ASTM F2503 Medical Device Labeling & Compatibility

Key Takeaways

  • ASTM International F2503 permanently replaced ambiguous legacy terms like 'MR compatible' with three definitive categories: MR Safe, MR Conditional, and MR Unsafe.

  • The MR Safe icon is a solid green square containing white letters 'MR', identifying devices fabricated entirely from non-metallic, non-conductive, and non-magnetic materials that present zero hazard in any MR environment.

  • The MR Conditional icon is a yellow equilateral triangle with a black border and black letters 'MR', indicating safety only when every manufacturer-defined condition (field strength, spatial gradient, SAR, coils) is strictly satisfied.

  • A medical device demonstrated to be MR Conditional at 3.0 Tesla is not automatically safe at 1.5 Tesla; each static magnetic field strength represents a distinct electromagnetic environment requiring explicit independent certification.

  • The MR Unsafe icon is a solid red circle with a diagonal slash over black letters 'MR', denoting items that present direct, life-threatening hazards (missile projectile, severe heating, or device destruction) in Zone IV.

ASTM F2503 Medical Device Labeling & Compatibility

Note

The clinical magnetic resonance environment combines a high-strength static magnetic field, rapidly pulsing radiofrequency (RF) energy, and switched magnetic field gradients. Medical devices and surgical implants introduced into this multi-energy field experience complex mechanical, thermal, and electrical interactions. Standardized, unambiguous device labeling is the primary operational defense against catastrophic patient injury.

The Evolution of Medical Device Safety: ASTM International F2503

Historically, the medical imaging community relied on colloquial and poorly regulated terminology to describe how implants behaved in the magnetic field. Phrases such as "MR Safe" and "MR Compatible" were used interchangeably across clinical literature and manufacturer brochures.

By the mid-1990s, the Food and Drug Administration (FDA) and safety organizations recognized that the term "MR Compatible" was dangerously ambiguous. In clinical practice, "compatible" could imply that an implant would not harm the patient, or it could mean that the scanner would not corrupt the device's microcircuitry, or merely that the implant produced minimal image artifact. The term failed to convey whether an implant had been tested at 0.5 Tesla, 1.5 Tesla, or under specific RF absorption limits. Following a series of severe patient injuries and near-miss projectile events, the FDA Center for Devices and Radiological Health (CDRH) partnered with ASTM International (formerly the American Society for Testing and Materials) to formulate an objective, standardized classification system.

In 2005, ASTM International published ASTM F2503: Standard Practice for Marking Medical Devices and Other Items for Safety in the Magnetic Resonance Environment. This standard formally retired "MR Compatible" from official medical labeling and established three mutually exclusive classifications, each tied to specific visual iconography, geometric shapes, and international color-coding standards.

Standardized Device Classifications and Visual Iconography

The ASTM F2503 standard mandates that every medical device, ancillary accessory, and piece of patient care equipment intended for use in an imaging facility must fall into one of three standardized categories.

1. MR Safe (Solid Green Square)

An item that poses no known hazards resulting from exposure to any MR environment.

  • Physical Criteria: To achieve an MR Safe designation, an item must be completely non-magnetic, non-metallic, and non-electrically conductive. It cannot contain any trace of iron, nickel, cobalt, titanium, aluminum, copper, carbon-fiber composites, or electrically conductive polymers.
  • Environmental Freedom: MR Safe items may be introduced into any MR scanner room (Zone IV) at any magnetic field strength (e.g., 0.5T, 1.5T, 3.0T, 7.0T or higher), under any spatial gradient, and during any pulse sequence without restriction.
  • Clinical Examples: Pure high-density polymers (polyethylene, polypropylene), pure silicone catheters without wire reinforcement, molded medical-grade plastics, porcelain, pure cotton linens, and borosilicate glass containers.

Important

An item containing even a microscopic or non-ferromagnetic metal component (such as a tiny titanium spring or brass rivet) can never be labeled MR Safe under ASTM F2503. It must instead be evaluated and classified as MR Conditional.

2. MR Conditional (Yellow Equilateral Triangle)

An item with demonstrated safety in the MR environment within defined conditions. At a minimum, address the conditions of the static magnetic field, the switched gradient magnetic field and the radiofrequency fields. Additional conditions, including specific configurations of the item, may be required.

  • Physical Criteria: These devices frequently contain metallic components (e.g., titanium alloys, stainless steels, nitinol, platinum-iridium, or cobalt-chromium) or sensitive microelectronics. While they do not pose an uncontrolled missile hazard, they present conditional risks such as RF-induced thermal heating, rotational torque, electrical current induction, or operational disruption.
  • The Conditional Mandate: An MR Conditional device is safe ONLY when all manufacturer-specified operating conditions are verified and adhered to. If a condition is unmet or unknown, stop and escalate; the proposed configuration is not covered by the labeling. This does not automatically reclassify the device as MR Unsafe. Any consideration outside labeling requires the responsible MR physician's documented risk assessment under the site policy, not assistant clearance.
  • Clinical Examples: Orthopedic spinal fixation hardware, coronary stents, prosthetic heart valves, modern MR Conditional cardiac pacemakers, neurostimulation pulse generators, and specialized monitoring transducers.

3. MR Unsafe (Red Circle with Diagonal Slash)

An item which poses unacceptable risks to the patient, medical staff or other persons within the MR environment.

  • Physical Criteria: Items that are strongly ferromagnetic, electrically hazardous, or subject to violent mechanical translation and torque in Zone IV.
  • Clinical Impact: These items are strictly forbidden from entering Zone IV under any circumstances during routine operations. When brought near the bore opening, they become lethal high-velocity projectiles or induce catastrophic electrical burns.
  • Clinical Examples: Ferromagnetic steel oxygen tanks, standard hospital stretchers and wheelchairs, carbon-steel scissors, hemostats, utility wrenches, clipboard clips, floor buffers, conventional defibrillators, and legacy cardiac pacemakers.
