1Under Article 20, Part 1 of Federal Law No. 323-FZ, what must a medical worker give the patient, in an accessible form, before the patient's informed voluntary consent (информированное добровольное согласие) to a medical intervention is valid?
A.The name of the procedure, the date it is scheduled and the name of the doctor performing it
B.A summary of the patient's rights and duties under the compulsory medical insurance contract
C.The aims and methods of care, the risks, the possible options, their consequences and expected results
D.A copy of the relevant clinical guideline and the organization's price list for paid services
Explanation: Article 20, Part 1 makes informed voluntary consent a necessary precondition of any medical intervention. The consent is valid only if the medical worker has first given complete information, in an accessible form, about the aims and methods of care, the related risk, the possible options, their consequences and the expected results.
2For ordinary medical care outside the statutory exceptions, what age threshold does Article 54, Part 2 of Federal Law No. 323-FZ set for a minor without drug addiction to give or refuse informed voluntary consent?
A.Older than 15 years
B.Older than 14 years
C.Older than 16 years
D.Only from 18 years, the age of full legal capacity
Explanation: Article 54, Part 2 gives minors older than 15 years the right to give or refuse informed voluntary consent themselves. For minors with drug addiction the threshold is older than 16 years. Below these ages a parent or other legal representative consents under Article 20, Part 2.
3An unconscious adult with a life-threatening haemorrhage is brought to a hospital, and no legal representative can be reached. Under Article 20, Part 9 of Federal Law No. 323-FZ, may doctors operate without consent?
A.No; the operation must wait until a court authorises an intervention without consent
B.No; consent must first be obtained by telephone from the patient's adult children
C.Only after the Medical Commission meets in full session and approves the operation
D.Yes, because there is an emergency threat to life and the patient cannot express their will
Explanation: Article 20, Part 9, Item 1 allows a medical intervention without consent when it is needed on emergency grounds to remove a threat to life and the person's condition does not allow them to express their will. Under Part 10, the decision is taken by a council of doctors or, if one cannot be convened, by the attending (duty) doctor.
4Under Article 20, Part 10 of Federal Law No. 323-FZ, who decides on an emergency intervention without consent for a patient whose condition prevents them from expressing their will?
A.The head of the medical organization alone, acting on a written request submitted by the attending doctor
B.A council of doctors or, if none can be convened, the attending (duty) doctor, with later notification
C.The territorial body of Roszdravnadzor, by an urgent written order sent to the hospital
D.The patient's insurance company, which must first confirm that the intervention will be paid
Explanation: For emergency cases under Part 9, Items 1 and 2, the decision is taken by a council of doctors (консилиум врачей). If it is impossible to convene one, the attending or duty doctor decides, enters the decision in the medical record and then notifies the head of the organization or department and the patient or legal representative.
5An ambulance team arrives at a street scene where a man has collapsed with a life-threatening arrhythmia. Before care began he had not refused treatment. Under the 2023 amendment to Article 20 of Federal Law No. 323-FZ, who decides on an intervention without his consent?
A.The senior doctor of the ambulance dispatch centre, who authorises the team by radio
B.The medical worker of the ambulance team, who records the decision in the medical documentation
C.A council of at least three doctors, convened at the scene before treatment begins
D.A police officer at the scene, who signs a written authorisation for the treatment
Explanation: Item 1.1 of Part 9 (added by Law No. 678-FZ of 25.12.2023) allows intervention without consent when ambulance care is given outside a medical organization, the intervention is needed to remove a threat to life and no refusal was expressed before care began. Part 10, Item 4 assigns the decision to the medical worker of the mobile ambulance team, who records it in the medical documentation.
6The parents of a 10-year-old refuse an operation that is needed to save the child's life. Under Article 20 of Federal Law No. 323-FZ, what may the medical organization do?
A.Operate immediately, because parental refusal has no legal effect for children
B.Accept the refusal as final and discharge the child
C.Ask the regional ministry of health to cancel the parents' rights by an urgent administrative order
D.Explain the consequences of refusal and apply to a court to protect the child's interests
Explanation: Under Part 4, the possible consequences of refusal must be explained in an accessible form. Under Part 5, when a parent or other legal representative refuses an intervention necessary to save the life of a person listed in Part 2, the medical organization has the right to go to court to protect that person's interests.
7Under Article 13, Part 1 of Federal Law No. 323-FZ, which information makes up medical confidentiality (врачебная тайна)?
A.The fact of seeking care, the state of health, the diagnosis and other data from examination and treatment
B.Only the diagnosis and the treatment plan, but not the fact that the citizen sought care
C.Only information about psychiatric disorders, HIV infection and sexually transmitted infections
D.Only information recorded after the patient has signed a separate confidentiality agreement

8In which situation does Article 13, Part 4 of Federal Law No. 323-FZ allow information that is a medical secret to be provided without the consent of the citizen or their legal representative?
A.When the patient's employer asks in writing why the employee was absent from work
B.When a bank requests health data in connection with a mortgage or loan application
C.When there is a threat of the spread of infectious diseases, mass poisonings or injuries
D.When a journalist investigating a complaint about the hospital asks in writing for the case details
Explanation: Part 4 lists the exceptions to consent. They include examination and treatment of a person unable to express their will, a threat of the spread of infectious diseases, mass poisonings and injuries, and requests from bodies of inquiry and investigation, a court or a prosecutor in connection with proceedings or supervision.
9After a patient's death, their adult daughter asks the hospital about the cause of illness. The patient had not forbidden disclosure. Under Article 13, Part 3.1 of Federal Law No. 323-FZ, how should the hospital respond?
A.Refuse, because medical secrecy can never be lifted after the patient's death
B.Provide the information only after a court order obtained by the daughter
C.Refuse unless the daughter was named in the patient's written consent form
D.Disclose it on her request, as the patient did not forbid disclosure to close relatives
Explanation: Part 3.1 allows disclosure after a citizen's death to the spouse, close relatives (children, parents, adopted children and adoptive parents, siblings, grandchildren, grandparents) or persons named in a written consent, on their request, unless the citizen or their legal representative forbade it.
10Under Article 22, Part 3 of Federal Law No. 323-FZ, how must information about an unfavourable prognosis be handled?
A.It must always be given to the patient in full, even if the patient does not want to know
B.It is told delicately to the patient, or to the spouse or a close relative unless the patient forbade it
C.It may be given only to the head of the medical organization, who decides on further disclosure
D.It is recorded in the medical file but never communicated to the patient or to relatives
Explanation: Part 3 states that health information may not be given to a patient against their will. Russia Foreign Medical Accreditation — Preliminary Stage: Suggested Study Strategy

1Memorize the key timelines in Federal Law No. 59-FZ: citizens' appeals must be registered within 3 days and resolved within 30 days of registration, with an exceptional 30-day extension allowed with notice.
2Understand the boundaries of physician authority under Order No. 1089n: an attending physician can independently issue and extend a sick-leave certificate for up to 15 calendar days; beyond 15 days, extension requires the Medical Commission (ВК).
3Study the dispanserization rules in Order No. 404n: adults aged 18–39 are examined every 3 years; adults aged 40 and older are examined annually. Master the age brackets for mammography (40–75 biennially) and fecal occult blood testing (40–64 biennially, 65–75 annually).
4Read Section I (General Provisions) of Government Decree No. 2188, the only part of the State Guarantees Programme on the official source list: what the Programme establishes, who approves territorial programmes, the ban on moving ambulance-care OMS funds, and priority wage indexation.
5Review informed voluntary consent (ИДС) under Article 20 of Federal Law No. 323-FZ: exceptions where care can be provided without consent (emergency threats to life when patient cannot express will, dangerous diseases, severe psychiatric disorders, forensic examination).

Frequently Asked Questions

What is the Preliminary Stage of Medical Accreditation in Russia?

It is a mandatory computer-based test established by Minzdrav Order No. 709n (paras. 84–87) for people who obtained medical, pharmaceutical or other education abroad. It is held before the usual primary or primary specialized accreditation stages and is based on nine official sources: Federal Laws 323-FZ, 326-FZ and 59-FZ, Minzdrav Orders 180n, 203n, 404n, 785n and 1089n, and the general provisions of Government Decree No. 2188.

What happens after passing the preliminary stage?

Passing the preliminary stage (≥70%) entitles the foreign-trained practitioner to proceed to the regular stages of primary accreditation (PA) or primary specialized accreditation (PSA) in their specific medical specialty (e.g. general medicine, pediatrics, surgery, dentistry, or pharmacy).

What is the test format, timing, and passing score?

Since 01.06.2026, each variant has 80 items in 60 minutes: single-answer items, multiple-answer items and situational judgement items. The result is 'passed' at 70% or more correct answers (56 of 80). The Methodological Center states that no rehearsal exam is provided for this stage.

Is there a fee for taking the preliminary stage?

No. Under Russian legislation, state accreditation of healthcare specialists is completely free of charge.

In what language is the official examination conducted?

The preliminary stage is held in Russian (Order No. 709n, para. 86), so candidates need Russian legal and medical terminology. This bank is an independent English-language MCQ study adaptation: it uses single-best-answer questions only, is not an official translation, and does not simulate the official multiple-answer or situational judgement items.
