The American Society for Clinical Pathology Board of Certification (ASCP BOC) offers a Qualification in Immunohistochemistry. After you pass, the credential is reported as QIHC(ASCP). QIHC is a qualification, not a certification. It is written for laboratory professionals who perform immunohistochemistry (IHC), who train others to perform it, or who supervise an IHC service. It is not the Histotechnician HT(ASCP) certification and it is not the Histotechnologist HTL(ASCP) scientist certification. HT and HTL remain the histotechnology certifications that cover a broader tissue-laboratory practice. QIHC is the IHC-focused qualification that sits beside them.

Certification Versus Qualification

Keep the credential types separate while you study:

  • HT(ASCP) and HTL(ASCP) are histotechnology certifications. Most ASCP BOC certifications are taken at Pearson VUE centers as computerized adaptive tests (CAT) with a longer clock and a different content guideline.
  • QIHC(ASCP) is an IHC qualification: 50 items, 90 minutes, non-adaptive, self-administered on your own computer.
  • Passing QIHC does not grant HT or HTL. Passing HT or HTL does not grant QIHC.
  • Many QIHC candidates already hold HT or HTL. That is a common eligibility route (covered in the next section). It is not evidence that the two examinations are the same test.

50 Questions, 90 Minutes, One Best Answer

The QIHC Examination Topic Outline, revised January 22, 2026, states that the examination is composed of 50 questions given in a 90-minute time frame. Every item is multiple-choice with one best answer. ASCP BOC's About the Examination page states the qualification rules in more detail:

  • Four options per item; one option is the best answer.
  • You must answer the current item before the next item appears.
  • Items cannot be skipped.
  • You may flag an item you want to revisit.
  • Flagged review is available only after you have answered all questions, if time remains.
  • Changing an answer does not change previously answered items.
  • The test is computer-based and non-adaptive.
  • You take it on your own computer. It is not administered at a Pearson VUE testing center and it is not CAT.
  • You may not use notes, textbooks, or other study materials, and you may not receive help from another person, during the 90-minute examination period.

That administration model is the most common source of bad study advice. Colleagues who recently sat HT or HTL will talk about Pearson VUE, CAT item difficulty, field-test items, and a 2-hour-30-minute certification clock. None of that is the QIHC qualification. QIHC presents 50 independent one-best-answer items. The software does not make the next question harder because you just answered correctly, and it does not make the next question easier because you just missed one.

Time math is straightforward. Ninety minutes for 50 items is 1.8 minutes per item if you leave no review bank. A workable first pass is about 75 to 80 minutes (roughly 90 to 95 seconds per item) so that 10 to 15 minutes remain for flagged review. Because you cannot skip, you must select an answer to move forward—even if that answer is a flagged guess you intend to replace later. Do not spend four minutes arguing with the first chromogen stem and then rush the last ten laboratory-operations items.

Scaled Scores Run from 100 to 999; 400 Passes

ASCP BOC reports qualification results as pass or fail plus a scaled score. The published scale is 100 to 999. The published minimum passing score is 400. The Board does not publish a raw number of correct answers that equals 400, and this guide will not invent one. Do not convert 400 into "35 of 50" or "80 percent." Scaled scoring places performance on a common metric; 400 is the passing standard on that metric, not a classroom percentage.

You will not receive an on-screen pass/fail printout at a testing center, because you are not in a Pearson VUE center. ASCP BOC emails a notice to log in and view the official score report within four business days after you take the examination. The official report in your ASCP account is the only place results are provided.

Application Fee

The current QIHC credential page lists a non-refundable application fee of $260. Confirm the live page when you apply. The figure in this chapter is the published fee from that page, not a third-party estimate. Because the fee is not refundable, confirm your eligibility route and documentation before you submit (the next section covers how those routes are structured).

Five Content Areas on the January 22, 2026 Outline

Every QIHC item is classified into one of five content areas. The outline publishes a percentage range for each area as a share of the 50-question examination:

Staining is the largest domain. If your study calendar treats "IHC" as mostly antigen retrieval plus DAB, you will under-prepare. Detection systems are the second-largest share. Laboratory operations is larger than many bench technologists expect, because validation, controls, safety, mathematics, and U.S. regulatory sources are in scope. Specimen handling and epitope enhancement are smaller ranges, but they are not optional: poorly fixed tissue and the wrong retrieval buffer present as staining failures, so those items still test applied judgment.

The outline states that questions may be theoretical, procedural, or both. Theoretical items measure the skills needed to apply knowledge, calculate results, and correlate reactions or patient results to histology. Procedural items measure the skills needed to select or perform laboratory methods, evaluate reactions or results, and follow quality-assurance protocols. Regulatory questions are based on U.S. sources (the outline's examples include FDA and CLIA). Study U.S. federal and accrediting-agency frames—CLIA, FDA, CAP-style laboratory practice, OSHA chemical limits—as the tested default, not a non-U.S. scheme.

The outline also warns that examples marked "e.g." are not limited to the listed examples. An unlisted chromogen, retrieval device, or marker can still be a fair question if it sits inside a published topic.

Theoretical Versus Procedural Stems

A theoretical stem might ask which enzyme-chromogen pair produces a permanent brown reaction product that can be coverslipped with an organic mounting medium. A procedural stem might describe a control that stained while the patient tissue stayed blank and ask what you do next: confirm antibody application and retrieval, inspect the reagent lot, or repeat only the negative reagent control. Both styles appear. Later chapters teach the facts those stems use. This chapter exists so you know the exam will mix recall with bench decisions.

Knowledge, Comprehension, and Application

ASCP BOC classifies examination questions into taxonomy levels:

  • Knowledge — recalling and remembering (for example, that the passing scaled score is 400, or that a common peroxidase chromogen is brown and alcohol-stable).
  • Comprehension — interpretation and understanding (for example, why an alcohol-soluble red chromogen cannot be coverslipped with a xylene-based mountant).
  • Application or analysis — problem solving and justification (for example, choosing a biotin-free polymer detection system on biotin-rich liver, or recognizing that an antibody clone change requires full revalidation rather than a one-plus-one lot check).

Expect all three. A candidate who only flash-cards definitions will miss the application stems. A candidate who only "thinks through cases" without memorizing published numbers will miss the knowledge stems. Read every stem and all four options before you choose. Eliminate options you can prove wrong, then pick the best remaining answer—not the first phrase that looks familiar.
