The Two Documents Required for the Certificate

Consent and Release of Liability Form

This form authorizes the OACP and TNT Justice Consultants to release your assessment results directly to the police services you apply to; you never see your own raw scores. It must be:

• Signed with either a handwritten or an electronic signature and returned as instructed in your Part A email - OACP now has applicants sign and submit the consent form and code of conduct electronically
• Submitted as a PDF document with all pages attached; a partial submission stalls the file

Applicant Code of Conduct

This form sets out the academic-integrity rules you agree to follow while testing - for example, the prohibition on writing down full SSPO questions to share with other applicants. Like the Consent and Release form, it must be signed and returned as a PDF.

What Slows a Part A Submission Down

• Sending only some pages of a form instead of the complete signed PDF
• Sending documents OACP did not expressly request - OACP states that unrequested additional documents are deleted from its servers immediately, so they do nothing except add confusion to your file
• Working from a form copy you found elsewhere online rather than the one attached to your official Part A email

The Medical Examination Package

The medical package is not required to receive your OACP Certificate - you complete it during the Part A window but deliver the finished forms directly to the police service(s) you apply to, alongside your certificate.

Vision Component

• Completed by your own optometrist or ophthalmologist, located in Canada
• Minimum standard: 20/40 uncorrected vision, or 20/20 corrected, with both eyes open
• Applicants who have had refractive eye surgery must wait the required post-surgery period - at least 30 days, and additional documentation is needed if the surgery occurred within 6 months of the exam - before the results count
• Valid for 2 years from the date the practitioner signs it

Hearing Component

• Completed by an audiologist or hearing instrument specialist, located in Canada
• Uses a three-stage process. Stage 1 is a pure-tone audiogram: thresholds must not exceed 25 dB HL in each ear at 500, 1000, 2000, 3000, and 4000 Hz. Clear Stage 1 and you are done - no further stages are required
• Stage 2 is the headphone QuickSIN speech-in-noise test, taken at the same clinic only if you do not clear Stage 1; the criterion is an SNR loss no greater than 3 dB in each ear, and one immediate retest is allowed during the same visit

Who Cannot Sign It

Family doctors and military doctors are explicitly excluded - only a qualifying optometrist/ophthalmologist and a separate hearing specialist can complete the forms, and both must practice in Canada.
