- Atypical Agents: Olanzapine, Risperidone, Quetiapine, Aripiprazole, Ziprasidone, Clozapine.
- Mechanism & Benefits: Dual serotonin ($5-HT_{2A}$) and dopamine ($D_2$) antagonism; low risk of EPS and tardive dyskinesia; effective for both positive and negative symptoms.
- Metabolic Syndrome Hazard: Second-generation agents carry high risks of significant weight gain, hyperlipidemia, and new-onset Type 2 diabetes mellitus (highest risk with Olanzapine and Clozapine; lowest risk with Aripiprazole and Ziprasidone). Requires baseline and routine monitoring of BMI, waist circumference, fasting lipid panels, and fasting blood glucose / HbA1c.
- The Clozapine Protocol & Agranulocytosis Mandate:
- Clinical Indication: Indicated exclusively for severe, treatment-resistant schizophrenia (failure of at least two prior antipsychotic trials) or persistent recurrent suicidal behavior in schizophrenia.
- Black-Box Hazard: Agranulocytosis (severe, potentially fatal depletion of absolute neutrophils, leaving the patient completely vulnerable to fulminant sepsis).
- Mandatory Blood Monitoring: Participation in the mandatory Clozapine REMS monitoring registry. Baseline Absolute Neutrophil Count (ANC) must be >= 1,500/mm³ (>= 1,000/mm³ for Benign Ethnic Neutropenia [BEN]) prior to treatment initiation. ANC is monitored weekly for the first 6 months, biweekly for months 6 to 12, and monthly thereafter for the duration of therapy.
