MR Personnel Classifications & Supervisory Roles

Key Takeaways

  • Level 1 and Level 2 personnel have different safety training responsibilities.

  • Level 1 personnel cannot admit or supervise non-MR personnel in Zones III/IV.

  • Facility authorization and screening govern access; a hospital badge alone is insufficient.

MR Personnel Classifications & Supervisory Roles

Note

Maintaining clinical safety within magnetic resonance imaging suites depends on clearly defined personnel classifications, well-understood supervisory hierarchies, and specialized institutional leadership roles. Every individual entering the facility must operate within their verified training level to prevent human error from compromising patient and staff safety.

ACR and ARRT Personnel Classifications

The American College of Radiology (ACR) and the American Registry of Radiologic Technologists (ARRT) categorize all individuals within an MR facility into three distinct groups based on documented safety education, clinical competency, and operational authority.

[Non-MR Personnel] ─────────────> [Level 1 MR Personnel] ─────────────> [Level 2 MR Personnel]
(Unscreened / No Training) (Annual Safety Education) (Advanced Bioeffects & Physics)
(Must Be Accompanied) (Self-Preservation in Zone III) (Direct Supervision in Zone III/IV)

Non-MR Personnel

Non-MR Personnel encompasses any individual who has not undergone formal, documented MR safety training within the preceding 12 months. This group represents the overwhelming majority of individuals who encounter the MR suite and carries the greatest risk of inadvertent safety violations.

  • Populations Included: Patients, family members, visitors, and patient caregivers. It also includes general healthcare facility staff from other departments who lack specialized MR safety certification, such as inpatient nurses, certified nursing assistants, phlebotomists, social workers, respiratory therapists, physical therapists, volunteer escorts, hospital security personnel, facility maintenance staff, custodial and environmental services (housekeeping) staff, and non-radiology physicians (surgeons, emergency physicians, hospitalists, anesthesiologists without MR credentialing).
  • Access Privileges and Supervision: Non-MR personnel possess zero independent access rights to Zone III or Zone IV. They may only enter Zone III after passing formal ferromagnetic screening and must be accompanied and directly supervised by Level 2 MR personnel throughout their entire duration in Zone III and Zone IV. Non-MR personnel may never be left unattended in Zone III or Zone IV under any circumstances.

Level 1 MR Personnel

Level 1 MR Personnel are individuals who have successfully completed documented, institutional MR safety training on an annual basis. This training provides sufficient education to ensure their own personal safety and self-preservation while working within the controlled boundaries of Zone III.

  • Educational Scope: Understanding static magnetic field fundamentals, recognizing the "magnet is always on" rule, understanding the site's controlled field boundary, recognizing projectile hazards, adhering to personal screening and gowning standards, and knowing how to respond to emergency alarms.
  • Populations Included: Dedicated MR department receptionists, patient access representatives, imaging service desk clerks, dedicated patient transporters who routinely bring inpatients into Zone III holding bays, and imaging assistants during their initial orientation or foundational training phases.
  • Access Privileges and Limitations: After appropriate screening and facility authorization, Level 1 personnel may be permitted unaccompanied access in Zones III and IV to carry out their assigned job duties (such as greeting screened patients, stocking supplies, or operating administrative workstations). However, Level 1 personnel are strictly prohibited from escorting, supervising, or monitoring Non-MR personnel within Zone IV. They do not possess the advanced bioeffects training required to protect others inside the magnet room.

Level 2 MR Personnel

Level 2 MR Personnel are highly trained healthcare professionals who have completed extensive, comprehensive education in MR safety, electromagnetic biophysics, and suite emergency management. Level 2 personnel serve as the primary clinical and operational gatekeepers of the MR environment.

  • Educational Scope: Advanced knowledge of electromagnetic bioeffects, including:
    • Static magnetic field mechanics: Translational projectile acceleration, rotational torque, and spatial gradient magnetic field displacement forces.
    • Radiofrequency (RF) biophysics: Specific Absorption Rate (SAR), thermal tissue heating, antenna effect resonance, conductive loop burn prevention, and safe surface coil cable placement.
    • Time-varying magnetic gradient bioeffects: Rapid field switching rates (dB/dtdB/dt), acoustic noise mitigation, and peripheral nerve stimulation (PNS).
    • Emergency response: Magnet quench protocols, cryogenic asphyxiation management, rapid Zone IV patient extraction during cardiac arrest, and electrical power shutoff procedures.
  • Populations Included: ARRT-certified and registered MR technologists (R.T.(MR)), licensed diagnostic radiologists, MR physicists, and designated MR Safety Officers.
  • Access Privileges and Authority: Level 2 personnel possess complete, independent access to Zone III and Zone IV. They hold the legal and clinical authority to conduct comprehensive patient screening, grant final clearance for entry into Zone III and Zone IV, accompany and directly supervise Non-MR personnel in Zone III and Zone IV, but the Level 2 designation alone does not authorize clinical scanner operation.
