NHPC's BPT licensing exam is a computer-based test of 100 four-option MCQs with a pass mark of 50/100 and no negative marking. The 19th sitting (Asoj 2083) used 90-minute sessions. Nine syllabus areas carry 10% each, and biomechanics and the combined microbiology/pathology/biochemistry/pharmacology area carry 5% each.

1Which rotator cuff muscle initiates the first 15° or so of shoulder abduction before the deltoid takes over?
A.Infraspinatus
B.Teres minor
C.Supraspinatus
D.Subscapularis
Explanation: Supraspinatus (suprascapular nerve, C5-C6) starts abduction and compresses the humeral head into the glenoid so that the deltoid can lift the arm efficiently. It is also the cuff tendon most often torn.
2A mid-shaft humeral fracture involves the spiral (radial) groove. Which nerve injury and deficit are most likely?
A.Radial nerve, causing wrist drop
B.Median nerve, causing ape-hand deformity
C.Ulnar nerve, causing claw hand
D.Axillary nerve, causing loss of abduction
Explanation: The radial nerve winds around the back of the humerus in the spiral groove, so shaft fractures can injure it. Loss of the wrist and finger extensors produces wrist drop, while triceps is usually spared because its branches arise higher.
3Which knee ligament is the primary restraint to anterior translation of the tibia on the femur?
A.Posterior cruciate ligament
B.Medial collateral ligament
C.Lateral collateral ligament
D.Anterior cruciate ligament
Explanation: The ACL runs from the anterior tibia to the lateral femoral condyle and resists anterior tibial translation and rotation. The Lachman test assesses it clinically.
4In the femoral triangle, what is the order of the main structures from lateral to medial?
A.Femoral artery, nerve, vein, canal
B.Femoral nerve, artery, vein, canal
C.Femoral canal, vein, artery, nerve
D.Femoral vein, artery, nerve, canal
Explanation: From lateral to medial the order is nerve, artery, vein and the femoral canal containing lymphatics (NAVEL). The pulse of the femoral artery lies just below the mid-inguinal point.
5Which cranial nerve carries parasympathetic supply to the heart, bronchi and most of the gut?
A.Glossopharyngeal nerve (IX)
B.Accessory nerve (XI)
C.Vagus nerve (X)
D.Hypoglossal nerve (XII)
Explanation: The vagus nerve supplies parasympathetic fibres to the thoracic and abdominal viscera as far as the left colic (splenic) flexure. Vagal activity slows the heart and constricts the bronchi.
6Besides the median nerve, what passes through the carpal tunnel?
A.Flexor carpi radialis and the ulnar artery
B.Extensor pollicis longus and the radial artery
C.Abductor pollicis longus and extensor pollicis brevis
D.Nine flexor tendons (four FDS, four FDP and FPL)
Explanation: The carpal tunnel contains the median nerve and nine flexor tendons. Swelling of the tendon sheaths or other space-occupying changes compress the nerve, giving carpal tunnel syndrome.
7Scapular winging after a long thoracic nerve palsy reflects the loss of which serratus anterior action?
A.Protraction and upward rotation of the scapula
B.Retraction and downward rotation of the scapula
C.Elevation and anterior tilt of the scapula
D.Depression of the scapula only
Explanation: Serratus anterior holds the scapula against the chest wall and protracts and upwardly rotates it during arm elevation. When it is paralysed, the medial border lifts away from the thorax, especially when pushing against a wall.
8Where is the primary motor cortex located?
A.Postcentral gyrus of the parietal lobe
B.Precentral gyrus of the frontal lobe
C.Superior temporal gyrus
D.Occipital lobe around the calcarine sulcus
Explanation: The primary motor cortex (Brodmann area 4) lies in the precentral gyrus and is arranged as a motor homunculus. A stroke affecting it causes contralateral weakness in the corresponding body region.
9Which structure is the main static restraint to valgus stress at the elbow during overhead throwing?
A.Posterior bundle of the ulnar collateral ligament
B.Radial collateral ligament
C.Anterior bundle of the ulnar collateral ligament
D.Annular ligament
Explanation: The anterior bundle of the ulnar (medial) collateral ligament resists valgus load through most of the arc of elbow flexion used in throwing. Repeated throwing can stretch or tear it.
10A patient has burning numbness over the anterolateral thigh with normal strength and reflexes. Which nerve is most likely compressed?
A.Femoral nerve (L2-L4)
B.Obturator nerve (L2-L4)
C.Genitofemoral nerve (femoral branch)
D.Lateral femoral cutaneous nerve
Explanation: The lateral femoral cutaneous nerve (L2-L3) is purely sensory and can be trapped under the inguinal ligament near the ASIS, causing meralgia paraesthetica. Tight belts, obesity and pregnancy are common triggers.

About the NHPC BPT Exam

The NHPC BPT Licensing Examination is the Nepal Health Professional Council's Name Registration Certificate Examination for Bachelor of Physiotherapy graduates; passing it is required for registration to practise physiotherapy in Nepal. This free practice bank follows the eleven areas and weights of the official NHPC licensing syllabus.

Exam sponsor: Nepal Health Professional Council. The requirements and fees below concern the certification or admission exam, separate from our free practice resources.

Assessment

Computer-based exam of 100 four-option MCQs (full marks 100, pass marks 50) with no negative marking. NHPC BPT: Suggested Study Strategy

1The syllabus is spread evenly: nine of the eleven areas carry 10 marks each, so avoid over-investing in your favourite specialty.
2Revise the burn, plastic surgery and gynaecology area (10%), which is often under-prepared: scar management, pelvic floor training and antenatal/postnatal care.
3Know outcome scales and clinical tests by name and purpose, such as GCS, ASIA, Ashworth, the Berg Balance Scale and special orthopaedic tests.
4Link electrotherapy parameters (frequency, pulse duration, polarity) to their physiological effects rather than memorising settings.
