When are Apgar scores assigned, and what must they never be used for?
At 1 and 5 minutes, then every 5 minutes through 20 minutes whenever the 5-minute score is under 7. The score documents transition and response to resuscitation; it never delays or dictates resuscitation, which follows respirations, heart rate, and oxygenation.
How many calories are in mature human milk, and why is it fortified for preterm infants?

How many questions are on the RNC-LRN exam?
The 2026 NCC Candidate Guide lists 175 multiple-choice items in a 3-hour test. Of those, 150 are scored and 25 are unscored pretest items used to gather statistics for future exams. Every question has one stem and three answer options: one correct answer and two distractors.
What is the RNC-LRN pass rate?
NCC publishes annual exam results. For Low Risk Neonatal Intensive Care Nursing, 201 candidates tested in 2025 and 54% passed; in 2024, 377 candidates tested and 67% passed. From 2020 through 2023 the pass rate ranged from 70% to 82%.
What score do I need to pass the RNC-LRN exam?
NCC exams are criterion-referenced and there is no set percentage passing level. An ability level is calculated from the number of correct answers, with no penalty for wrong answers, and compared with a predetermined pass point. Results are reported as pass/fail plus word descriptors (very weak through very strong) by content area.
Who is eligible to take the RNC-LRN exam?
Candidates need current, active, unencumbered RN licensure in the US or Canada, 24 months of specialty experience as an RN comprising at least 2,000 hours, and employment in the specialty sometime in the last 24 months. Both the practice time and the hours must be met; it is not an either/or criterion.
What happens if I fail the RNC-LRN exam?
Candidates must wait 45 days from the date the exam was taken before submitting a new application, and must pay the full $325 application and testing fees again. There is no lifetime limit on attempts, but the same NCC exam can be taken only twice in a calendar year.
How long does RNC-LRN certification last?
NCC awards certification for three years. Certification is maintained by completing the Continuing Competency Assessment, earning the continuing education specified in the resulting education plan after taking the assessment, and submitting a maintenance application with fees before the maintenance due date.
