Replacement before the frequency limit requires documentation of medical necessity (size change, damage).
What is WHCRA and how does it affect coverage?
WHCRA (Women's Health and Cancer Rights Act of 1998) requires group health plans + insurers covering mastectomy to ALSO cover: reconstruction of the affected breast, surgery on the contralateral breast for symmetry, prostheses, and treatment of physical complications including lymphedema. WHCRA does NOT change Medicare DMEPOS rules but ensures private insurance coverage. Medicaid coverage varies by state.
Can a patient receive a prosthesis without reconstruction?
Yes — patient choice. Many women choose to NOT pursue reconstruction (the "going flat" option) and instead use external prostheses, or no prosthesis at all. The CMF must respect the patient choice. External prostheses are valid long-term solutions: silicone weighted (gold standard for posture/balance) or lightweight options.
