The Gold-Standard Entry Credential

The Certified Registered Central Service Technician (CRCST) is the leading entry-level certification for sterile processing professionals. It is awarded by the Healthcare Sterile Processing Association (HSPA) — the organization known until 2022 as IAHCSMM (International Association of Healthcare Central Service Materiel Management). When older textbooks, job postings, or practice tests say "IAHCSMM," they mean the same body that now issues the CRCST.

The CRCST proves you can safely decontaminate, inspect, assemble, package, sterilize, store, and distribute reusable medical devices. Because a single missed step can transmit infection to a surgical patient, the credential is increasingly tied to hiring and patient-safety policy.

Exam Structure (Verified for 2026)

Note the fee structure carefully: HSPA charges a single $140 application fee that bundles the first attempt and a retake. This replaced the older two-tier "member vs. non-member" pricing some study sites still quote, so do not trust outdated $125/$200 figures.

Seven Domains & Their Weights

The three core processing domains together carry 60% of the score. A worked example: if you scored 90% on those three domains but 40% on the other four, your weighted score is (0.60 x 90) + (0.40 x 40) = 54 + 16 = 70% — exactly at the cut. That math shows why mastering the big three is non-negotiable, but also why you cannot ignore the 10% domains.

Eligibility & the Provisional Path

• Full CRCST: complete 400 hours of hands-on SP experience (within five years before testing) and pass the exam.
• Provisional CRCST: pass the exam first, then complete the 400 hours within 6 months and submit documentation to convert to full status. This is the common route for students and new hires.
• No degree is required; many employers prefer a high-school diploma or GED. Minimum age is generally 17-18 depending on jurisdiction.

Reference Materials & Recertification

The exam is built from the HSPA Sterile Processing Technical Manual, 9th Edition (2023) — your primary source — reinforced by ANSI/AAMI ST79 (steam sterilization) and the AORN Guidelines for Perioperative Practice. To stay certified you must earn 12 CE credits per year (technical, SP-relevant content such as webinars, in-services, or HSPA lesson plans) and pay the $50 annual renewal fee. Letting certification lapse means re-testing.

State Mandates

Even where no law applies, many health systems require CRCST as a condition of employment, making the credential a practical entry ticket to the field.

How HSPA Builds and Scores the Exam

The CRCST is a criterion-referenced exam, which means your result depends on whether you meet a fixed knowledge standard, not on how other candidates performed that day. There is no curve. HSPA reports scores on a scaled basis where 70% represents the minimum competent performance, so a candidate must demonstrate solid command across all seven domains rather than excelling in one and failing another. The 25 unscored pretest items are scattered invisibly among the 150 questions; they are being field-tested for future exams, so you should answer every item with full effort because you cannot tell which ones count.

Think carefully about what "125 scored questions" means for your margin. At a 70% cut, you need roughly 88 correct scored answers. Missing 37 scored items still passes, but because the four small domains hold only about 13 scored questions each, a complete collapse in even one of them eats heavily into that buffer. This is why a balanced study plan beats cramming a single favorite topic.

Registration, Scheduling, and Retakes

You apply through HSPA, pay the flat $140 fee, and receive authorization to schedule your in-person appointment at a Prometric testing facility (scheduled through Prometric at www.prometric.com/hspa). Because the $140 already covers one retake, a candidate who does not pass the first attempt can re-test without paying the full fee again, within the window HSPA specifies. Bring valid government identification matching your registration name; personal study materials, phones, and smartwatches are prohibited in the testing room. Plan to arrive early, since late arrivals can forfeit the seat and the fee.

Career Value of the Credential

Beyond meeting state mandates, the CRCST is recognized by Navy COOL and other workforce programs, frequently triggers pay differentials, and is the prerequisite stepping stone to advanced HSPA credentials such as the CIS (Certified Instrument Specialist) and CHL (Certified Healthcare Leader). Earning it signals to an OR team that you understand the standards that protect every patient whose instruments you process.
