Contain at the point of use

At chairside, keep used instruments organized so sharp points do not create hidden hazards. Do not reach blindly into a tray, carry loose instruments in a hand, or leave a bur projecting from a handpiece. Remove disposable sharps safely and place them in a sharps container. Place reusable instruments in a rigid or otherwise puncture-resistant, leak-resistant, closed container for transport to the processing area. Label or color-code the container when required for contaminated material.

If cleaning will be delayed and the instrument manufacturer permits it, use a compatible pretreatment that keeps soil from drying. Do not use a corrosive improvised soak, and do not assume every device can be immersed. Transport containers themselves must be processed on a schedule and in a manner appropriate to contamination.

One-way processing flow

The processing area should have distinct functions: receiving/cleaning/decontamination, preparation and packaging, sterilization, and storage. Physical separation is ideal; where space is limited, clear spatial or temporal separation, signage, and disciplined work practices prevent clean packages from returning to the dirty side. A washer-disinfector uses programmed stages and may reduce handling further. Use the equipment and detergent instructions for loading, water quality, degassing if required, solution change, operating time, routine checks, and maintenance. There is no universal five-to-fifteen-minute ultrasonic cycle or fixed foil-test day that applies to every device.

If manual cleaning is necessary or specified, keep instruments below the waterline when feasible to limit splashing, use a long-handled brush, brush away from the body, and wear suitable utility gloves and face protection. Manual cleaning is higher risk but is not limited to an “emergency”; it may be required for a device that cannot enter available automated equipment. Never clean a sharp by holding it in the bare hand.

Rinse, dry, and inspect

After the validated cleaning stage, rinse as directed to remove soil and detergent residue, then dry thoroughly. Inspect under adequate lighting for debris, corrosion, cracks, loose joints, dull cutting edges, and function. Hinged instruments are opened and disassembled when the instructions require it. Reclean an instrument if soil remains. Remove damaged or corroded items from service rather than allowing a defect to puncture packaging or compromise treatment.

Apply only a sterilizer-compatible, instrument-approved lubricant where directed. Household oil or a product that blocks sterilant contact is not acceptable. Assemble instrument sets consistently and verify counts where the office procedure uses them.

Quality checks and decisions

Performance checks for ultrasonic cleaners and washers follow their manufacturer instructions and facility policy. Record tests, maintenance, and corrective action when the program requires them. If a washer cycle fails or an ultrasonic test is unacceptable, do not move the load forward merely because it looks clean; investigate, reclean as needed, and confirm the equipment is working.

For exam scenarios, keep the sequence intact: contain → transport → clean → rinse/dry → inspect → package → sterilize → store. Skipping cleaning, using examination gloves for sharp cleaning, or letting dirty and sterile items cross paths are clear defects even when a later sterilizer cycle appears normal.

Complex instruments and cassettes

Lumened instruments require flushing or adapters specified by their manufacturer; an ultrasonic bath may not clean an internal channel unless validated for it. Hinged items remain open, and multipart items are disassembled to expose interfaces. Cassettes must be opened or configured so cleaning spray and sterilant can contact the instruments.

Do not rely on eyesight alone for complex devices. Some washer-disinfectors use routine process checks, while cleaning-verification tests can assess residual protein or other soil under the facility program. These tests complement inspection and equipment maintenance; they do not replace correct loading.

If an employee receives a puncture during manual cleaning, stop the task safely, wash and report the injury, preserve the involved device information, and begin evaluation. Complete the remaining instrument processing only after the exposure response and area safety are controlled.
