard of knowledge rather than against the performance of everyone else who sat that week. A norm-referenced exam (SAT, GRE) ranks you within the test-taking population; this one does not. Your likelihood of passing depends on demonstrating competence in the healthcare executive field—never on the strength or weakness of your cohort.

ACHE sets the cut score using a modified Angoff method. Subject-matter experts rate the likelihood that a minimally competent candidate would answer each item correctly, informed by statistics from previous administrations of that item and by discussion among the SMEs. Those ratings are averaged across experts and summed to produce the exam’s pass point. Because the pass point is assembled from the specific items on a form, it is not a fixed percentage and can vary slightly between forms.

ACHE does not publish a passing percentage, and it is not ACHE’s policy to reveal a candidate’s raw score. Results are reported as pass or fail, with category-level feedback showing your relative performance in each of the ten knowledge areas so you know where you were strong or weak. Third-party study materials routinely quote “about 65%” as the pass level; treat that as an unofficial estimate useful only as a rough practice-test benchmark—not as ACHE’s standard, and not as license to aim for exactly 130 of 200.

ACHE likewise does not routinely publish an official annual pass rate. Some ACHE presentation materials have cited first-time figures in the mid-70% range, but a historical presentation statistic is not a published standard. Focus on mastering the outline: you cannot game a criterion you are not shown.

Ten Domains and Weights (Outline Effective August 1, 2023)

Your score is built from the ten knowledge domains below. Percentages and scored-question counts are the study-planning weights:

Study implication: Healthcare + Management and Leadership + Finance + HR = 52% of scored items (104 questions). Weakness in those four domains is hard to offset with perfect ethics or governance scores alone. Conversely, Governance is only 12 scored items—still must-pass knowledge for executives, but a lower time investment if your diagnostic is already strong there.

Scenario — Misallocated study plan. A CFO candidate spends 80% of study hours on Finance because that is the day job. Finance is 12% of the exam. Under-preparing Healthcare (15%) and Management and Leadership (13%) can fail a strong finance leader who cannot reason about continuum-of-care models, NIMS preparedness, or team change management. Job expertise ≠ domain weight.

Pacing Across Six Hours

Six hours for 230 items averages roughly about 1.5 minutes per item if you use the full clock evenly—but smart pacing includes review time.

Practical approach many candidates use:

1. First pass: answer confidently known items; flag multi-step scenarios
2. Second pass: work flagged items with structured elimination
3. Final pass: check marked answers for misreads (especially “except,” “best,” “first”)

Trap: Spending four minutes early on a dense finance vignette, then rushing ethics and quality later. Pretest items are mixed throughout; a slow start can compress the back half of the exam when fatigue is highest.

Trap: Assuming longer time means you should overthink every option. Executive exams often reward the best management action among plausible choices, not a search for a single absolute truth.

Fees and Registration Reality

Budget the Fellow application fee ($250) and exam fee ($225 per attempt) separately. Application approval precedes exam registration invitation. Retakes require another exam fee and observe the waiting period between attempts (covered in the next section). Always verify current fees, any limited-time application fee waivers, and policies on ache.org before you plan cash flow—fee schedules can change, and OpenExamPrep content should be treated as study orientation, not an invoice.

Pearson VUE: Center-Based Only

The Board of Governors Exam is delivered as Pearson VUE computer-based testing at authorized centers. Remote online proctoring is not the standard delivery channel for this exam. Plan for:

• Seat scarcity. Seats are filled first-come, first-served; book early for your preferred center and date
• Arrive 30 minutes early. Arriving more than 15 minutes late can mean refused admission and forfeiture of the exam fee
• ID. A current government-issued photo ID with signature whose name matches your registration exactly; name changes go through ACHE, not the test center
• No personal items in the testing room—bags, books, notes, phones, pagers, watches, and wallets are all excluded
• Cancel or reschedule at least 24 hours ahead through Pearson VUE; less than 24 hours, or a no-show, forfeits the fee and a new fee is required to reopen an authorization
• Accommodations under the ADA are requested through ACHE before scheduling, since not all centers can support every accommodation
• Travel time so you are not starting a 6-hour exam already depleted

How Domain Weights Should Shape Your Calendar

If you have roughly 12 weeks to prepare:

• Weeks 1–4: Healthcare + Management and Leadership (largest combined weight)
• Weeks 5–7: Finance + Human Resources
• Weeks 8–9: Laws + Quality
• Weeks 10–11: Business + HIT + Ethics + Governance
• Week 12: Full-length timed sets, weak-domain drills, Code of Ethics refresh

Adjust using a diagnostic: if you already lead revenue cycle, still sample Finance but reallocate hours to weaker domains. The outline’s knowledge statements (about 109 items across domains) are the real checklist—weights tell you how many questions, knowledge statements tell you what can be asked.
