DANB derives ICE content from a content-validation and job-analysis process and reviews the outline with subject-matter experts. The current outline divides the exam into four domains:

What the new outline changes
Older local material may display three buckets such as Infection Prevention, Sterilization and Disinfection, and Occupational Safety with 34% / 41% / 25% weights. Do not use that retired structure for the current ICE exam. The March 2025 outline separated Disease Transmission from Cross-contamination, gave instrument processing its own 26% domain, and expanded the administrative program tasks within Domain IV.
This creates several easily missed study areas. Cross-contamination is not just disinfecting the dental chair: it includes unit dosing, aseptic retrieval, single-use devices, dental-unit waterlines, evacuation lines and traps, radiography equipment, impressions and appliances, and waste. Administration is not just an Exposure Control Plan: it includes infection-control breaches, quality improvement, sterilization records, and training records.
A proportional study cycle
Use a four-pass cycle rather than memorizing isolated numbers:
- Map the workflow. Walk through a patient visit from medical-history review and hand hygiene to tray setup, treatment, operatory turnover, instrument transport, reprocessing, storage, and documentation.
- Apply classifications. For each item, decide whether it is single-use or reusable, critical/semicritical/noncritical, heat tolerant or heat sensitive, and which manufacturer instructions govern it.
- Separate authority types. OSHA rules protect employees and are enforceable; CDC recommendations describe safe clinical practice; EPA registers surface disinfectants; FDA regulates medical-device labeling and certain sterilants. State and local rules can add requirements.
- Practice decision questions. Explain why the best action protects the patient or employee. When a number is product-specific—such as a disinfectant contact time or sterilizer cycle—choose the product label or device instructions rather than a memorized universal value.
Adaptive-exam pacing
Sixty minutes divided by 75 questions equals 48 seconds per question on average. That is a checkpoint, not a command to spend exactly 48 seconds on every item. Read qualifiers such as first, best, and except; identify the hazard; choose the controlling rule; then submit deliberately.
DANB’s own preparation page states that once an answer is submitted, the candidate cannot return to that item or change the response. A three-pass or flagged-item strategy is therefore inappropriate. Use the demonstration exam before test day to learn the interface. On the real exam, answer each item before moving forward and maintain a one-way pace.
For unfamiliar questions, eliminate options that skip cleaning, reuse a single-use item, contradict manufacturer instructions, expose a worker unnecessarily, or treat a recommendation as a universal numeric law. Because unscored pretest items are indistinguishable from scored items, answer every question to the best of your ability.
Turn tasks into retrieval prompts
Rewrite every outline task as a “what would I do next?” prompt. For example: a BI is positive once; a surface barrier tears; a water sample exceeds the action limit; an employee reports a needlestick; a secondary bottle will remain for the next shift. For each prompt, state the controlling source and the first safe action. This reveals whether knowledge is usable under time pressure.
Weighting should shape the number of practice scenarios, not the standard of mastery. A candidate who studies only the 34% domain can miss straightforward OSHA or disease-transmission items and also misunderstand integrated scenarios. Instrument transport, for example, joins cross-contamination, sharps protection, PPE, and processing flow.
Use approximate counts carefully: 34% of 75 is 25.5 and 26% is 19.5, so any integer table necessarily rounds. Do not claim that the first domain always occupies questions 1–15 or that adaptive delivery reveals domain order. Study all four areas and let the published percentages determine emphasis.
