When do I use 90460/90461 vs 90471/90472?

Use 90460 (each first vaccine/toxoid component admin) and 90461 (each addt'l component) when admin is to a patient UNDER 19 AND counseling is provided by physician/QHP. Use 90471/90472 when patient is 19+ OR no counseling for <19. For combination vaccines, count immunizing components: e.g., MMR (3 components) under 19 with counseling = 90460 + 90461×2; without counseling = 90471 + 90472×2.

When can perinatal P-codes be used?

Per ICD-10-CM Official Guidelines, perinatal Chapter 16 codes (P00-P96) are intended for conditions originating in the perinatal period — they may be used ONLY through the first 28 days of life. After 28 days, the underlying condition codes (chapter-specific) take over even if related to perinatal origin. For history of perinatal condition affecting current care, use Z87.7 personal history.
