Expiration is not a suggestion and not a visual judgment. 21 CFR 610.53 and the component label set a hard stop. A unit transfused one minute after the labeled date and time is an out-of-date biologic, even if the supernatant looks perfect. This section is about how that date is calculated, what happens to the product while it waits, and what “out of storage” actually means.

Validated transport and the 30-minute myth

AABB’s storage and transport standards focus on maintaining the required temperature in equipment or shipping containers that have been validated and are monitored. Red cells travel at 1–10 °C. Platelets travel as close as possible to 20–24 °C. Frozen products travel solidly frozen. The container, not the wall clock, is the control.

Many hospitals still teach a 30-minute rule: if a red-cell unit is returned within 30 minutes, restock it; if not, discard it. That interval is a facility SOP shortcut. It is a reasonable way to guess that an unsupervised unit has not exceeded 10 °C, but it is not an AABB absolute that every unit outside a refrigerator for 31 minutes is automatically unsuitable. If a unit left the controlled refrigerator, the correct questions are: Is the original tamper-evident seal intact? Are segments still attached? Did a temperature indicator or validated cooler document that the product stayed in range? If the answer is no, do not restock. If a validated cooler kept the unit at 1–10 °C for two hours, discarding it solely because “it has been 30 minutes” is not what AABB requires.

Reissue of whole blood after it has left licensed storage is separately constrained by 21 CFR 640.2(c): unbroken tamper-proof seal, attached segment, continuous 1–6 °C storage and 1–10 °C shipping, and visual inspection before reissue. Hospital issue-and-return SOPs are built on that same idea.

Storage lesion

While red cells sit at 1–6 °C they continue to metabolize, just slowly. The storage lesion is the set of predictable biochemical and membrane changes:

  • Potassium leak. The membrane Na+/K+ ATPase slows in the cold, so extracellular K+ rises throughout dating. Irradiation damages the membrane further and accelerates that leak — the reason irradiated red cells have a shortened outdate and why large-volume or neonate transfusions often prefer fresher or washed cells.
  • 2,3-DPG fall. 2,3-DPG declines over the first 1–2 weeks, shifting the oxygen-dissociation curve left (higher oxygen affinity, less unloading). Levels recover in the recipient over about 24 hours, which is why the clinical effect is usually modest in stable adults but matters more when immediate oxygen delivery is critical.
  • pH drop. Glycolysis produces lactic acid; pH falls over storage.
  • ATP decline and hemolysis. Membrane flexibility falls; free hemoglobin and lactate dehydrogenase rise. Additive solutions and mannitol or citrate are there to limit that hemolysis through day 42.

Platelets stored at 20–24 °C undergo their own lesion: loss of discoid shape (loss of swirl), falling pH from lactate, and accumulating activation markers. That is why pH at outdate is a QC specification (section 3.4) and why agitation and gas-permeable bags exist. Frozen plasma’s “lesion” is mainly decay of labile factors if the product thaws or is stored too warm; Factor VIII is the sentinel labile factor.

Irradiation. The 28-day rule is a “whichever comes first” calculation. An AS-1 unit on day 30 that is irradiated keeps its original day-42 outdate (12 days remain, which is less than 28). An AS-1 unit irradiated on day 5 is recoded to day 33 (28 days from irradiation), not day 42. Platelets do not get a new shorter date after irradiation.

Washing and open versus closed. Entering the bag in a way that breaks the closed system starts a 24-hour red-cell clock at 1–6 °C (or 4 hours if the opened product is held at room temperature). Sterile-connecting devices can keep a system closed. 21 CFR 610.53 specifically dates closed-system deglycerolized red cells with additive solution at 14 days. Other closed-system washers follow their own cleared instructions — the exam phrase “or per closed-system device” is there because not every washer is the deglycerolization 14-day pathway. Do not give every washed unit 14 days.

Thawed plasma products. FFP thawed and stored at 1–6 °C remains labeled FFP for 24 hours. After that, AABB practice allows relabeling as thawed plasma with dating through 5 days from thaw. Factor VIII declines; other factors are relatively better preserved, which is why thawed plasma is used for coagulopathy and massive transfusion but is not a Factor VIII concentrate. Cryoprecipitate is thawed at 30–37 °C and then held at room temperature. Refrigerating thawed cryo re-precipitates proteins. A single closed unit expires in 6 hours; if units are pooled in an open system, dating collapses to 4 hours.

Platelets 5 versus 7 days. 21 CFR 610.53 lists 5 days at 20–24 °C unless the licensed system specifies otherwise. Current FDA bacterial-risk-control strategies and 7-day containers can extend dating to 7 days. Pathogen-reduced platelets follow their approved dating, commonly 5 days. The exam answer is “5–7 days depending on the bacterial strategy and container,” not a blanket 7 days for every platelet.

Rejuvenation. Licensed rejuvenation solutions restore ATP and 2,3-DPG in stored red cells. CPD/CPDA-1 cells may be rejuvenated up to 3 days after expiration; some additive-system cells may be rejuvenated up to day 42. After rejuvenation the unit is either frozen or washed (the rejuvenation solution is not left in for transfusion). It is not returned to a 42-day liquid shelf as if nothing happened.

Thirty minutes versus four hours after issue

Keep these clocks separate:

  • Return-to-inventory (often 30 minutes in SOP): used only if the unit was not spiked, the seal is intact, and required temperature was maintained. AABB’s real test is temperature and closed-system integrity, not a universal 30-minute statute.
  • Infusion after the unit is entered (4 hours): once the container is spiked, transfusion should be completed within 4 hours. That clock protects against bacterial growth in a now-open, room-temperature line. It is not a 4-hour permission to leave an unspiked unit on the counter and then restock it.

Worked scenario. An AS-1 RBC collected 2 June has an original 42-day outdate of 14 July. Irradiated on 20 June, the 28-day-from-irradiation date is 18 July. The earlier date wins, so the label stays 14 July. If that same unit is irradiated on 10 June instead, 28 days from irradiation is 8 July, which is earlier than 14 July, so the new date is 8 July. Nursing returns an unspiked bag after 25 minutes with an intact temperature indicator in range: restock is defensible under a 30-minute SOP and under AABB temperature logic. The same bag returned after 25 minutes with the spike port opened is not restocked; if it is transfused, the 4-hour infusion clock has already started.

Exam traps. Do not treat 30 minutes as an AABB discard law. Do not give irradiated red cells a flat 28-day date without comparing it to the original outdate. Do not refrigerate thawed cryo. Do not keep thawed FFP labeled as FFP past 24 hours. Do not assign 14 days to an open-system wash.
